The aim of the study was to investigate the serial changes in inflammatory indices derived from blood cell counts and C-reactive protein (CRP) levels in COVID-19 patients with good and poor outcomes. We retrospectively analyzed the serial changes in the inflammatory indices in 169 COVID-19 patients. Comparative analyses were performed on the first and last days of a hospital stay or death and serially from day 1 to day 30 from the symptom onset. On admission, non-survivors had higher CRP to lymphocytes ratio (CLR) and multi-inflammatory index (MII) values than survivors, while at the time of discharge/death, the largest differences were found for the neutrophil to lymphocyte ratio (NLR), systemic inflammation response index (SIRI), and MII. A significant decrease in NLR, CLR, and MII by the time of discharge was documented in the survivors, and a significant increase in NLR was documented in the non-survivors. The NLR was the only one that remained significant from days 7–30 of disease in intergroup comparisons. The correlation between the indices and the outcome was observed starting from days 13–15. The changes in the index values over time proved to be more helpful in predicting COVID-19 outcomes than those measured on admission. The values of the inflammatory indices could reliably predict the outcome no earlier than days 13–15 of the disease.

Исследование электрического заряда эритроцитов (ЭЗЭ) у больных метаболическим синдромом (МС). Материал и методы. Обследовано 112 больных МС (средний возраст 61,4±7,2 лет, средняя продолжительность МС 8,7±5,2 лет). Группу контроля (25 человек) составили относительно здоровые люди сходного возраста. Уровень ЭЗЭ определяли методом адсорбции положи-тельного катионного красителя (катионный синий О) на поверхности плазматической мембраны эритроцитов до полной нейтрализации их отрицательного заряда с последующей фотометрией раствора и расчета числа зарядов на клеточной поверхности эритроцитов. Результаты исследования обрабатывались с помощью программы Statistica 10.0. Результаты. В основной группе больных МС артериальная гипертензия выявлена у 73%, гипергликемия -у 39%, сахарный диабет -у 36%, дислипидемия -у 80%. Величина ЭЗЭ у больных МС (1,59±0,05×10 7 ) была статистически значимо ниже, чем в группе контроля (1,65±0,03×10 7 ; р<0,05). При длительности МС более 5 лет ЭЗЭ была статистически значимо ниже по сравнению с группой контроля (р<0,05). У больных с ожирением II-III степени, а также при дислипидеми показатели ЭЗЭ были статистически значимо ниже, чем в конт-рольной группе(р<0,05). У больных с МС была выявлена статистически значимая отрицательная корреляция между ЭЗЭ и уровнями общего холестерина (r=-0,51; p<0,05) и триглицеридов (r=-0,46; p<0,05). Заключение. У больных МС с увеличением длительности МС, степени ожирения и нарастания нарушений липидного обмена отмечаются статистически значимо более низкие значения ЭЗЭ.Ключевые слова: метаболический синдром, электрический заряд эритроцитов, ожирение, дислипидемия, микроциркуляция. Aim. To study the erythrocyte electric charge (EEC) in patients with metabolic syndrome (MS). Material and methods. 112 patients (mean age 61.4±7.2 years) with MS (average duration of MS 8.7±5.2 years) were examined. Control group consisted of healthy volunteers (n=25) of similar age. The level of EEC was detected by the method of adsorption of a positive cationic dye (cationic blue O) on the surface of the erythrocyte plasma membrane up to the complete neutralization of their negative charge, followed by photometry of the solution and calculation of the number of charges on the erythrocyte cell surface. The results of the study were processed by Statistica 10.0 software. Results. Hypertension was found in 73% of patients with MS, hyperglycemia -in 39%, diabetes -in 36%, dyslipidemia -in 80% of patients. The EEC in MS patients (1.59±0.05×10 7 ) was lower than this in the control group (1.65±0.03×10 7 ; p<0.05). With MS duration more than 5 years, the EEC was significantly lower in comparison with control group (p<0.05). Usually, it requires long-term monitoring of the state of many organs due to the gradual character of its manifestations. Case Report: We report a case of a long-term follow-up of a patient with eosinophilic granulomatosis with polyangiitis with emphasis on specific clinical features in this patient. A 64-year-old man was being followed up for 10 years. The initial diagnosis was allergic bronchial asthma; however, as new clinical signs and symptoms developed, the diagnosis of EGPA became obvious. A positive treatment response was seen, mainly manifested as reduced polyneuropathy. Meanwhile, bronchial asthma remained uncontrolled and bronchiectasis and Klebsiella pneumoniae colonization developed despite the combination treatment with prednisolone and methotrexate. Furthermore, the patient suffered a cerebral ischemic infarction. During the last hospital admission, severe uncontrolled bronchial asthma complicated with pneumonia resulted in the patient’s death. Conclusions: This clinical case shows the gradual development of EGPA with multiple-organ involvement, including respiratory manifestations and peripheral and central nervous system damage. Immunosuppressive treatment combined with complications of EGPA could have contributed to severe pneumonia development and death of the patient.

На долю болезней сердечно-сосудистой си-стемы приходится более чем 50% всех смертей в Рос-сии [1]. У лиц трудоспособного возраста, от состояния здоровья которых напрямую зависит благосостояние на-ции, сердечно-сосудистые заболевания (ССЗ) являют-ся причинами более трети всех смертей. Можно также говорить о «сверхсмертности» мужчин (уровень их смертности от болезней системы кровообращения в це-лом превышает таковой у женщин в 4,7 раза).Несмотря на снижение смертности от ССЗ в РФ в по-следние годы, этот показатель на 100000 населения сре-ди мужчин и женщин существенно выше, чем в стра-нах Евросоюза (573 против 81 и 173 против 28, соот-ветственно) [2]. Повышение активности ренин-ангиотензин-альдостероновой системы -один из важнейших механизмов реализации сердечно-сосудисто-го континуума. An increase in the activity of the renin-angiotensin-aldosterone system is one of the most important mechanisms for the realization of the cardiovascular continuum. The role that angiotensin receptor blockers play in achieving target figures of blood pressure and reducing cardiovascular risk is discussed. The importance of pleiotropic properties of angiotensin receptor blockers (in particular, activation of peroxisome proliferator-activated receptors gamma -PPAR-γ) in the management of patients with insulin resistance, obesity, dyslipidemia is also covered. The evidence base for the use of telmisartan as a drug with pleiotropic effect in patients with arterial hypertension and associated diseases (diabetes mellitus, obesity, renal dysfunction) is discussed.Keywords: angiotensin receptor blockers, telmisartan, hypertension, angiotensin II, cardiovascular continuum.
