ГБУЗ «Московский городской научно-практический центр борьбы с туберкулезом ДЗМ», Москва, Россия Цель исследования: изучение распространенности внелегочного туберкулеза у больных ВИЧ-инфекцией в Москве и определение возможно-стей рентгеновской компьютерной томографии (КТ) в выявлении внелегочных локализаций туберкулеза у ВИЧ-инфицированных пациентов� Материалы и методы. Проанализированы данные 6 588 впервые выявленных в Москве в 2014-2015 гг� больных туберкулезом, а также проведен анализ КТ-исследований за 2014-2016 гг� 118 пациентов с ВИЧ-инфекцией и туберкулезом (ВИЧ-и/ТБ)� Результаты. С 2012 по 2016 г� снижение абсолютного числа больных внелегочным туберкулезом в Москве составило 67,5% (140 человек)� Доля внелегочных форм среди впервые выявленных больных туберкулезом в РФ и в Москве также имеет тенденцию к снижению и составляет около 3% против 4-5% в 2011-2012 гг� Среди ВИЧ-негативных больных туберкулезом доля локализаций туберкулеза в пределах легочной паренхимы составила 94,0%, а среди ВИЧ-позитивных -74,8% (p < 0,05)� Среди последних ожидаемо больше было больных с внелегочным туберкулезом (4,6% против 2,1%)� По данным регистра больных ВИЧ-и/ТБ, из 593 впервые выявленных больных было две и более локализа-ции туберкулеза у 41,3% (95%-ный ДИ 37,4-45,3%)� При оценке результатов КТ, выполненной по расширенному протоколу у 118 пациентов с ВИЧ-и/ТБ со средним уровнем CD4 Results. In Moscow, the absolute number of extrapulmonary tuberculosis patients went down by 67�5% from 2012 to 2016 (140 persons)� In Russia and in Moscow, the number of new extrapulmonary tuberculosis patients tends to decrease and currently makes approxamtely 3% versus 4-5% in 2011-2012� Among HIV negative TB patients the part of those with lesions within pulmonary parenchyma made 94%, and among the HIV positive it made 74�8% (p < 0,05)� As expected HIV infected patients prevailed among those suffering from extrapulmonary tuberculosis (4�6% versus 2�1%)� According to the data of TB/HIV register, out of 593 new TB patients, 41�3% had two and more sites of tuberculosis lesions (95% CI 37�4-45�3%)� When estimating results of advanced CT protocol in 118 patients with TB/HIV co-infection and the average CD4 + count of 134�0±19�8 cells/mcl, additional sites with tuberculous lesions were detected in all of them� Conclusion. Проанализированы результаты хирургического лечения 345 больных ВИЧ-инфекцией и туберкулезом органов дыхания, им выполнено 355 операций� Небольшие вмешательства (189/355; 53,2%) выполнены как срочные у 181 пациента, в основном с низким иммунным статусом� Плановые операции (166/355; 46,8%) выполнены у 164 пациентов, в основном по поводу туберкулеза или его осложнений� Результаты. У 345 прооперированных пациентов было 2,0% послеоперационных осложнений, связанных с операцией, 1,2% осложнений, не связанных напрямую с хирургическим вмешательством, 0,9% летальных исходов� Резекционные операции были успешно выполнены и у пациентов, не получавших антиретровирусную терапию при низком иммунном статусе� Все 3 случая летальных исходов были у пациентов с уровнем CD4 SURGICAL TREATMENT OF TUBERCULOSIS IN HIV PATIENTS M. Surgery outcomes in 345 HIV patients with respiratory tuberculosis were analyzed; totally, 355 surgeries were performed on them� Minor surgeries (189/355; 53�2%) done due to emergency reasons were performed in 181 patients, mostly in those with severe immune suppression� 164 patients had planned surgeries (166/355; 46�8%) mostly due to tuberculosis and its complications� Results.
