Introduction: Administration of S. boulardii in addition to rehydration therapy in diarrhea found to be beneficial in many aspects owing to a variety of causes and importantly it is was not associated with any adverse effects.Material and Methods: We conducted a prospective study of children suffering from acute diarrhoea, at a private tertiary care hospital. Children were divided into 2 groups randomly as per odd(Group 1 ) and even (Group 2) bed allotted in indoor at the time of admission: Group 1 included children on oral rehydration therapy (ORT) + Zinc + Saccharomyces boulardii (Probiotic 5 billion CFU twice daily) and Group 2 comprised of children on ORT+ Zinc. Our objective was to systematically review data on the effect of S. boulardii on acute childhood diarrhoea.Results: Out of a total of 126 children less than 2 years, 2-6 years and 6-14 years were 72 (57.14%), 42(33.33%) and 12(09.52%) respectively. The duration of diarrhoea in Group 1 was 26.31 hours and Group 2 was 47.81 hours (p<0.01). The frequency of diarrhoea showed improvement within 24 and 72 hours in Group 1 and Group 2 respectively (p<0.01).Similarly, the mean duration of hospital stay was 2.68 days in Group 1 and 4.8 days in Group 2.The treatment cost was INR 850 and INR 1650 while social cost was INR 1250 and 2600 in Group 1 and 2 respectively.Conclusion:This study shows that S. boulardii reduced the duration, frequency and hospital stay of diarrhoea thereby reducing the treatment and social costs.J Nepal Paediatr Soc 2016;36(3):250-255
Introduction: Diarrhoea is defined as the passage of three or more loose stools per day. Diarrhoea is one of the leading causes of childhood mortality in India. The objectives of the study were to assess household Knowledge, Attitude and Practice for prevention of Diarrhoea in children through preformed questionnaires. Material and Methods: Four hundred and sixty eight under five children were selected for study by Cluster sampling method. Background information, details of acute diarrhoea and treatment modalities were obtained from mother of the under five children. This was a hospital based study conducted at a tertiary care hospital, from 1st May 2017 to 30th December 2017. Results: About 70% of the moms were between the age assemble 25-32 years and the vast majority of them were housewives (72%). A greater part (>66%) of the moms were having essential/auxiliary education. Learning with respect to different parts of the diarrhoea and ORS surprisingly extremely unacceptable. In the present examination it is seen that learning in regards to ORS in looseness of the bowels is more in educated mother than uneducated. Three sitting of Educational mediations realized huge enhancement of the middle scores of learning, state of mind and practice expanded from 9, 6, 3 to 19, 14, 8, individually, Conclusion: The household knowledge regarding method of use and preparation of ORS for management of diarrhoea was observed to be lacking in this investigation. More estimates should be taken to enhance this learning and make moms mindful about the planning and utilization of ORS.
BackgroundForeign body aspiration is one of the leading causes of childhood morbidity and mortality among older infants and toddler age groups. Missed and delayed diagnosis of foreign body aspiration can lead to increased incidence of complications. Early diagnosis can prevent life-threatening complications and morbidity. In this study, we aimed to evaluate the clinical and radiological details, types, localization of foreign bodies, complications, and outcomes in pediatric patients who presented to our hospital with foreign body aspiration. MethodologyWe conducted a retrospective analysis of hospital case records of children aged one month to 14 years who were admitted to the Department of Pediatrics between June 2018 and May 2020, with clinical suspicion of foreign body aspiration. ResultsA total of 22 children with a diagnosis of airway foreign body were included. The mean age of presentation was three years (SD: ±2.22), with a boy-to-girl ratio of 3.4:1. Cough (81.8%) and tachypnea (72.7%) were the most common clinical symptoms. The median duration between symptom onset and diagnosis was three (interquartile range: 6) days. Unilateral reduced breath sound (81.8%) was the most common clinical examination finding. The common site of impaction was the right main bronchus in 59.1% of cases. The foreign bodies retrieved during bronchoscopy were organic substances in 63.6% of cases, with peanuts being the most common (31.8%). Chest radiographs were normal in 36.3% of cases, and common abnormalities included hyperinflation, collapse, consolidation, and mediastinal shift. Mechanical ventilation was required in 54.5% of cases. The mean duration of hospitalization was five (SD: ±2.84) days. Complications such as pneumothorax were seen in one (4.5%) case. Mortality was seen in 4.54% of cases during the bronchoscopic procedures. ConclusionsForeign body aspiration was common in young male children, with cough being the common symptom. Normal X-rays of the chest were seen in one-third of cases. The common site of impaction was the right main bronchus, and organic substances such as peanuts were common foreign bodies retrieved. Strong clinical suspicion of foreign body aspiration should be kept in cases with acute onset of cough in young children. Prompt medical attention is needed to reduce the morbidity and mortality associated with foreign body aspiration.
Objective: Congenital anomaly is one of the most important causes & being the 5th most common cause of neonatal mortality & morbidity. It may present as a structural or functional abnormality. These defects occur due to defective embryogenesis. Associated factors may be maternal age, maternal TORCH infection, drugs, genetic factors. Antenatal USG reduces the incidence.Materials and Methods: A cross-sectional study was done in the Pediatric department over 1 year. Diagnosis of all congenital anomalies was done by the concerned pediatrician& pediatric surgeon. Data was collected in the specified format.Results: A total of 10205 cases of age group 1 month to 5 years presented to the paediatric OPD, out of which 193 children were diagnosed as congenital anomalies in 1 year. Males were found to be affected the most. The most common system involved was found to be the genitourinary system (36.78%). The second most common system involved was the gastrointestinal system (33.67%). The least common system involved was the musculoskeletal system.Conclusion: Congenital anomalies are a major cause of neonatal & infantile mortality & morbidity. Routine screening with a level II targeted scan for all the pregnant mothers should be mandatory. Adequate nutrition, parental education & Rubella vaccination of the mother can decrease the prevalence of congenital anomalies to some extend.
Introduction: According to NHFS-4 data, around 38% of under-five Indian children are malnourished and stunted. In addition to poor socio-economic status, faulty complementary feeding practice is a major contributor to this. The objective of this study is to know the prevailing complementary feeding practices in our area, the most common food type preferred for introduction during initiation of complementary feeds, knowledge of the mother and their family members regarding complementary feeding, the factors influencing in decision making and its financial burden on the family. Methods: This hospital based cross-sectional study was conducted in a private Medical College in Bhubaneswar, Odisha, India. 256 mothers of infants between six months to two years attending Paediatric OPD from December 2018 to June 2019 were selected by random sampling technique. Data were collected using a structured questionnaire. Results: Out of the total 256 subjects interviewed, 134 (76.13%) out of 176 families belonging to lower income group preferred commercially available processed food over home food as the initial weaning food as compared to 32 (40%) out of 80 of the high income group preferring the same. The lower income group spent 22.3% of the total family income on commercial preparations to feed their infants in the age group six to 12 months. Whereas high income group families spend an average of 14.3% of family income on baby food products in the same age group. Despite being in regular contact with the local physician, in 85% of the total visits to the doctor, the opportunity wasn’t utilised to counsel the family member about complementary feeding practices. Conclusions: Commercial preparations are the primary preferred weaning food. The dietary diversity of complementary food is very poor, thus affecting growth and development. The false perception that commercial preparations are critical to child growth and development is overburdening the family finances.
