Фертильность-сложное физиологически и поведенчески детерминированное явление. Естественная способность к зачатию и вынашиванию обусловлена возрастом. Женская репродуктивная система отличается от других систем организма тем, что имеет более высокую скорость старения. Исследования последних нескольких десятилетий значительно расширили знания о репродуктивной системе женщины, что позволило сформировать представления о персональном биологическом возрасте по состоянию яичников-овариальному резерву. Овариальный резерв составляет основу репродуктивного потенциала, определяет функциональный запас яичников, их способность к формированию здорового фолликула с полноценным овоцитом, возможность адекватного ответа на стимуляцию гормонами в программах вспомогательных репродуктивных технологий. Существует большое количество маркеров оценки овариального резерва и более 20 моделей прогнозирования исходов применения вспомогательных репродуктивных технологий. Для решения вопроса о принятии мер по дальнейшему управлению сниженной функцией яичников необходима комплексная оценка состояния репродуктивной системы.
