BackgroundBetter communication among field health care teams and points of care, together with investments focused on improving teamwork, individual management, and clinical skills, are strategies for achieving better outcomes in patient-oriented care. This research aims to implement and evaluate interventions focused on improving communication and knowledge among health teams based on points of care in a regional public health outreach network, assessing the following hypotheses: 1) A better-working communication process between hospitals and primary health care providers can improve the sharing of information on patients as well as patients’ outcomes. 2) A skill-upgrading education tool offered to health providers at their work sites can improve patients’ care and outcomes.Methods/DesignA quasi-experimental study protocol with a mixed-methods approach (quantitative and qualitative) was developed to evaluate communication tools for health care professionals based in primary care units and in a general hospital in the southern region of São Paulo City, Brazil. The usefulness and implementation processes of the integration strategies will be evaluated, considering: 1) An Internet-based communication platform that facilitates continuity and integrality of care to patients, and 2) A tailored updating distance-learning course on ambulatory care sensitive conditions for clinical skills improvements.The observational study will evaluate a non-randomized cohort of adult patients, with historical controls. Hospitalized patients diagnosed with an ambulatory care sensitive condition will be selected and followed for 1 year after hospital discharge. Data will be collected using validated questionnaires and from patients’ medical records. Health care professionals will be evaluated related to their use of education and communication tools and their demographic and psychological profiles. The primary outcome measured will be the patients’ 30-day hospital readmission rates. A sample size of 560 patients was calculated to fit a valid logistic model. In addition, qualitative approaches will be used to identify subjective perceptions of providers about the implementation process and of patients about health system use.DiscussionThis research project will gather relevant information about implementation processes for education and communication tools and their impact on human resources training, rates of readmission, and patient-related outcomes.Electronic supplementary materialThe online version of this article (doi:10.1186/s12913-016-1626-9) contains supplementary material, which is available to authorized users.
Este artigo é uma versão adaptada, para este número suplementar da Revista Brasileira de Medicina de Família e Comunidade, do documento original da Sociedade Brasileira de Medicina de Família e Comunidade (SBMFC), editado em 2005, sob o mesmo título . disponível na íntegra no portal www.sbmfc.org.br. O texto situa a Medicina de Família e Comunidade (MFC) como uma especialidade médica centrada no paradigma da integralidade biopsicossocial que prioriza a APS, fazendo um nítido contraste com as especialidades focais. As políticas de saúde historicamente adotadas no Brasil privilegiaram, no entanto, um modelo assistencial hospitalocêntrico, bloqueando o desenvolvimento da APS e da MFC. Esta é uma tendência em mudança em face da Estratégia Saúde da Família e da experiência internacional nesta área. Neste contexto, a SBMFC, em parceria com o MS, vem incentivando a expansão dos programas de residência em MFC. Considerando a importância de que tal expansão seja implementada sem prejuízo da qualidade, a SBMFC apresentou um conjunto de critérios e recomendações, sintonizadas com a Resolução da CNRM . que regulamenta estes programas ., para que servissem de base para tal expansão. Neste artigo, são retomadas as exigências e recomendações relativas à organização do programa, à distribuição temporal das atividades e às competências básicas que deverão ser desenvolvidas ao longo do programa de formação. Além disso, são revistos os requisitos referentes à infra-estrutura e ao fluxo assistencial necessários para criar um ambiente favorável de treinamento em serviço. Por último, faz-se um pequeno relato sobre as iniciativas mais recentes que a SBMFC vem desenvolvendo no sentido de minorar a atual defasagem entre necessidade e oferta de preceptores especializados em MFC.
Este artigo relata a experiência de aplicação de metodologias ativas na disciplina “Programa de Interação Serviço Ensino e Comunidade” (PISEC) do curso de Medicina da Universidade Vila Velha (UVV). Utilizou-se a metodologia da problematização, que consiste em cinco passos: observação da realidade; definição de pontos-chave; teorização; hipótese de solução e aplicação à realidade. A problematização oportuniza a reflexão mediada pelo professor sobre atividades de interação, sendo um elemento chave para a satisfação pessoal dos alunos e para a melhoria do processo de aprendizagem.
A necessidade de um processo de especialização em larga escala em Medicina de Família e Comunidade (MFC) para os médicos da Estratégia Saúde da Família no Brasil (ESF) é identificada nesse documento. Os programas de residência médica (PRMs) seguem como padrão-ouro de especialização, mas não são suficientes para atingir o objetivo acima. Estimula-se que o processo seja acreditado pela Sociedade Brasileira de Medicina de Família e Comunidade (SBMFC) e que seus concluintes sejam certificados por meio do concurso de titulação TEMFC. Modalidades à distância com momentos presenciais são as mais adequadas, observando-se propiciar acesso a profissionais já inseridos em serviço, valorizar sua continuidade nos mesmos, orientar o aprendizado para uma integralidade inteligente e coordenar diferentes recursos para viabilizar o ensino-aprendizagem. As competências buscadas são as definidas pela WONCA, adaptadas à realidade nacional. Aprender a aprender como um MFC é o principal objetivo visado. Casos complexos,construídos de acordo com uma matriz de agravos e problemas diversos, atendidas às realidades regionais, são a principal ferramenta do processo. A SBMFC propõe-se a impulsionar tal estratégia
Objetivo: Discutir o processo de implantação de três Programas de Residência Médica em Medicina de Família e Comunidade (PRM-MFC) em uma capital da Região Sudeste do Brasil na perspectiva da interface dos programas com a organização do sistema de saúde local. Método: Trata-se de um relato de experiência oriundo das vivências dos supervisores dos programas e da equipe técnica do município responsável pela gestão da integração ensino-serviço. Foi produzida uma narrativa significativa aos atores do processo em momento presencial coletivo. Uma matriz SWOT foi elaborada para elencar os fatores que influenciam a implementação dos PRM-MFC na rede de serviços. Resultados: Há três PRM-MFC em atividade no município, todos em estágio inicial de implementação, totalizando nove vagas de R1. Os residentes são beneficiados por uma rede de serviços bem estruturada, diversificada e informatizada, incluindo preceptores sensibilizados e um forte apoio multiprofissional. Contudo, há questões que dificultam esse processo, como a incipiente política de valorização da preceptoria, as insuficiências do modelo de atenção primária existente para a formação médica, as dificuldades na gestão compartilhada do processo e a pouca legitimidade da RM perante a comunidade e os trabalhadores do serviço. Conclusão: Há íntima relação entre o desenvolvimento da rede local de saúde, em particular da APS, com a qualidade do PRM. Assim, é fundamental ter em vista não apenas as questões pedagógicas, mas também todo um conjunto de dispositivos de gestão compartilhada e qualificação da rede de serviços.
