The search for molecular markers to improve diagnosis, individualize treatment and predict behavior of tumors has been the focus of several studies. This study aimed to analyze homeobox gene expression profile in oral squamous cell carcinoma (OSCC) as well as to investigate whether some of these genes are relevant molecular markers of prognosis and/or tumor aggressiveness. Homeobox gene expression levels were assessed by microarrays and qRT-PCR in OSCC tissues and adjacent non-cancerous matched tissues (margin), as well as in OSCC cell lines. Analysis of microarray data revealed the expression of 147 homeobox genes, including one set of six at least 2-fold up-regulated, and another set of 34 at least 2-fold down-regulated homeobox genes in OSCC. After qRT-PCR assays, the three most up-regulated homeobox genes (HOXA5, HOXD10 and HOXD11) revealed higher and statistically significant expression levels in OSCC samples when compared to margins. Patients presenting lower expression of HOXA5 had poorer prognosis compared to those with higher expression (P=0.03). Additionally, the status of HOXA5, HOXD10 and HOXD11 expression levels in OSCC cell lines also showed a significant up-regulation when compared to normal oral keratinocytes. Results confirm the presence of three significantly upregulated (>4-fold) homeobox genes (HOXA5, HOXD10 and HOXD11) in OSCC that may play a significant role in the pathogenesis of these tumors. Moreover, since lower levels of HOXA5 predict poor prognosis, this gene may be a novel candidate for development of therapeutic strategies in OSCC.
Objectives Investigate the biochemistry of in vivo healthy oral tissues through Raman spectroscopy. We aimed to characterize the biochemical features of healthy condition in oral subsites (buccal mucosa, lip, tongue, and gingiva) of healthy subjects. More specifically, we investigated Raman spectral characteristics and biochemical content of in vivo healthy tissues on Brazilian population. This characterization can be used to better define normal tissue and improve the detection of oral premalignant conditions in future studies. Materials and methods For spectroscopic analysis a Raman spectrometer (Kaiser Optical Systems imaging spectrograph Holospec, f / 1.8i-NIR) coupled with a laser 785 nm, 60 mW was used. Raman measurements were obtained by means of an optical fiber (EMVision fiber optic probe) coupled between the laser and the spectrometer. Three spectra per site were acquired from the lip, buccal mucosa, tongue, and gingiva of ten healthy volunteers. This resulted in 30 spectra per oral sub-site and in total 120 spectra.Results We report detailed biochemical information on these subsites and their relative composition based on deconvolution studies of their spectra. Finally, we also report classification efficiency of 61, 83, 41, and 93% for buccal, gingiva, lip, and tongue respectively after applying multivariate statistical tools. Conclusions We quantitated the contribution of various biochemicals in terms of percentage, and this will enable comparison not only across anatomical sites but also across studies. Raman spectroscopy can rapidly probe tissue biochemistry of healthy oral regions. Moreover, the study suggests the possibility of using Raman spectroscopy combined with signal processing and multivariate analysis methods to differentiate the oral sites in healthy conditions and compare with pathological conditions in future studies. Clinical relevance The spectral characterization of the healthy condition of oral tissues by a noninvasive, label-free, and real-time analytical techniques is important to create a spectral reference for future diagnosis of pathological conditions.
OBJETIVO: Este trabalho tem como objetivo avaliar a influência dos fatores prognósticos em portadores de carcinoma papilífero da tireóide, tratados no Hospital do Câncer (INCA-RJ). MÉTODO: Com base em testes estatísticos (Wilcoxon e Cox) foram analisados 126 prontuários de pacientes atendidos no período de 1986 a 1994, portadores de carcinoma papilífero da tireóide, pertencentes ao grupo de alto risco, segundo os fatores de risco do carcinoma diferenciado da tireóide, considerando uma sobrevida de dez anos livre de doença. RESULTADOS: Observou-se que 104 pacientes eram mulheres (83%); a idade variou de sete a 79 anos, média de 40 anos; invasão capsular ocorreu em 15% (18/126); houve metástase regional em 38% (47/126) e metástase a distância em 11% (13/126). A sobrevida em dez anos livre de doença foi de 81% para os pacientes com menos de 45 anos, e de 76% para os mais idosos: p = 0,0008 (análise univariada) e p = 0,01 (análise multivariada). Dos pacientes que tinham invasão capsular, 72% viveram dez anos, assim como 60% dos que tinham metástase regional, e 28% dos que apresentavam metástase a distância. CONCLUSÕES: A utilização dos fatores de risco no carcinoma papilífero da tireóide é válida mesmo para doença avançada, sendo também de grande importância na projeção do prognóstico e do futuro desenvolvimento da doença.
Caracteristicamente os estágios avançados (III e IV) do câncer da laringe são tratados inicialmente por cirurgia, seguida de radioterapia complementar. Esta tem sido a combinação terapêutica que tem obtido os melhores resultados, com relação à maior sobrevida e ao maior tempo livre de doença, nestes pacientes. Porém, como freqüentemente a cirurgia a ser adotada nestes casos representa uma mutilação, perda da voz laríngea, nem sempre esta opção de tratamento é aceita pelos pacientes. Por outro lado, pacientes portadores de doença em estágio avançado podem encontrar-se com suas condições clínicas bastante comprometidas, o que impossibilita a realização de uma cirurgia de grande porte. A associação de quimioterapia e radioterapia tem tido pouca utilidade neste tipo de pacientes, pois geralmente apresentam estado clínico geral comprometido e não suportam a alta toxicidade dos esquemas terapêuticos empregados. Nestes casos a indicação de radioterapia exclusiva, como forma de tentar controlar a doença, tem sido a única arma terapêutica que nos resta, e cujos resultados sempre foram uma incógnita no nosso meio. Foram estudados 62 pacientes portadores de carcinoma epidermóide avançado (estágios III e IV) da laringe, tratados por radioterapia exclusiva, no Hospital do Câncer (INCa), durante os anos de 1992 e 1993. Foi realizado tratamento radioterápico com intenção curativa, sendo empregada a dose clássica de 50cGy em cinco semanas. Estes pacientes foram retrospectivamente analisados quanto à sobrevida, ao estágio da doença, à presença de linfonodos cervicais metastáticos, à idade e à necessidade de traqueostomia prévia. Além da constatação da total ineficácia da radioterapia no controle da doença laríngea estágio IV, detectou-se que a sobrevida de três anos igual a 54% para os pacientes do estágio III e a preservação da função da laringe em 2/3 dos pacientes, poderão representar, dependendo de estudos mais abrangentes, uma alternativa terapêutica para pacientes selecionados.
