A inserção de equipes de saúde bucal no Programa Saúde da Família no Estado de Minas Gerais
Objective: To evaluate the association of breastfeeding duration, pacifier use and nasal air flow with occlusal disorders among children. Methods: This cross-sectional observational study included 138 children aged 4 and 5 years selected in all the daycare centers of the city of Campo Limpo Paulista, Brazil. Questionnaires were applied to mothers to identify total duration of exclusive breastfeeding and non-nutritive sucking. The independent variables were: Sucking (pacifier, bottle, finger), breastfeeding duration and nasal air flow. The dependent variables were: Open bite, crossbite, overjet, overbite, diastema and maxillary deficiency. Frequency distribution calculations were performed, a chi-square test, the Fisher exact test and, after that, stepwise logistic regression were used for statistical analysis, and the level of significance was set at 5%. Results: The use of a pacifier was the most deleterious factor and increased chances of having open bite in 33.3 times, marked overjet in 2.77 times and posterior crossbite in 5.26 times. Conclusion: There was a significant association between non-nutritive sucking, particularly the use of a pacifier, and occlusal disorders. These findings are important to plan the treatment of preschoolers.Keywords: Breastfeeding. Malocclusion. Primary dentition. Objetivo: avaliar a associação do tempo de amamentação natural, tempo de uso de chupeta e aeração nasal em relação a problemas oclusais em crianças. Métodos: foi realizado um estudo observacional transversal com universo de 138 crianças entre 4 e 5 anos, de creches de Campo Limpo Paulista/SP. Foram aplicados questionários às mães com o intuito de identificar o tempo de amamentação natural total e exclusiva e hábitos deletérios. Utilizaram-se as variáveis independentes hábitos deletérios (chupeta, mamadeira e dedo), tempo de amamentação natural e aeração nasal; e as variáveis dependentes mordida aberta, mordida cruzada, overjet, sobremordida, presença de diastema e atresia. Foram realizados cálculos das distribuições das frequências, teste de associação do qui-quadrado, exato de Fisher e, em seguida, regressão logística pelo método de stepwise, utilizando nível de significância de 5%. Resultados: o uso de chupeta foi considerado o mais prejudicial dos hábitos deletérios, aumentando em 33,3 vezes a chance de mordida aberta; 2,77 vezes a chance de apresentar overjet acentuado e 5,26 vezes as chances de apresentar mordida cruzada posterior. Conclusão: houve uma forte associação entre hábitos deletérios (especialmente o tempo de uso de chupeta) e problemas oclusais, sendo esse fato importante no planejamento do tratamento de pacientes pré-escolares.Palavras-chave: Aleitamento materno. This study aimed to evaluate the impact of dental caries treatment on oral health-related quality of life (OHRQoL) among schoolchildren and the responsiveness of the Child Perceptions Questionnaire (CPQ8-10 ) instrument. Brazilian schoolchildren, 8-10 yr of age, were randomly selected and assigned to two groups--dental caries treatment (DCT) and caries-free (CF)--according to their caries experience [decayed, missing, or filled primary teeth (dmft) and decayed, missing or filled secondary teeth (DMFT) values of ≥ 0]. The CPQ8-10 instrument was administered at baseline and at 4 wk of follow-up (i.e. 4 wk after completion of dental treatment). In the DCT group, increases in CPQ8-10 scores were observed between the baseline and follow-up results. However, longitudinal evaluation of the CF group demonstrated no statistically significant difference in CPQ8-10 scores. Responsiveness of the CPQ8-10 instrument (magnitude of change in CPQ8-10 scores) in the DCT group was greater (effect size >0.7) than in the CF group. The findings of this study show that dental caries treatment has an important impact on OHRQoL of children. The CPQ8-10 was considered an acceptable instrument for longitudinal measurement of changes in OHRQoL.
The goal of this study was to evaluate a 15-month educational program designed to children. The sample consisted of 60 six-year olds, randomly assigned into control and experimental group. The control consisted of tooth brushing training, once a year. The experimental group received intensive individual tooth brushing training every three months and guidance on oral health. Initially, both groups were assessed using plaque, gingival, dmfs and DMF-S indexes every three months. In the control, no statistically significant difference was observed for plaque and gingival indexes. The experimental group showed a statistically significant reduction in mean values for two indexes. The caries indexes showed no statistically significant difference. The proposed educational program developed was efficient in reducing gingival and plaque indexes as well caries incidence.
Self-perception of oral health in non-institutionalised elderly of Piracicaba city, BrazilObjectives: To associate the self-perception of oral health with sociodemographic, clinical, quality of life and geriatric depression, evaluating what influence in the self-perception of the elderly and the importance to the oral health. Background: The current demographic transition and poor oral health of the elderly deserves particular attention, especially the impact of oral health on the quality of life. Methods: In this cross-sectional study, we evaluated 371 elderly, aged 60 years or more, adscript to Health Family Units, Piracicaba city, Brazil. Clinically, the indexes decayed missing filled teeth (DMFT), CPI and Use/Need of Prosthesis (WHO/99) were used and evaluated by means of instruments such as the selfperceived oral health [geriatric oral health assessment index (GOHAI)], quality of life (SF-36) and geriatric depression (Geriatric Depression Scale). The statistical analyses were carried out by means of univariate analysis of chi-square and Fisher's exact (a = 0.05) and multiple logistic regression analysis. Results: The mean age was 67.35 (2.8), with a mean DMFT of 28.5 (4.8) and a positive GOHAI score (>30). Most were women (63.3%), between 60 and 70 years (72.2%), married (60.4%), had just the elementary school (75.5%) education. Around 80% used some removable prosthetic. The results were associated with characteristics of self-perception of oral health and were significative of schooling (OR = 2.46) and the emotional aspects of quality of life (OR = 0.30). Over 20% of the sample had traits of depression, and these results were statistically significant (OR = 1.65) when associated with the GOHAI scores. Conclusion: Educational and emotional aspects and geriatric depression are factors that influence the positive self-perception of oral health of elderly people of Piracicaba USFs. These results represent that education, feelings and behaviours deserve special attention in their oral health, thereby approaching the self-perception of real dental condition.
The present study investigated the distribution profile of dental caries and its association with areas of social deprivation at the individual and contextual level. The cluster sample consisted of 1,002 12-year-old schoolchildren from Piracicaba, SP, Brazil. The DMFT Index was used for dental caries and the Care Index was used to determine access to dental services. On the individual level, variables were associated with a better oral status. On the contextual level, areas were not associated with oral status. However, maps enabled determining that the central districts have better social and oral conditions than the deprived outlying districts.

Purpose: This paper aimed to assess the self-perceived oral health status in 137 elderly from Piracicaba city, São Paulo state, Brazil, and to investigate the influence of socio-demographic variables, institutionalization status and access to dental care services on the Geriatric Oral Health Assessment Index (GOHAI) index final score.Methods: The sample comprised institutionalized and non-institutionalized elderly, from 60 to 92 years old, socially independent or partially dependent, without significant cognitive alterations, with different educational and monthly income levels. An oral examination and two questionnaires were employed as instruments to gather subject characteristics. The first questionnaire included the 12 items of the GOHAI, and the second questionnaire gathered socio-demographic variables. The data were evaluated with a Chi-square test and logistic regression, with P < 0.05 as the significance cut-off.Results: The GOHAI final mean score of 27.49 indicated a low oral health self-perception, and the score was significantly associated with life style and institutionalization. The risk indicators for low oral health self-perception were the demand for urgent dental care and the self-perceived need for dental treatment. These indicators correlated with a lower GOHAI score, and the differences were statistically significant (P<0.0001), demonstrating that a low oral health selfperception is directly correlated with a worse oral health clinical status. This information can be useful for planning public health policies.Conclusion: The GOHAI final score in this study was considered low. Self-motivation and self-perception of the need for dental treatment were considered risk indicators for a low final GOHAI score.Key words: Elderly; quality of life; GOHAI index ResumoObjetivo: Este trabalho objetivou medir a auto-percepção nas condições de saúde bucal de 137 idosos do município de Piracicaba, São Paulo, Brasil; considerando a influência de variáveis sociodemográficas, situação de institucionalização, e acesso aos serviços de saúde bucal sobre a pontuação final do índice GOHAI.Metodologia: A amostra deste estudo foi composta por idosos institucionalizados e não institucionalizados, com idade variando de 60 a 92 anos de idade, socialmente independente ou parcialmente dependente, sem alterações cognitivas significantes, com diferentes níveis educacionais e renda mensal. Exame oral e dois questionários foram utilizados para coletar informações necessárias sobre os sujeitos da pesquisa: um primeiro composto pelos 12 itens do índice GOHAI, e um segundo para análise das variáveis sociodemográficas. Os dados foram tratados pelo teste do Qui-Quadrado e Regressão Logística, considerando P<0,05.Resultados: A média final do índice GOHAI foi de 27,49 denotando um baixo nível de auto-percepção, e uma associação estatisticamente significante para as variáveis estilo de vida e institucionalização.
