In the last fifty years evolution of scientific knowledge on the spleen provoked an entirely new approach to splenic surgery. It was shown that virulence may emerge as a significant consequence of environmental and evolutionary changes of some microbial communities, and devastating pathogenetic results of these changes become visible in human hosts without the splenic function. In other words: the spleen plays a pivotal role in the dynamic balance between biodiversity, microorganisms and immunogenecity in human hosts. Therefore, to preserve the "splenic immunologic repertoire" became an increasing commitment among surgeons. Understanding the integration of these multiple information on spleen, seems central to understand the new splenic surgery. Partial splenectomies (Réglées) -based on anatomical, experimental and clinical studies, developed at the University of Minas Gerais since the fifties -were successfully applied initially to treat the traumatic injuries of the spleen; in a following step, partial splenectomy were used to control hematological diseases. "Réglées" techniques on the spleen have conquered "ethical support, consilience status and clinical governance" to give birth to surgical therapeutic decisions on the spleen, in order to spare the structural integrity of the immune system. Splenic réglées procedures became a seminal achievement of splenic surgical practice. Initial results of "Partial splenectomies" -with conventional surgical armamentaria and techniques -were confirmed and improved by the introduction of techniques based on laparoscopic and endovascular approaches. And current usage of surgical splenic saving procedures propitiated the emergence of an appropriate lexicon for medical communication and became an "end point" of a "long-standing surgical inhibition" over the spleen (Rev. valva mitral (SVM), foram registrados entre 332 pacientes. Os autores reconhecem , entre seus casos, 3 roturas na junção atrioventricular, ocorridas logo após a saída de bypass e 1 rotura em parede posterior do VE, ocorrida na unidade de terapia intensiva (UTI). Os pacientes eram todos do sexo feminino e tinham, em média, 58 anos de idade. Tais acidentes aconteceram 3 vezes após substituição isolada da valva mitral e 1 vez em operação combinada de SVM e revascularização do miocárdio. Admitem que o mecanismo principal da rotura está ligado à criação de zona de acinesia isquêmica, localizada em parede do VE , secundária à superdistensão de anel mitral. Relacionam o desencadeamento dos acidentes com a superdistensão dos anéis valvares, avaliados com medidores inadequados, usados em corações profundamente relaxados pela cardioplegia. Preconizam modificações na cabeça desses medidores, adaptando-os a cabos maleáveis, de modo a permitir um posicionamento mais perfeito da cabeça do medidor do anel mitral. Admitem que o perfil mais ou menos elevado das próteses não parece haver influenciado no aparecimento, ou na prevenção desses acidentes, mas sim na determinação do tipo anatómico da lesão. Estão de acordo com outros autores, quando admitem que a fragilidade do miocárdio, em pacientes idosos, agravada com a remoção do aparelho valvar mitral (ventrlculo sem sustentação), criaria condições para o aparecimento desses acidentes. Consideram desejável a preservação do aparelho valvar mitral nas SVM, mas consideram que técnicas seguras, com essa finalidade, precisam ser ainda desenvolvidas.

Objective: To identify useful predictive data on chance mechanisms of postoperative outcome, the impact on symptoms of terminal heart failure -after partial left ventriculectomies (PLV) -was critically evaluated through the analysis of results, on accumulated descriptive data on reports, between 1995 and 1998.Method: Available routine clinical data on surgical aspects and clinical outcomes were gathered and, when possible, validated for comparative analysis.Results: PLV can provide a significant short to medium term amelioration in the quality-of-life in event-free survivors, but it was also watched out that in important proportion of them -as an evolutive sequence -PLV was incapable of changing the myocardial fibers leading tendency to conservatism of the preoperative vicious geometric dynamic pattern in late evolution. Importantly, the LV end-diastolic echocardiographic diameter of 7.5 cm (± 1.4 cm) was the steadiest quantitative significant numerical appeal to heart reduction surgery, in a setting of 465 patients, aged two to 74 years. And in a succession of individual reports of PLV results, whose mortality varied from 0 a 60%, survival after PLV showed a significant relationship with morphologic evolution Descriptors: Heart ventricle, surgery.
