Năm 2020 là một năm đầy khó khăn, thách thức đối với ngành Y tế Việt Nam và thế giới khi phải chiến đấu với đại dịch COVID-19. Trong tình hình đó, các nhà khoa học Việt Nam vẫn nỗ lực không ngừng nghỉ, lần lượt chinh phục những đỉnh cao của y học, được giới chuyên môn trong nước và cộng đồng quốc tế đánh giá cao. Tiêu biểu trong những thành tích đó là hai ca ghép chi thể được thực hiện tại Bệnh viện Trung ương Quân đội 108 (BV TWQĐ 108): Một ca ghép được thực hiện bằng cách sử dụng chi thể lấy từ người cho sống đầu tiên trên thế giới và một ca ghép hai cẳng bàn tay lấy từ người cho chết não lần đầu được thực hiện ở khu vực Đông Nam Á. Thông tấn xã Việt Nam đã bình chọn đây là một trong 10 sự kiện nổi bật của Việt Nam năm 2020. Thực hiện các nội dung của Đề án khoa học công nghệ “Tăng cường năng lực nghiên cứu để phát triển kỹ thuật ghép mô, bộ phận cơ thể người tại BV TWQĐ 108”, trong giai đoạn 2016 - 2021, Bệnh viện đã triển khai nhiều đề tài nghiên cứu ứng dụng, tập trung đào tạo đội ngũ nhân lực chất lượng cao, bổ sung nhiều trang thiết bị hiện đại, đồng bộ đảm bảo thực hiện thành công các kỹ thuật ghép mô, tạng. Tính đến tháng 3/2021, Bệnh viện TWQĐ 108 đã thực hiện thành công kỹ thuật ghép 7 loại mô, tạng khác nhau trên 316 bệnh nhân, bao gồm: Ghép tế bào gốc 130 ca, ghép tủy 16 ca, ghép giác mạc 18 ca, ghép phổi 03 ca, ghép thận 78 ca, ghép gan 70 ca, ghép chi thể 02 ca, lấy - ghép đa tạng từ người cho chết não 05 ca; chức năng các tạng sau ghép tốt, kéo dài thời gian và chất lượng sống cho bệnh nhân, mang lại nhiều giá trị khoa học và giá trị nhân văn sâu sắc. Hướng đi trọng tâm của Bệnh viện trong thời gian tới đó là tiếp tục phát triển các kỹ thuật ghép, triển khai thêm nhiều kỹ thuật khó như ghép mặt, ghép tụy, ghép ruột… nghiên cứu tạo nguồn mô tạng sinh học và vật liệu thay thế (công nghệ sao chép cơ quan 3D), tạng nhân tạo; kiểm soát vấn đề thải ghép; ứng dụng công nghệ thông tin, trí tuệ nhân tạo trong ghép tạng; tăng cường công tác tổ chức- điều phối ghép tạng, xây dựng nhiều chương trình vận động và vinh danh người hiến tạng. Bài báo này tổng kết đánh giá các kết quả chính của quá trình triển khai nghiên cứu, ứng dụng kỹ thuật ghép mô, bộ phận cơ thể người của Bệnh viện TWQĐ 108 trong giai đoạn (2016-2021) - giai đoạn 1 của Đề án khoa học công nghệ “Tăng cường năng lực nghiên cứu để phát triển kỹ thuật ghép mô, bộ phận cơ thể người tại BV TWQĐ 108” và phương hướng phát triển kỹ thuật ghép tạng trong tương lai.
Mục tiêu: Đánh giá tốc độ tự tiêu và khả năng tương thích sinh học của vật liệu kết xương magie (Mg) phủ hydroxyapatite (HAp) nhằm ứng dụng trong chấn thương chỉnh hình. Đối tượng và phương pháp: Vật liệu kết xương Mg phủ HAp được sử dụng cho thí nghiệm nuôi cấy tế bào trong môi trường MEM-a. Tốc độ tự tiêu của vật liệu kết xương được đánh giá bằng lượng khí hydro giải phóng và hình thái bề mặt mẫu. Khả năng tương thích sinh học của vật liệu kết xương được đánh giá qua hình thái tế bào, khả năng tăng sinh của tế bào. Kết quả: Thí nghiệm nuôi cấy tế bào MC3T3-E1 trong môi trường MEM-a cho thấy mẫu được phủ HAp không những thể hiện lượng khí hydro thoát ra giảm mà còn có mật độ tế bào tăng sinh lớn hơn nhiều so với mẫu không được phủ HAp. Kết luận: Lớp phủ HAp vừa có khả năng làm giảm tốc độ tự tiêu vừa làm tăng tính tương thích sinh học của vật liệu kết xương phù hợp để ứng dụng vào chấn thương chỉnh hình.
Mục tiêu: Tìm hiểu các yếu tố liên quan và kết quả lâm sàng của phương pháp phẫu thuật nắn chỉnh mở và kết xương bên trong trong điều trị các trường hợp gãy xương - sai khớp cùng chậu. Đối tượng và phương pháp: 48 trường hợp gãy xương - sai khớp cùng chậu, được phẫu thuật kết xương bên trong tại Khoa Chấn thương Chỉnh hình, Bệnh viện Chợ Rẫy (từ tháng 01/2015 đến tháng 6/2019). Kết quả: Phần lớn các trường hợp gãy xương - sai khớp cùng chậu do nguyên nhân tai nạn giao thông chiếm tỷ lệ 75% và tổn thương các cơ quan khác đi kèm (47/48). Có 36 tổn thương khớp cùng chậu được phân loại theo Day: Day I (27,8%), Day II (30,6%) và Day III (41,7%). Mức độ phục hồi hình thể giải phẫu tốt và rất tốt theo tiêu chuẩn của Lindahl chiếm tỷ lệ 87,5%. Mức độ phục hồi hình thể giải phẫu không phụ thuộc vào kiểu tổn thương và thời điểm can thiệp phẫu thuật. Có 1 trường hợp tổn thương nhánh mông trên của động mạch chậu trong và được làm tắc mạch qua DSA. 4 trường hợp nhiễm trùng vết mổ được phẫu thuật cắt lọc và không có trường hợp nào phải tháo dụng cụ. Tất cả bệnh nhân đều lành xương. Mức độ phục hồi cơ năng rất tốt và tốt theo thang điểm Majeed chiếm tỷ lệ 93,8%. Mức độ phục hồi cơ năng không liên quan đến các yếu tố như kiểu tổn thương, thời điểm can thiệp phẫu thuật và mức độ phục hồi hình thể giải phẫu. Kết luận: Gãy xương - sai khớp cùng chậu thường kèm với tổn thương các cơ quan khác trong bệnh cảnh đa chấn thương. Phẫu thuật nắn chỉnh mở, kết xương bên trong có kết quả phục hồi hình thể giải phẫu và cơ năng rất tốt.
Mục tiêu: Tìm hiểu các hình thái tổn thương của gãy xương - sai khớp cùng chậu trên phim CT scan dựng hình 3D khung chậu. Đối tượng và phương pháp: Hình ảnh CT scan khung chậu của các trường hợp sai khớp cùng chậu đơn thuần, gãy xương kết hợp với sai khớp cùng châu, được điều trị tại Khoa Chấn Thương Chỉnh hình, Bệnh viện Chợ Rẫy (từ tháng 01/2015 - 6/2019). Kết quả: Gãy mảnh hình liềm kèm sai khớp cùng chậu là tổn thương thường gặp (64,3%), trong đó kiểu Day III chiếm tỷ lệ cao nhất (41,7%). Hướng di lệch chủ yếu là ra sau, lên trên và ra ngoài (42,9%). Chúng tôi cũng ghi nhận 3 trường hợp sai khớp cùng chậu ra trước và vào trong kiểu khóa. Phần lớn các trường hợp đều đi kèm với gãy ngành ngồi mu, chậu mu, có 8/48 BN tổn thương gãy xương - sai khớp cùng chậu ở cả 2 bên. Kết luận: Nghiên cứu cung cấp một số hình thái tổn thương gãy xương - sai khớp cùng chậu, giúp chẩn đoán, phân loại gãy xương - sai khớp cùng chậu và hỗ trợ phẫu thuật viên trong việc lựa chọn phương pháp điều trị.
