Utilisation de sérum salé isotonique versus soluté balancé en unité de soins intensifs : un essai clinique avec randomisation en cluster (étude SMART) Semler MW, Self WH, Wanderer JP, et al (2018) Balanced crystalloids versus saline in critically ill adults. N Engl J Med 378:829-39 Problématique : Le chlorure de sodium isotonique à 0,9 % ou soluté salé isotonique (SSI) est utilisé pour l'expansion volémique des patients en état de choc. Ce soluté serait à l'origine d'acidose métabolique hyperchlorémique et d'insuffisances rénales aiguës [1,2]. Une alternative à ce soluté de remplissage est l'utilisation de solutés dits balancés comme le Ringer Lactate. Les données comparant ces deux solutés sont contradictoires et sont issues d'études avec de faibles niveaux de preuve [3]. Objectif : Comparer les effets cliniques de l'utilisation de SSI avec ceux de solutés balancés (SB) parmi les patients admis en unité de soins intensifs (USI). Type d'étude : Étude ouverte avec randomisation par grappes parmi cinq USI de l'hôpital universitaire de Vanderbilt (États-Unis) menée de 2015 à 2017. Pour chaque mois de l'étude, les services étaient tenus d'utiliser soit exclusivement du SSI, soit exclusivement du SB (Ringer Lactate ou Plasmalyte ®). La randomisation assignait à chaque USI un mois pair ou impair pour l'utilisation du SSI et donc l'inverse pour le SB. Les trois USI recevant préférentiellement les patients de la structure des urgences (SU) étaient randomisés à l'identique tout comme le bloc opératoire adressant les patients préférentiellement dans les deux autres USI. Ont été inclus tous les patients de plus de 18 ans lors de leur admission en USI. Le critère de jugement principal était un critère composite nommé « effet rénal majeur » (ERM) composé du décès, d'une nécessité de dialyse de novo ou d'une insuffisance rénale aiguë persistante. Les critères de jugement secondaires étaient le décès en USI, à 30 ou 60 jours, le nombre de jours vivant sans ventilation, sans amines, sans dialyse durant les 28 jours suivant l'inclusion. Résultats principaux : Durant la période, 15 802 patients ont été inclus dont 7 942 dans le groupe SB et 7 860 dans le groupe SSI. Il n'y avait pas de différence dans les caractéristiques générales à l'admission entre les patients. Les volumes médians administrés dans les groupes SB et SSI ont été respectivement de 1 000 ml (intervalle interquartile : [0−3210]) et de 1 020 ml [0−3500]. Concernant l'objectif principal, 14 % dans le groupe SB et 15 % dans le groupe SSI ont présenté un EMR (odds ratio (OR) = 0,90 ; intervalle de confiance à 95 % : [0,82−0,99] ; p = 0,04). Concernant les objectifs secondaires, aucune différence n'était statistiquement significative. Dans une analyse en sous-groupe, l'utilisation de SB était significativement associée à moins d'EMR par rapport au SSI parmi les patients pris en charge en USI à orientation médicale (OR = 0,87 : [0,77−0,99]) et les patients présentant un sepsis (OR = 0,80 : [0,67−0,94]).
