The article analyzes the features of coronary arteries changes in patients with acute myocardial infarction (AMI) depending on the presence or absence of concomitant diabetes mellitus (DM) type 2. 60 patients with AMI and concomitant DM type 2 who were treated at the infarctional department of Kharkiv City Clinical Hospital No. 27 (Kharkiv, Ukraine) and cardiology department of the Kharkiv Clinical Hospital on Railway Transport №1 (Kharkiv, Ukraine) were investigated. In order to assess the status of the coronary flow, all patients in both groups underwent coronary angiography. The proportion of cases with multi-vessel lesions of coronary arteries exceeded the proportion of cases with one and twovessel lesions of coronary arteries in patients with acute myocardial infarction and type 2 diabetes.
Worldwide, ischaemic heart disease is the single most common cause of death and its frequency is increasing. However, in Europe, there has been an overall trend for a reduction in ischaemic heart disease mortality over the past three decades [1]. Ischaemic heart disease now accounts for almost 1.8 million annual deaths, or 20 % of all deaths in Europe, although with large variations between countries [2, 3].Basu et al. found that althrought IL-22 was secreted by various types of cells, Th22 (CD4 + IL – 22 + T) cells were the critical source of IL-22 during the later stages of inflammation, indicating that Th22 may be essential for controlling chronic inflammation. The effector cytokine of Th22 cells is IL-22, which belongs to the IL-10 cytokine family and binds to the heterodimeric receptor complex consisting of the IL-10 receptor (IL-10R) β chain and IL-22R.
Мета дослідженняоцінити зміни про-та протизапальних параметрів цитокінового статусу на підставі визначення активності кальпротектину й інтерлейкіну-22 у хворих на гострий інфаркт міокарда в залежності від наявності або відсутності супутнього цукрового діабету 2 типу, а також проаналізувати зв'язки між цитокінемією та показниками вуглеводного метаболізму у досліджуваних когортах хворих.Обстежено 110 хворих (середній вік 65,25±0,09 років), які знаходились на лікуванні у інфарктному відділенні харківської міської клінічної лікарні № 27 та харківської клінічної лікарні на залізничному транспорті № 1 До основної групи було залучено 64 хворих (середній вік 65,31±1,62 років) на гострий інфаркт міокарда із супутнім цукровим діабетом 2 типу. Групу зіставлення склали 46 хворих на ізольований гострий інфаркт міокарда (середній вік 65,19±1,22 років).Концентрація глюкози сироватки крові визначали глюкозооксидазним методом. Визначення рівня інсуліну здійснювали імуноферментним методом з використанням тест-системи EIA-2935, Insulin ELISA. Визначення рівня кальпротектину сироватки крові здійснювали імуноферментним методом з використанням тест-системи MRP8/14 ELISA KIT. Визначення рівня інтерлейкіну-22 сироватки крові здійснювали імуноферментним методом з використанням тест-системи Human IL-22 ELISA KIT.Отримані зв'язки між кальпротектином та ІЛ-22 у хворих на ізольований гострий інфаркт міокарду свідчать про високу активність як про-, так і протизапальної ланки цитокінового статусу. У хворих з гострий інфаркт міокарду 2 типу гіперактивність прозапального індикатора кальпротектину асоціювалась із зниженням протизапального ІЛ-22, що дає змогу стверджувати про відсутність компенсаторної реакції у даної когорти хворих.Ключові слова: цукровий діабет 2 типу, гострий інфаркт міокарду, інтерлейкін-22, кальпротектин сироватки крові.Зв'язок роботи з науковими програмами, планами, темами. Nowadays, an acute myocardial infarction is one of the leading causes of mortality among the population. The EHS-DH registry data clearly illustrate the association between the comorbidities and high mortality following acute myocardial infarction during a year period of follow up. The pronounced influence of carbohydrate metabolism disturbances on the survival of such patients has already been reported. The aim of the study was to analyze the immune inflammation relationships based on assessing calprotectin and the parameters of lipid and carbohydrate metabolism, to evaluate the presence and nature of the relationship between these parameters and carbohydrate metabolism parameters based on the study of blood glucose, insulin and insulin resistance (by the indices HOMA, QUICKI, Caro), anthropometric indicators and inflammatory indicators (monocyte and neutrophile levels). Materials and methods. The study included 64 patients (mean age 65, 31 ± 1.62 years) with acute myocardial infarction and concomitant diabetes mellitus type 2. The design of the study included the primary laboratory investigation of patients during the first day since the onset of acute myocardial infarction with the elevation of the ST segment before the initiation of thrombolytic therapy or percutaneous intervention. The direct correlation between the calprotectin concentration and the HOMA insulin resistance index (R = 0.52; p <0.05), insulinemia (R = 0.57; p <0.05), fasting glycaemia (R = 0, 59; p <0.05), as well as inverse correlation relationships between the Caro index (R = 0.68; p <0.05) and the QUICKI index (R = 0.59; p <0.05) were found out. Moreover, a direct correlation between calprotectin and triglyceride levels (R = 0.31; p <0.05), and negative correlation with high density lipoprotein (R = 0.35; p <0.05) was established as well. The level of total cholesterol and low density lipoproteins showed no significant association with the proinflammatory factor (R = 0.12; p> 0.05 and R = 0.18; p> 0.05, respectively). Conclusions. The increase in the body mass index and the activity of serum monocytes and neutrophils is associated with high concentrations of calprotectin that is accompanied by disturbances of carbohydrate homeostasis towards the growth of insulin resistance and changes of lipidograms of proatherrogenic nature.
