The last decade is characterized by significant progress in the treatment of rectal cancer (reduction in the number of relapses to 5–6 % with the use of prolonged radiation therapy) before surgery. The greatest success has been achieved in the treatment of cancer of the lower ampulla of the rectum, when it is possible to develop a complete clinical response of the rectal tumor to chemoradiotherapy. Nevertheless, the requirement issues to improve the results of treatment of cancer of the upper and middle ampullar rectum with an increase in the survival of patients remain. Which makes it relevant to develop new methods, that increase the effectiveness of the treatment of rectal cancer.The method of modified chemoradiotherapy for cancer of the upper ampulla of the rectum was developed in our study. The method is as follows: at the first stage, one day before the start of radiation therapy, the patient undergoes superselective catheterization of the superior rectal artery through the radial or femoral artery, followed by regional administration of radiomodifying chemotherapy drugs: cisplatin 50 mg and fluorouracil 500 mg. In one day, patients begin to undergo a course of conformal remote large- fraction radiation therapy to the primary focus and metastasis pathways for 5 sessions with a single focal dose of 5 Gy to a total focal dose of 25 Gy using a low-energy linear accelerator. During the entire course of radiation therapy, fluorouracil 500 mg is administered daily intravenously for 30 minutes in 30 minutes before the session. Surgical intervention with the sampling of material for research is carried out 6–8 weeks after the radiation therapy is completed. To assess the effectiveness of the modified chemoradiotherapy, the stage of tumor regression was determined according to the RECIST scale, and the level of therapeutic pathomorphology of the tumor according to Dworak was determined during a morphological study of the rectal tumor removed during the operation.The developed method of modified chemoradiotherapy makes it possible to achieve regression of the rectal tumor in a short time, reduce the time and increase the effectiveness of treatment. The method of modified chemoradiotherapy is intended for patients with cancer of the upper and middle ampullar rectum T3-4N0-2M0, for whom radiation therapy is indicated as the first stage of treatment, after which resection of the rectum is performed in a standard volume.
Лучевая терапия является одним из методов лечения опухолей толстой кишки (РТК). Однако в ряде случаев наблюдается отсутствие эффективности подобной терапии, обусловленное эффектом радиорезистентности опухолевых клеток. Это требует корректировки протоколов лучевой терапии, для чего требуется серия исследований. Начальным этапом таких исследований может стать анализ эффективности воздействия разных доз лучевой терапии на выживаемость опухолевых клеток в модельных экспериментах на соответствующих клеточных культурах. Целью работы стало определение влияния стандартных доз лучевой терапии 5 и 7 Гр на протяжении 5 дней (изоэффективная доза 37,5 Гр и 49,6 Гр соответственно) на выживаемость клеток аденокарциномы толстой кишки линии HT-29. Культивирование клеток проводили в стерильных флаконах в среде RPMI-1640 c 10% фетальной телячьей сывороткой и 50 мкг/мл гентамицина. Облучение проводили на линейном ускорителе Novalis TX (Varian, США). После каждого дня облучения проводили микроскопирование с помощью инвертированного микроскопа, после 5-го дня облучения с помощью проточной цитофлюориметрии проводили оценку количества клеток, находящихся на различных стадиях апоптоза. В ходе модельного эксперимента показано, что пятидневное облучение на линейном ускорителе Novalis TX культуры клеток НТ-29 в дозах 5 и 7 Гр (изоэффективная доза 37,5 Гр и 49,6 Гр соответственно) привело к значительному снижению количества жизнеспособных клеток, при этом возросло количество опухолевых клеток, находящихся на разных стадиях апоптоза, и мертвых клеток. Цитотоксический эффект от доз 5 и 7 Гр проявился уже после первого облучения, и для дозы 7 Гр был выражен сильнее. Тем не менее после пятидневного облучения в дозах 5 и 7 Гр (изоэффективная доза 37,5 Гр и 49,6 Гр соответственно) 33,7% и 18,5% соответственно от изначального количества клеток HT-29 сохранили свою жизнеспособность, что может быть связано с формированием радиорезистентности. За последние 30 лет в клиническую и мировую практику вошли и стали широко применяться различные методы рентгеноэндоваскулярной диагностики и лечения, преимуществом которых является малоинвазивность вмешательства. Интервенционная хирургия является перспективной и динамически развивающейся отраслью современной клинической медицины, сутью которой служит фармацевтическое и механическое воздействие на сосуды, участвующие в кровоснабжении опухоли, и тем самым достигается необходимый лечебный эффект. Рентгеноэндоваскулярная хирургия хорошо зарекомендовала себя в онкологической практике. Важным аспектом данного метода является возможность применения его в регионарной терапии опухолевого процесса. В данной статье освещены преимущества применения рентгеноэндоваскулярной хирургии в различных областях медицины, в том числе и в онкологии. Проанализированы работы по использованию суперселективной интраартериальной химиоэмболизации, применяемой при опухолях различных локализаций. Одним из технических методов, способствующих повышению внутриопухолевой концентрации цитостатика, является методика селективной и суперселективной эмболизации и химиоэмболизации сосудов опухоли. Химиоэмболизация различными микросферами осуществляет более длительный контакт злокачественных клеток с химиопрепаратом. Возможности проведения суперселективной внутриартериальной химиоэмболизации в качестве одного из компонентов противоопухолевого лечения, применяемых на предоперационном этапе лечения злокачественных новообразований головы и шеи, в настоящее время мало изучены, что требует дальнейшего изучения. e15526 Background: Colorectal cancer (CRC) is among the most common cancers. The leading cause of high mortality is tumor progression developed due to the epithelial to mesenchymal transition (EMT). The ZEB1 protein is one of the critical regulators of this process. In this regard, our study aimed to assess the ZEB1 expression in different stages of colorectal cancer. Methods: This study included samples of 206 patients with stage II-IV CRC aged 42 to 86 years (mean age 64.2±1.7). All patients were divided into three groups: group 1 - patients with T3-4 N0 M0 (stage II) with high-risk factors for recurrence, n = 6; group 2 - patients with T1-4 N1-2 M0 (stage III), n = 88; group 3 - patients with T1-4 N0-2 M1 (stage IV), n = 58. IHC study was performed using polyclonal rabbit antibodies to ZEB1 (Biorbyt Ltd.) diluted 1:200 and a Reveal Polyvalent HRP-DAB Detection System. The staining percentage and intensity were assessed: 0, 1+ weak, 2+ moderate, 3+ strong. The nuclear reaction of the ZEB1 protein was considered positive when staining was detected in more than 10% (cut-off) of tumor cells with intensities of 2+ and 3+. Statistical analysis of the results was carried out using the STATISTICA 13.0 software (StatSoft Inc., USA). Results: A positive nuclear reaction for ZEB1 was detected in 80.6% (166 of 206 patients), while negative in 19.4% (40 of 206 patients). The maximal percentage of patients with positive staining for ZEB1 was among those with stage IV (94.8%), the minimal percentage - stage II (60%). The prevalence of ZEB1+ in patients with stages III and IV significantly increased the risk of tumor progression by 3.5 (95% CI 1.8-8.4) and 12.2 (95% CI 3.4-43.6) times, respectively, compared with stage II patients. No statistical significance was observed in the comparison between patients with stages III and IV (95% CI 0.9-11.7). The percentage of ZEB1+ samples increased in more advanced tumors. The ratio of ZEB1+/ZEB1- tumors in stage II was 1.5, in stage III - 5.8, in stage IV - 18.3 (p < 0.05). Conclusions: The immunohistochemical study revealed the features of ZEB1 expression in different stages of colorectal cancer, which can serve as prognostic factors that determine the disease progression.
