Objetivos: Determinar la prevalencia de amiloidosis cardíaca en pacientes con amiloidosis sistémica. Comparar la supervivencia según sea que presenten o no compromiso cardíaco. Métodos: Cohorte retrospectiva de pacientes con amiloidosis sistémica del Registro Institucional de Amiloidosis del Hospital Italiano de Buenos Aires, entre 2010 y 2019. Se consideró como compromiso cardíaco la presencia de síntomas o imágenes consistentes con amiloidosis no explicado por otras causas. Se evaluó la muerte por todas las causas. Se calculó la sobrevida mediante Kaplan-Meier. Los factores relacionados con mortalidad se evaluaron con un modelo de regresión de Cox. Se evaluó el trasplante cardíaco en un modelo de regresión de riesgo competitivo. Resultados: La prevalencia de compromiso cardíaco fue del 63%. La incidencia de muerte fue de 14/1,000 personas-meses para el grupo con compromiso cardíaco y de 5/1,000 personas-meses para los pacientes sin compromiso. Los pacientes con compromiso cardíaco tuvieron una sobrevida global a los cinco años de 44% contra 67% en los que no tenían compromiso (p = 0.02). El HR crudo para el compromiso cardíaco fue de 2.09 (p = 0.02). La edad mostró un HR ajustado de 1.06 (p <0.01). El modelo de regresión de riesgos competitivos estableció un sub-HR de 1.86 (IC95%, 0.99-3.49; p = 0.05). Conclusiones: El compromiso cardíaco es un factor pronóstico importante en pacientes con amiloidosis.
Objective:The study aims to estimate the prevalence of cardiac amyloidosis in patients with systemic amyloidosis. Compare survival rates based on whether they show cardiac involvement. Methods: A retrospective cohort study of patients with systemic amyloidosis from the Institutional Amyloidosis Registry of the Hospital Italian of Buenos Aires from 2010 to 2019. Heart involvement is considered to be the presence of symptoms and/or images consistent with amyloidosis, and there is no other reason to explain it. Death for all causes were evaluated. The survival rate was estimated by Kaplan-Meier. Cox regression model was used to evaluate factors related to mortality. Heart transplantation was evaluated in a competitive risk regression model. Results: The prevalence of heart involvement is 63%. For the group with heart involvement, the death rate was 14/1000 person-months, and for patients without damage, the death rate was 5/1000 person-months. The 5-year overall survival rate for patients with heart involvement was 44%, while that for patients without damage was 67% (p = 0.02). Crude hazard ratio (HR) for heart involvement was 2.09 (p= 0.02). Age showed an adjusted HR of 1.06 (p <0.01). The sub-HR estimated by the competitive risk regression model was 1.86 (95% confidence intervals 0.99-3.49) p = 0.05. Conclusion: Cardiac involvement is an important prognostic factor in patients with amyloidosis.
Introducción: la amiloidosis AA puede ser una complicación de ciertos trastornos inflamatorios crónicos, aunque entre el 21% y 50% puede ser idiopática. No existe un tratamiento específico. El tocilizumab, dirigido contra el receptor de IL-6 y orientado a disminuir la producción de SAA, podría ser eficaz. Métodos: en este estudio informamos datos de 6 pacientes con amiloidosis AA tratados con tocilizumab monoterapia subcutáneo en el período 2011-2018. Los criterios de valoración principales fueron la mejora clínica y bioquímica de los órganos afectados y los parámetros bioquímicos marcadores de inflamación. Resultados: el riñón estaba afectado en todos los pacientes, manifestándose con caída del filtrado glomerular y síndrome nefrótico. La hemorragia digestiva se presentó en un paciente y otro tenía afectación pulmonar en la biopsia. Luego del posterior al tratamiento, todos mejoraron el hematocrito, la albúmina sérica y el índice de masa corporal. El SAA disminuyó en 5 pacientes. Un paciente mejoró su función renal, mientras 4 se mantuvieron estables. Tres pacientes disminuyeron los valores de proteinuria. Conclusión: el tratamiento con tocilizumab podría ser eficaz en el tratamiento de los pacientes con amiloidosis AA.
Introducción: La amiloidosis es una enfermedad crónica infrecuente, que se caracteriza por el depósito extracelular de proteínas fibrilares en diversos órganos y tejidos. Las manifestaciones clínicas se basan según el sitio de los depósitos y de la proteína precursora involucrada. La principal herramienta de diagnóstico es la sospecha clínica. El pronóstico suele ser sombrío. El objetivo de este estudio es describir los acontecimientos que fueron sucediendo en el tiempo para llegar a lo que conocemos en la actualidad como amiloidosis. Materiales y métodos: se realizó un estudio exploratorio, narrativo, de carácter histórico. Se revisaron fuentes primarias y fuentes documentales existentes. Desarrollo: Las primeras descripciones de pacientes con amiloidosis datan de mediados de 1600 d.c. Éstas eran descripciones anatómicas, reportadas a partir de autopsias de pacientes. Las primeras aproximaciones al diagnóstico fueron a través de tinciones con iodo y anilinas, llegando por último a la difracción por rayos x, y la microscopía con luz polarizada bajo tinción con rojo Congo. Conclusiones: A pesar de ser una entidad nosológica conocida desde hace cuatro siglos, las dificultades para caracterizar el origen y comprender la fisiopatología de la amiloidosis aún permanecen. Durante décadas hubo controversias acerca de la nomenclatura y la naturaleza del material amiloideo. El diagnóstico y el manejo de esta enfermedad siguen dependiendo de los avances en la tecnología, siendo actualmente un desafío en el manejo de los pacientes que padecen amiloidosis.
Introducción: El tratamiento oncológico es cada vez más agresivo. El objetivo de este trabajo fue estimar las personas fallecidas por causa oncológica, reportar la frecuencia de uso de quimioterapia (QT) en los últimos 3 meses de vida, y describir las características clínicas-epidemiológicas de estos pacientes. Métodos: Se incluyó una muestra consecutiva de fallecidos durante el 2017, afiliados a prepaga del Hospital Italiano de Buenos Aires. Mediante revisión de historias clínicas, se los clasificó según la causa de muerte (oncológico u otra causa), validando diagnóstico y estadio de base, performance status (PS). Se reportan prevalencias con IC95% y se utilizó estadística descriptiva. Resultados: Fallecieron 2293 personas, 59% mujeres con mediana de 84 años. Hubo un total de 736 fallecidos por cáncer, representando el 32% (IC95% 30-34). Este último subgrupo presentó una mediana de 75 años, 54% eran mujeres y sólo una paciente tenía directivas anticipadas. En cuanto al lugar de fallecimiento, 80% ocurrió hospitalizado (65% sala general y 15% unidad cerrada). Los tumores más frecuentes fueron: pulmón, colorrectal-gástrico, hematológico, y mama. Un total de 390 pacientes recibieron QT al final de la vida (53%; IC95% 49-57), siendo 53% mujeres y con promedio de 68 años. En cuanto a la enfermedad oncológica de base: 81% tenían tumor sólido, 75% estadío avanzado, y mayoritariamente con mala/regular capacidad para realizar actividades cotidianas (25% PS3 y 32% PS4 respectivamente). Conclusión: Existe una frecuencia preocupante del uso de QT en el fin de vida y los fallecimientos siguen siendo principalmente intrahospitalarios.
