Warthin-like tumour is a rare variant of PC, occurring predominantly in elderly women. Its histological features are distinct and well recognizable, differentiating this tumour from a more aggressive tall-cell variant of PC. The apparent indolent behaviour seems to be consistent with the presence of dendritic/Langerhans cells and with low proliferative activity. A possible role of EBV in pathogenesis of this lesion was not proven. Further studies are necessary to determine the prognosis and metastatic potential of this neoplasm.
Aims:The diff erential diagnosis of thyroid nodules in routine practice can be problemmatic for both pathologists and clinicians. Eff ective treatment requires a determination of the biological nature of the lesions. For this reason, ancilliary diagnostic markers along with histological examination of the nodules may be useful. The objective of this study was to evaluate the diagnostic usefulness of novel markers in the diagnosis of hyperplastic and neoplastic nodules.Methods: Forty eight thyroid lesions forming four diagnostic groups including adenomatous goiters (AS), follicular adenomas (FA), follicular (FC) and papillary carcinomas (PC) were examined using standard immunohistochemical methods. Monoclonal antibodies against galectin-3, matrix metalloproteinases (MMPs) -2 and -7 and endothelial markers CD31 and CD105 were used.Results: The cytoplasmatic expression of galectin-3 was positive in all cases of papillary carcinoma. Moreover, statistically signifi cant diff erences between fused groups of benign (AS and FA) and malignant lesions (FC and PC) were found Fischer's exact test (p = 0.0001). No signifi cant diff erences in cytoplasmic expression of MMPs -2 and -7 and in vascular density assessed by using of both endothelial markers between benign lesions and malignant tumors were revealed.Conclusions: Galectin-3 appears to be a useful marker in the diagnosis of papillary carcinoma only. The matrix metalloproteinases-2 and -7 are not helpful in distinguishing hyperplastic and neoplastic thyroid nodules. Endothelial markers do not appear to be suitable for thyroid diff erential diagnosis. A panel of antibodies in the diff erential diagnosis of thyroid nodular lesions would seem most suitable and further studies with larger sets of patients are awaited.
Zusammenfassung. Grundlagen: Die Tschechische Republik gehört zu den Ländern mit der höchsten Rektumkarzinominzidenz. Die Erfahrungen weisen darauf hin, dass es selten zu intramuraler Tumorausbreitung distal vom palpablen Unterrand des Rektumkarzinoms kommt, und dies meist nur bei malignen, undifferenzierten Tumortypen. Eine submuköse Ausbreitung kommt in etwa in 10 % der Fälle vor und zeigt eine schlechte, von der Art der Operation unabhängige Prognose. Ziel der prospektiven Studie war, die Ergebnisse nach chirurgischer Therapie des Rektumkarzinoms zu evaluieren. So wie es in den Ergebnissen dieses Beitrags angeführt wird, wurden gewisse onkologische Aspekte beachtet -der distale Resektionsabstand sowie die distale intramurale Tumorausbreitung.Methodik: In unserer Studie wurden jene Patienten mit Mastdarmkrebs nachuntersucht, welche innerhalb von drei Jahren (2000)(2001)(2002) in der Chirurgischen Abteilung des Fakultätskrankenhauses Ostrava operiert worden sind. Während dieser Zeit wurden an unserer Abteilung 188 Patienten mit bioptisch nachgewiesenem Rektumkarzinom operiert. Während der Studie pflegten wir eine enge Zusammenarbeit mit dem Pathologisch-anatomischen Institut des Fakultätskrankenhauses Ostrava, wo die Resektate sorgfältig entsprechend eines spezifizierten, den onkologischen Kriterien entsprechenden Protokolls untersucht wurden. Jedes Resektat wurde bezüglich mikroskopischer Tumorausbreitung distal vom makroskopisch-aboralen Rand des Tumors in Abständen von 2 mm, 5 mm, 1 cm, 2 cm und 5 cm untersucht. Diese Resultate wurden statistisch ausgewertet und mit früheren Serien verglichen.Ergebnisse: In unserer Auswertung haben wir untersucht, ob die distale submuköse Tumorausbreitung mit dem distalen Tumorabstand abnimmt. In 11,8 % der Fälle wurde eine distale submuköse Tumorausbreitung beobachtet. Eine distale Tumorausbreitung von mehr als 5 cm unter dem unteren Rand des Tumors wurde nicht beobachtet. Im Falle einer mikroskopischen Positivität unter dem unteren Rand des Tumors bestand ein mitteloder tiefdifferenziertes Adenokarzinom. Es wurde keine Ausbreitung eines gut differenzierten Adenokarzinoms gefunden.Schlussfolgerungen: Wir haben die Absicht, die Resultate unserer Studie der medizinischen Öffentlichkeit zugänglich zu machen.Summary. Background: The Czech Republic is one of the countries with the highest incidence of rectal cancer. Distal intramural spread below the palpable edge of a rectal cancer is unusual and a sign of very advanced or highly malignant disease. Submucosal spread occurs in approximately 10% of cases and indicates poor prognosis, regardless of type of operation.The aim of the prospective study was to monitor surgical treatment of patients with rectal cancer.
