В различных рекомендациях в качестве препаратов выбора при урогенитальном хламидиозе наряду с доксициклином указаны джозамицин и азитромицин. Вместе с тем, учитывая накопленный опыт проведения клинических исследований, отвечающих требованиям доказательной медицины, такие рекомендации вызывают ряд вопросов, требующих как минимум уточнений. Цель исследования. Оценить доказательную базу эффективности использования различных макролидных антибиотиков в лечении урогенитальных инфекций. Материал и методы исследования. Проведен глубокий временной литературный анализ клинических результатов использования, фармакокинетических, фармакологических свойств, механизма резистентности макролидов, назначаемых при лечении урогенитальной инфекции. Результаты исследования и их обсуждение. Отмечены слабые стороны джозамицина в сравнении с азитромицином: наименьший период полувыведения, отсутствие двух пиков концентрации в крови, менее выраженное противовоспалительное действие, отсутствие суб-МПК эффекта и комплаентности. Указывается на неправомерность включения джозамицина в федеральные клинические рекомендации по лечению различных форм урогенитальной патологии, включая беременных, в качестве препарата выбора, поскольку доказательная база в отношении эффективности и безопасности азитромицина значительно шире. Заключение. Результаты свидетельствуют в пользу большей коммерческой составляющей в продвижении джозамицина на фармацевтическом рынке, поскольку отсутствуют убедительные клинические доказательства и фармакокинетические обоснования его широкого применения.
