ObjectivesThe Brazilian constitution guarantees the right to health, including access to medicines. In May 2004, Brazil’s government announced the “Farmácia Popular” Program (FPP) as a new mechanism to improve the Brazilian population’s access to medicines. Under FPP, a selected list of medicines is subsidized by the government and provided in public and private pharmacies.The aim of this study is to describe the historical stages of the FPP and to identify associated changes in the geographical accessibility of medicines through the FPP over time.MethodsIt was performed documentary review and an ecological study assessing program coverage in terms of number of facilities and a FPP Pharmacy Facilities Density (PFD) index at national and regional levels from 2004 to 2013, using the FPP database. We used geographic information system mapping to depict a pharmaceutical facilities density (PFD) index at the municipality level on thematic maps.ResultsA growth of the PFD index coincident with the phases of the FPP was noticed. In the public sector, the program started in 2004; by 2006, there was a sharp increase in the numbers of participating pharmacies, stabilizing in 2009. In the private sector, the program started in 2006; by 2009 the PFD ratio had increased substantially and it continued to grow through 2011. There was an increase in FPP coverage in most regions between 2006, when the private pharmacy component started, and 2013, but participating pharmacies remain unequally distributed across geographical regions. Specifically, the wealthy areas in the South and Southeast have higher coverage, with lower coverage mostly in the North and Northeast, relatively poorer areas with greater need for access to medicines, health care, and other basic services such as potable water and sanitization.ConclusionsThere has been a substantial increase in the number of pharmacies participating in the FPP over time. This has led to greater program coverage and has potentially improved access to FPP medicines in the country. Resumo O Pleno do Conselho Nacional de Saúde (CNS) aprovou a Resolução nº 568/2017 que convoca a 16ª Conferência Nacional de Saúde, a ser realizada no ano de 2019 e deliberou pela realização de atividades temáticas, em oito áreas, incluindo Assistência Farmacêutica e Ciência e Tecnologia. Uma parceria do CNS com a Fiocruz e a Escola Nacional dos Farmacêuticos propôs a realização do 8º Simpósio Nacional de Ciência e Tecnologia e Assistência Farmacêutica, antecedido por dez encontros regionais preparatórios para o simpósio em todo o país. O objetivo deste artigo é apresentar e analisar os resultados dessa primeira etapa de encontros. Foi desenvolvida uma metodologia participativa para os encontros que inclui a apresentação de situações-problema em forma de “casos” com narrativas reais, fictícias ou adaptadas da realidade. Debates em grupos e construção de propostas, compilação e ponderação, com discussão geral completaram os encontros. As 150 propostas oriundas de cinco encontros foram lidas por cada membro da equipe de análise e pré-categorizado. Esses encontros reuniram 238 participantes. Quatro categorias foram definidas: Saúde como direito; Consolidação dos princípios do SUS; Financiamento adequado e suficiente para o SUS; Democracia participativa.
A região de Itacaré, no sul da Bahia possui grande parte das suas trilhas, praias, lagoas e rios situados na Área de Proteção Ambiental (APA) Itacaré Serra Grande. Nessa região encontram-se inúmeras pequenas propriedades rurais que perderam sua vocação agrícola com a crise cacaueira nos anos de 1990 e com as restrições consequentes da instituição da APA. Tais propriedades abrigam florestas de grande biodiversidade, que precisam ser preservadas das ações de caçadores, de extrações ilegais de madeira e dos impactos negativos de um turismo predatório. O trabalho de campo desenvolvido neste estudo objetivou realizar um diagnóstico para avaliar a possibilidade da criação de um fundo para Pagamento de Serviços Ambientais (PSA) tendo o trade turístico como potencial doador e pequenos proprietários rurais vinculados a projetos socioambientais já em desenvolvimento, como beneficiários do mecanismo de pagamento. Foram entrevistados proprietários rurais para calcular a renda agrária de cada propriedade e verificar o custo de oportunidade do uso da terra. A partir do custo de oportunidade seria possível estabelecer o valor do pagamento e o montante necessário para a composição do fundo. Com a finalidade de identificar a disponibilidade dos empreendedores locais a contribuir com o fundo, foram aplicados questionários de Valoração Contingente aos empreendedores turísticos locais. Tais questionários continham três tipos de questões: abertas, para ordenação (ranking) e de alternativas. Na intenção de articular a necessidade de manter a riqueza biológica, o potencial turístico da região e melhorar a qualidade de vida dos proprietários rurais, este trabalho analisou, também, a disponibilidade de contribuição dos turistas e do trade turístico da região para a implantação do projeto de PSA. Conclui-se que, é viável a implementação de um fundo para pagamento de serviços ambientais devido a disponibilidade para contribuição por parte dos empreendedores e turistas. No entanto, como hipótese derivada do estudo, entendese que este mecanismo não é suficiente para garantir a sustentabilidade ambiental da região, necessitando de ações complementares com o desenvolvimento de políticas públicas voltadas para o setor.
