The prevalence of doping in elite sports is relevant for all those involved in sports, particularly for evaluating anti-doping policy measures. Remarkably, few scientific articles have addressed this subject so far, and the last review dates back to 1997. As a consequence, the true prevalence of doping in elite sports is unknown. Even though it is virtually impossible to uncover the exact prevalence of a prohibited activity such as doping, various methods are available to uncover parts of this particular problem, which enables the circumvention (to a certain degree) of the issues of truthfulness, definition problems and the limits of pharmacological evidence. This review outlines the various methods that exist and presents the scarce data available in this area. It is concluded that a combination of questionnaires using the Randomised Response Technique and models of biological parameters is able to provide the statistical possibilities to reveal accurate estimates of this often undisclosed practice. Data gathered in this way yield an estimation of 14-39% of current adult elite athletes who intentionally used doping. These period prevalences have been found in specific sub-groups of elite athletes, and the available data suggest that the prevalence of doping is considerably different between sub-groups with varying types of sport, levels and nationalities. The above-mentioned figure of 14-39% is likely to be a more accurate reflection of the prevalence of intentional doping in elite sports than that provided by doping control test results (estimate of doping: 1-2% annually) or questionnaire-based research (estimations between 1 and 70% depending on sport, level and exact definitions of intent and doping). In the future, analytical science may play a more important role in this topic if it may become feasible to detect very low concentrations of prohibited substances in sewage systems downstream of major sporting events. However, it is clear that current doping control test results show a distinct underestimation of true doping prevalence. It does not seem feasible to distil better estimates of the prevalence of doping based on performance indicators or ego documents because of the various existing effects that influence athletic performance. Such information can only be used as extra information to augment the accuracy of prevalence rates that have been found by using other techniques. True doping prevalence studies have been scarce in elite sports so far. With the correct application of the available scientific methods, preferably using harmonised definitions of the terms 'doping' and 'elite sports', more information on this topic may be gathered in a relatively short time. This would assist anti-doping professionals in the future in order to evaluate the effects of possible anti-doping measures, and better anti-doping policies would serve athletes who compete without doping. Many governments use the trickle-down or demonstration effect to justify their high investments in an elite sport. The assumption is that elite success increases mass participation. Evidence of a relationship between elite success and mass participation is fragmented, and previous studies have mostly failed to demonstrate a connection. Nevertheless, many authors assert the need for more research on this relationship. This study examines the effect of elite sport on mass participation. The study uses Pearson's correlations to analyse the relationship between membership data and athlete success (using the elite sport index) in Flanders over the past 15 years. The membership figures from Flanders were compared with figures from the Netherlands to identify the common trends. Analysis across 20 sports revealed no consistent relationship between membership levels and success. Positive correlations were notable in Flanders in four of the eight sports in which elite Flemish competitors achieved significant international successes during the study period (athletics, gymnastics, judo and tennis). The analysis suggests that a trickle-down effect is not likely to occur automatically but might be found under specific conditions. This leads us to call for new studies that bypass the question of whether there is such a relationship and concentrate on why, how and when this relationship occurs.

This article aims to make a contribution to comparative sport research and details a method for comparing nations' elite sport systems less descriptively by measuring and comparing determinants of national competitiveness quantitatively. A mixed methods exploratory sequential design is used, consisting of two distinct phases. After qualitative exploration, a conceptual model was developed, revealing that there are nine sport policy dimensions or 'pillars' that are important for international sporting success. This article focuses on a second quantitative phase, where the model was tested in a pilot study with six sample nations to develop a scoring system. Data from each nation were collected through an overall sport policy questionnaire completed in each country, and through a survey with the main stakeholders in elite sport, namely athletes (n = 1090), coaches (n = 253), and performance directors (n = 71). Reflecting recognized principles of economic competitiveness measurement, this article demonstrates how 103 critical success factors containing quantitative and qualitative data can be aggregated into a final percentage score for the sample nations on each pillar. The findings suggest that the method is a useful way for objective comparison of nations, but it should not be isolated from qualitative descriptions and from a broader understanding of elite sport systems.

On the basis of an empirical analysis of the emergence, spread and transformation of No Holds Barred fighting contests during the 1990s, we argue that Norbert Elias's model of sportization represents a fruitful but not sufficiently differentiated framework for understanding the recent development of combat sports and fighting contests. Although the martial arts in the 20th century provide striking examples of processes of sportization and para-sportization, the rise of No Holds Barred events in the 1990s represented an opposing trend, a process of de-sportization. The analysis of No Holds Barred contests demonstrates that both sportization and de-sportization trends depend primarily on the interests of the organizers, and in particular on the degree to which they rely on the perspectives of practitioners, spectators, or viewers. The decisive factor for the predominance of the latter perspective was the formation of a new and poorly regulated market for visual material, which emerged with pay-per-view television. This allowed media entrepreneurs to commercialize nonsanctioned events, which depend primarily on the demands and fantasies of viewers who are less interested in the specifics of particular sports or games than in the antinomian excitement produced by the transgression of the rules and conventions of ordinary life. The case of No Holds Barred fighting thus suggests that new markets for visual material are likely to become an important factor in the development of spectator sports and sport-like forms of entertainment. It also suggests that regulatory regimes are an essential feature for the actual outcome of the changes that these new markets may bring about. Public pressure eventually led to the disappearance of No Holds Barred events from the major US cable television networks and from the full contact fighting scene in most Western European countries. In response, various initiatives worked towards a re-sportization of the matches, a process that has led to the transformation of No Holds Barred tournaments into Mixed Martial Arts matches.
