Năm 2020 là một năm đầy khó khăn, thách thức đối với ngành Y tế Việt Nam và thế giới khi phải chiến đấu với đại dịch COVID-19. Trong tình hình đó, các nhà khoa học Việt Nam vẫn nỗ lực không ngừng nghỉ, lần lượt chinh phục những đỉnh cao của y học, được giới chuyên môn trong nước và cộng đồng quốc tế đánh giá cao. Tiêu biểu trong những thành tích đó là hai ca ghép chi thể được thực hiện tại Bệnh viện Trung ương Quân đội 108 (BV TWQĐ 108): Một ca ghép được thực hiện bằng cách sử dụng chi thể lấy từ người cho sống đầu tiên trên thế giới và một ca ghép hai cẳng bàn tay lấy từ người cho chết não lần đầu được thực hiện ở khu vực Đông Nam Á. Thông tấn xã Việt Nam đã bình chọn đây là một trong 10 sự kiện nổi bật của Việt Nam năm 2020. Thực hiện các nội dung của Đề án khoa học công nghệ “Tăng cường năng lực nghiên cứu để phát triển kỹ thuật ghép mô, bộ phận cơ thể người tại BV TWQĐ 108”, trong giai đoạn 2016 - 2021, Bệnh viện đã triển khai nhiều đề tài nghiên cứu ứng dụng, tập trung đào tạo đội ngũ nhân lực chất lượng cao, bổ sung nhiều trang thiết bị hiện đại, đồng bộ đảm bảo thực hiện thành công các kỹ thuật ghép mô, tạng. Tính đến tháng 3/2021, Bệnh viện TWQĐ 108 đã thực hiện thành công kỹ thuật ghép 7 loại mô, tạng khác nhau trên 316 bệnh nhân, bao gồm: Ghép tế bào gốc 130 ca, ghép tủy 16 ca, ghép giác mạc 18 ca, ghép phổi 03 ca, ghép thận 78 ca, ghép gan 70 ca, ghép chi thể 02 ca, lấy - ghép đa tạng từ người cho chết não 05 ca; chức năng các tạng sau ghép tốt, kéo dài thời gian và chất lượng sống cho bệnh nhân, mang lại nhiều giá trị khoa học và giá trị nhân văn sâu sắc. Hướng đi trọng tâm của Bệnh viện trong thời gian tới đó là tiếp tục phát triển các kỹ thuật ghép, triển khai thêm nhiều kỹ thuật khó như ghép mặt, ghép tụy, ghép ruột… nghiên cứu tạo nguồn mô tạng sinh học và vật liệu thay thế (công nghệ sao chép cơ quan 3D), tạng nhân tạo; kiểm soát vấn đề thải ghép; ứng dụng công nghệ thông tin, trí tuệ nhân tạo trong ghép tạng; tăng cường công tác tổ chức- điều phối ghép tạng, xây dựng nhiều chương trình vận động và vinh danh người hiến tạng. Bài báo này tổng kết đánh giá các kết quả chính của quá trình triển khai nghiên cứu, ứng dụng kỹ thuật ghép mô, bộ phận cơ thể người của Bệnh viện TWQĐ 108 trong giai đoạn (2016-2021) - giai đoạn 1 của Đề án khoa học công nghệ “Tăng cường năng lực nghiên cứu để phát triển kỹ thuật ghép mô, bộ phận cơ thể người tại BV TWQĐ 108” và phương hướng phát triển kỹ thuật ghép tạng trong tương lai.
Mục tiêu: Đánh giá kết quả của phẫu thuật ghép gan từ người hiến sống sử dụng mảnh ghép gan phải. Đối tượng và phương pháp: Nghiên cứu tiến cứu loạt trường hợp được ghép gan từ người hiến sống sử dụng mảnh ghép gan phải, từ tháng 10/2017 đến tháng 1/2021, tại Bệnh viện Trung ương Quân đội 108. Kết quả: 60 bệnh nhân được ghép gan từ người hiến sống sử dụng mảnh ghép gan phải. Độ tuổi trung bình là: 48,87 ± 13,49 tuổi (13 - 72 tuổi). Tỷ lệ nam/ nữ là 9/1. Trong nhóm nghiên cứu có 27 bệnh nhân ung thư gan, 18 bệnh nhân xơ gan mất bù và 15 bệnh nhân suy gan cấp với điểm MELD trung bình là: 29,4 ± 8,5 (15 - 40). 100% các trường hợp ghép sử dụng mảnh ghép gan phải. 15 trường hợp được thực hiện với kĩ thuật tạo hình tĩnh mạch trên gan giữa sử dụng đoạn mạch nhân tạo và 45 trường hợp thực hiện với kĩ thuật lấy mảnh ghép gan phải mở rộng kèm theo tĩnh mạch gan giữa. Biến chứng liên quan đến mạch máu là 8,3% bao gồm: Huyết khối động mạch, hẹp động mạch gan và hẹp tĩnh mạch cửa. 7 trường hợp hẹp đường mật, đã được can thiệp đặt stent đường mật qua nội soi mật tuỵ ngược dòng thành công. Thời gian sống thêm 1 năm sau ghép là 86,7%. Kết luận: Phẫu thuật ghép gan từ người hiến sống sử dụng mảnh ghép gan phải đạt kết quả tốt.
