У статті оцінено дистрибуцію генотипів полі-морфізму А1166С гена рецептора ангіотензина II першого типа у хворих на артеріальну гіпертензію у поєднанні з неалкогольною жировою хворобою печінки. У результаті встановлено, що у групі хво-рих із поєднаним перебігом артеріальної гіпертен-зії та неалкогольної жирової хвороби печінки носія-ми генотипів А/А, А/С і С/С були 16 (29,09 %), 29 (52,72 %) і 10 (18,19 %) відповідно. Таким чи-ном, за результатами даного дослідження, генотип А/С гена рецептора ангіотензина II першого типа (А1166С) було асоційовано із поєднаним перебігом артеріальної гіпертензії та неалкогольної жирової хвороби печінки, тоді як протективні властивості А/А генотипу, навпаки, послаблювались.Ключові слова: артеріальна гіпертензія, неал-когольна жирова хвороба печінки, поліморфізм гена рецептора ангіотензина II першого типа.Зв'язок роботи з науковими програмами, планами, темами. Тісний зв'язок між захворюваннями сер-цево-судинної системи та захворюваннями печінки обумовлений безліччю різних факторів із різними механізмами, спільна односпрямована дія яких призводить до погіршення перебігу захворювань обох систем органів. За рахунок кардіологічної па-тології ураження печінки може бути як первинним (розвиваються незалежно), так і вторинним (внас-лідок погіршення серцево-судинної патології, відсу-тності корекції дисліпідемії, ожиріння, цукрового діабету) [1]. Тому своєчасна діагностика артеріаль-ної гіпертензії (АГ) у хворих із супутньою неалкого-льною жировою хворобою печінки (НАЖХП) є акту-альною задачею терапії.Найбільш поширеним способом діагностики АГ у хворих із супутньою НАЖХП є виміри та аналіз загально-клінічних та інструментальних обстежень [10]. Проте, сьогодні особливої актуальності набу-ває дослідження поліморфізмів генів, що пов'язані з розвитком та прогресуванням багатьох захворю-вань і можуть бути використані як маркери ранньої досимптомної діагностики коморбідних нозологій.Одним із генних поліморфізмів, який істотно впливає на перебіг АГ, є А1166С поліморфізм гена рецептора ангіотензина II першого типа (АGТR1). Щодо поширення А1166С поліморфізму гена АGТR1 в українській популяції, то, слід відмітити істотну варіабельність частот різних алелей та ге-нотипів цього поліморфізму між мешканцями захід-них і східних областей України: так, за даними Си-дорчук Л.П., алель А серед здорових мешканців Буковини зустрічався в 72,0%, алель С -в 28,0 %, генотип А/А -в 52,0 %, А/С -в 40,0 %, а генотип С/С -тільки в 8,0 % випадків [2]. Оцінювання показників кардіогемодинаміки у хворих на артеріальну гіпертензію та неалкогольну жирову хворобу печінки залежно від генотипів поліморфізму А1166С гена рецептора ангіотензину II першого типу О. Обстежили 55 хворих на артеріальну гіпертензію та неалкогольну жирову хворобу печінки. Досліджен-ня алельного поліморфізму А1166С гена рецептора ангіотензину II першого типу виконали методом полімеразної ланцюгової реакції з електрофоретичною детекцією результатів, використовуючи набори реактивів «SNP-ЕКСПРЕС» виробництва ТОВ НВФ «Літех» (РФ). Виділення ДНК із цільної крові виконали за допомогою комерційного набору «ДНК-сорб-В» виробництва «ІнтерЛабСервіс» (РФ) відповідно до інструкції. Правильність розподілу частот генотипів визначали відповідністю рівноваги Харді-Вайнберга (pi 2 + 2 pipj + pj 2 = 1). Згідно з Гельсінкською декларацією, всі пацієнти були поінформовані про клінічне дослідження та дали згоду на визначення поліморфізму досліджуваного гена.Результати. У підгрупі хворих на артеріальну гіпертензію та неалкогольну жирову хворобу печінки з генотипом С/С гена рецептора ангіотензину II першого типу значення кінцевого діастолічного об'єму перевищувало таке на 13,45 % і 28,31 %; кінцевого систолічного об'єму -на 22,41 % і 39,09 %; кінцевого діастолічного розміру -на 10,67 % і 38,95 %; кінцевого систолічного розміру -на 15,63 % і 29,11 %; маси міокарда лівого шлуночка -на 11,18 % і 19,38 %, а фракція викиду була нижчою порівняно з хворими з генотипами А/С і А/А відповідно (p < 0,05). У результаті дослідження не виявили відмінностей щодо діастолічної дисфункції лівого шлуночка залежно від генотипів поліморфізму А1166С гена рецептора ангіотензину II першого типу в обстежених.Висновки. Алель С поліморфізму А1166С гена рецептора ангіотензину II першого типу у хворих на артеріальну гіпертензію та неалкогольну жирову хворобу печінки асоційована з морфофункціональними змінами в серці, а саме прогресом гіпертрофії, збільшенням розмірів, об'ємів порожнини лівого шлуночка, зменшенням здатності міокарда до скорочення.
