The yellow fever virus (YFV) epidemic in Brazil is the largest in decades. The recent discovery of YFV in Brazilian Aedes species mosquitos highlights a need to monitor the risk of reestablishment of urban YFV transmission in the Americas. We use a suite of epidemiological, spatial, and genomic approaches to characterize YFV transmission. We show that the age and sex distribution of human cases is characteristic of sylvatic transmission. Analysis of YFV cases combined with genomes generated locally reveals an early phase of sylvatic YFV transmission and spatial expansion toward previously YFV-free areas, followed by a rise in viral spillover to humans in late 2016. Our results establish a framework for monitoring YFV transmission in real time that will contribute to a global strategy to eliminate future YFV epidemics.

O objetivo deste estudo foi explicar a associação dos fatores socioambientais e dos grandes usos da terra com a ocorrência de casos de leishmaniose tegumentar americana (LTA) nos circuitos espaciais de produção, no Estado de Minas Gerais, Brasil. Trata-se de um estudo ecológico do tipo analítico, baseado em dados secundários de casos de LTA dividido por triênio, no período entre 2007 a 2011, cujas unidades de análise foram os municípios pertencentes aos circuitos espaciais. Duas etapas distintas foram realizadas, sendo a elaboração de mapas temáticos com a identificação dos circuitos a primeira, e na segunda etapa um novo indicador casos de LTA por densidade demográfica foi associado com indicadores socioambientais e dos grandes usos da terra submetidos à análise multivariada de componentes principais (ACP). Para o período avaliado, identificou-se três circuitos distribuídos nas mesorregiões Norte de Minas Gerais, Vale do Rio Doce e Região Metropolitana de Belo Horizonte. Houve forte associação dos casos de LTA por densidade demográfica com lavoura temporária, pastagem natural, floresta natural, terras inaproveitáveis e população rural, e uma fraca associação com pastagem plantada. A associação de casos com variáveis dos grandes usos da terra em diferentes perfis agropecuários demonstra o caráter ocupacional da LTA, associado principalmente com trabalhadores da zona rural. A associação da doença com as variáveis ambientais e deficiência das condições de saneamento básico também demonstram relevância no perfil de transmissão nos circuitos espaciais de produção em Minas Gerais.
We investigated the influence of CD4؉ T lymphocytes, CD8 ؉ T lymphocytes, and B lymphocytes on the efficacy of posaconazole (POS) and the reference drug benznidazole (BZ) during treatment of acute Trypanosoma cruzi infection in a murine model. Wild-type mice infected with T. cruzi and treated with POS or BZ presented no parasitemia, 100% survival, and 86 to 89% cure rates, defined as the percentages of animals with negative hemocultures at the end of the observation period. CD4؉ -T-lymphocyte-knockout (KO) mice infected with T. cruzi and treated with BZ or POS controlled parasitemia during treatment, although circulating parasites reappeared after drug pressure cessation, leading to only a 6% survival rate and no cure. CD8؉ -Tlymphocyte-KO mice infected with T. cruzi and treated with POS or BZ had intermediate results, displaying discrete parasitemia after the treatment was ended, 81 and 86% survival, and cure rates of 31 and 66%, respectively. B-lymphocyte-KO mice infected with T. cruzi and treated with BZ relapsed with parasitemia 1 week after the end of treatment and had a 67% survival rate and only a 22% cure rate. In contrast, the activity of POS was much less affected in these animals, with permanent suppression of parasitemia, 100% survival, and a 71% cure rate. Our results demonstrate that abrogation of different lymphocytes' activities has distinct effects on the efficacy of POS and BZ in this experimental model, probably reflecting different parasite stages preferentially targeted by the two drugs and distinct cooperation patterns with the host immune system.
BackgroundIncreasing reports of high-resistant Triatominae populations concerns scientists and sanitarians as little is known about the factors behind the occurrence of such phenotype and its real impact on vector control strategies. Moreover, the utilization of a large variety of methodologies hinder the comparison of the reported studies.MethodsThis work aims to review laboratory bioassays, redefining the assessed biological features (age, generation and insecticide application area) and technical procedures (mortality recording time and the ideal diagnostic dose).ResultsResults were not influenced by the insecticide application area in nymphs or by their generation. Three days-old specimen’s revealed lower susceptibility to the tested insecticide. We determined that it is more appropriate to record mortality 72 h after treatment with insecticide, as well as using a diagnostic dose of 1xDL99.ConclusionThis work suggests more adequate methodological parameters for assessing insecticide resistance in triatomines, which also allows the comparison of results obtained by different research groups. For laboratory bioassays, we recommend: 1) the use of first instar nymphs from first or second generation; 2) 3 day-old specimens; 2) application of insecticide in the dorsal or ventral abdomen area; 3) mortality recording 72 h after treatment with pyrethroids and 4) a diagnostic dose of 1x LD99.
Case-fatality rates were higher than that observed among patients with VL treated with other therapies. Identification of prognostic factors of death from VL may allow early diagnosis of patients prone to such outcome and prompt an expeditious and appropriate management of VL to reduce fatality rates.
