For the first time in Ukraine, the article describes the clinical case, features of diagnostic examinations and the difficulty of the verification of Erdheim—Chester disease (ECD) of a 13-year-old girl. The disease debuted with arthritis of the right knee and right ankle joints. Subsequently, there was weakness in the legs, limping, it was difficult for the child to climb the stairs. Magnetic resonance imaging (MRI) of the brain in the supra-, intratentorial and intracerebral areas revealed numerous focal lesions of the cortex, the white medulla of both hemispheres of the brain and the left hemisphere of the cerebellum. 8 months after the initial complaints, she was diagnosed with lower flaccid paraparesis, mixed paresis of the right hand, atactic syndrome, she could not stand on her toes. Based on the results of computer tomography (CT), a widespread focal lesion of the bones, the central nervous system (including the brain, membranes of the spinal cord, cranial nerves, roots and spinal nerves), lungs, liver, pancreas, kidneys, abdominal lymph nodes, peritoneum, uterus, vagina, mammary glands. Scintigraphic signs of diffuse lesions of the bones of the skull, spine, pelvis, tubular bones of the extremities. After 9 month after the appearance of primary complaints the ECD diagnosis was confirmed using histological, immunohistochemical and molecular genetic studies. Histiocytic cells were positive for antigens CD68 and CD14, and negative for CD1a, CD207, S-100 and BRAFV600E. Our observation shows that this pathology can occur in childhood of a female person. The rarity of this disease and the variety of clinical and pathological features are a problem for clinicians and pathologists. Diagnosis of the disease is difficult and long even with the involvement of a wide range of specialists and various examination methods (ultrasound, radiography (RTG), MRI, CT. We hope that familiarity with the wide range of pathological changes in ECD described in this message, will help practicing clinicians and pathologists diagnose disease in the early stages and effectively provide professional medical care to such patients. Rett syndrome is one of the most common causes of mental retardation in girls. The aim of our work was to study a spectrum of genetic heterogeneity and various clinical manifestations of Rett syndrome in girls Western Ukraine. Materials and methods: there were used clinical, molecular and genetic methods. We observed seven girls with Rett syndrome aged from 6 months to 15 years who were diagnosed and followed-up at the Institute of Hereditary Pathology, National Academy of Medical Sciences of Ukraine, Lviv for three years (2019–2021) and underwent molecular genetic analyses confirmed by next-generation sequencing. Results: In this study, patients with Rett syndrome had individual clinical heterogeneity and age variability due to different mutations. Mental retardation was not observed among siblings in families with Rett syndrome. We identified seven different pathogenic mutations among seven girls, including two deletions and one duplication of the MECP2 gene. Microcephaly was observed in two girls with MECP2 c.880C>T (p.Arg294*) and MECP2 Gain (Entire coding sequence) at birth. The following developmental disabilities were found in five girls: lack of independent sitting, lack of independent gait (regression of development). Among musculoskeletal disorders, there were diagnosed scoliosis, X-shaped deformation of the lower extremities and muscular hypotonia. A two-year-old girl with Rett syndrome, along with other clinical symptoms, had breathing problems - hyperventilation (rapid shallow breathing). Conclusions: in cases with unknown cause of delayed developmental disability and mental retardation the patients should be referred for medical genetic counselling.
Синдром Альперса Гуттенлохера-рідкісне та важке мітохондріальне захворювання, яке виникає внаслідок виснаження мітохондріальної ДНК (мтДНК) та обумовлене мутаціями у гені POLG1. Характеризується тріадою симптомів-прогресуючою регресією розвитку, важкими судомами та печінковою недостатністю. Хвороба невпинно прогресує і часто призводить до смерті внаслідок печінкової недостатності або епілептичного статусу протягом чотирьох років від початку захворювання. Лікування епілепсії препаратами вальпроєвої кислоти призводить до швидкого початку печінкової недостатності, тому його слід уникати. У статті описано спостереження 8 місячної дитини із міоклонічною епілепсією та печінковою недостатністю, яка виникла на тлі лікування вальпроєвою кислотою із летальним наслідком. Діагноз підтверджено молекулярно генетичним дослідженням post mortum. Дослідження виконані відповідно до принципів Гельсінської Декларації. Протокол дослідження ухвалений Локальним етичним комітетом (ЛЕК) всіх зазначених у роботі установ. На проведення досліджень було отримано поінформовану згоду батьків дитини. Автори заявляють про відсутність конфлікту інтересів. Ключові слова: мітохондріальна хвороба, синдром Альперса-Гуттенлохера, вальпроати.
На фоне практически мировой пандемии недостаточности витамина D среди населения планеты, и особенно у детей, необходимо привлечь внимание врачей к состояниям, сопровождающимся гипервитаминозом D и тяжелыми нарушениями фосфорно-кальциевого обмена, которые приводят к опасным последствиям. К ним относится идиопатическая инфантильная гиперкальциемия. Идиопатическая инфантильная гиперкальциемия — редкостное наследственное аутосомно-рецессивное заболевание, связанное с нарушением инактивации активных метаболитов витамина D вследствие инактивирующих мутаций в гене CYP24A1. В статье приведен клинический случай гипервитаминоза D, гиперкальциемии, гипопаратиреоза, развития нефрокальциноза у ребенка раннего возраста на фоне инактивирующих мутаций гена CYP24A1.
Синдром Нунан (СН)-відносно поширений синдром множинних вроджених вад розвитку. Основними симптомами є постнатальна затримка росту, характерні дисморфії обличчя та широкий спектр вроджених вад серця. Найпоширенішими вродженими вадами серця є стеноз легеневої артерії та гіпертрофічна кардіоміопатія. Метою роботи було встановити діагноз у дитини із вродженими вадами серця, важкою гіпертрофічною кардіоміопатією, легеневою гіпертензією та стигмами дизембріогенезу. Мутації в гені RAF1 викликають 3-17% випадків синдрому Нунан і в 95% випадків асоціюються з важкою гіпертрофічною кардіоміопатією. Синдром Нунан є одним із групи споріднених станів, відомих під назвою RASopathie. RASopathies пов'язані з наявністю мутацій в генах зародкової лінії, які кодують специфічні білки шляху RAS/mitogen activated protein kinase (MAPK), що відіграє вирішальну роль в ембріональному і постнатальному розвитку. Наведено клінічне спостереження синдрому Нунан внаслідок мутації p.Ser257Leu у гені RAF1 із швидко прогресуючою гіпертрофічною кардіоміопатією та летальним випадком у немовляти. Hemophilia A is an X-linked recessive disorder caused by a deficiency of plasma coagulation FVIII, which may be inherited or arise from a spontaneous mutation. FVIII deficiency leads to a decrease in normal hemostasis and is manifested by spontaneous or induced bleeding. As a result of hemorrhages in the central nervous system, neurological complications are possible. In such cases, doctors should be on the alert so as not to miss another accompanying pathology. Neurodegenerative disease with iron accumulation in the brain is a genetically and clinically heterogeneous group of hereditary progressive disorders of the central nervous system with pronounced iron accumulation in the basal ganglia, which have a specific picture on magnetic resonance imaging of the brain in combination with characteristic clinical signs. Purpose - is to describe a clinical case of a combination of two complex hereditary diseases in a 10-year-old boy, hemophilia A of moderate severity, complicated by an inhibitor, and a progressive neurodegenerative disease with accumulation of iron in the brain, with associated neurodegeneration associated with the protein of the mitochondrial membrane. The publication reports for the first time a clinical case of a combination of two complex hereditary diseases in a 10-year-old boy, confirmed by molecular genetic studies: hemophilia A of moderate severity, complicated by an inhibitor with the detection of a large deletion of exons 23-26 in the gene, and progressive neurodegeneration with brain iron accumulation, with the presence of a pathogenic mutation of the C19orf12 gene, variant c.204_214del (p.Gly69Argfs*10) in a homozygous state, autosomal recessive type of inheritance, Mitochondrial-membrane Protein-Associated Neurodegeneration. Coagulopathy is controlled by prophylactic administration of emicizumab subcutaneously. Neurodegeneration with brain iron accumulation in the child was manifested by: Friedreich's foot, equinus feet, positive Babinski symptom, high tendon reflexes, optic nerve atrophy; partial dysplasia of both eyes; with myopia of both eyes, impaired accommodation, progressively increasing paresthesias in both legs, impaired gait, ataxic gait, coordination difficulties, muscle atrophy of both legs, visual impairment, rapid fatigue with preserved intelligence and mental development. Magnetic resonance imaging of the brain showed a moderate bilateral symmetrical lesion of the globus pallidus. Our report confirms that the use of molecular genetic studies plays an important decisive role in the verification of the disease, often determining its type and possible complications. Синдром «котячого ока» без атрезії ануса, з ампліфікацією генів ADA2 та IL17RA (клінічний випадок) 1 ДУ «Інститут спадкової патології НАМН України», м. Львів, Україна 2 Західноукраїнський спеціалізований дитячий медичний центр, м. Львів, Україна УДК 575.224+576.316 Синдром «котячого ока»-рідкісна хромосомна патологія із дуже різноманітною клінічною картиною. Характерними симптомами синдрому є колобома райдужної оболонки ока, преаурикулярні шкірні тяжі або фістули, аномалії ануса, вроджені вади серця та нирок. Мета: уточнення діагнозу у дитини із вродженою вадою серця та стигмами дизембріогенезу. Діагноз ґрунтується на виявленні додаткової маркерної хромосоми, яка походить з 22 хромосоми. Наведено клінічне спостереження 4 річної дівчинки із синдромом «котячого ока» без атрезії ануса. Діагноз встановлено методом секвенування нового покоління (next generation sequencing, NGS). У регіоні 22q11.1 ідентифіковано чотири копії генів ADA2 та IL17RA. Представлений випадок демонструє кореляцію між генетичною перебудовою та проявами фенотипу. Подвоєння не цілого плеча dup(22)(q11), а лише проксимального регіону довгого плеча 22 хромосоми, де локалізовані дупліковані гени ADA2 та IL17RA, зумовлює такі прояви фенотипу: ВВС, преаурикулярні шкірні тяжі, колобому райдужної та судинної оболонок та сітківки ока, затримку мовленнєвого розвитку, імунодефіцит, без атрезії ануса. Tyrosinemia is a rare metabolic disease resulting from a metabolic disorder of amino acids, which can occur under the «masks» of various diseases, debut as a hemorrhagic syndrome in children of all age groups, hepatolienal syndrome, hypoglycemia, rickets-like disease, peripheral neuropathy. Clinical case. In our publication, we report on a girl at the age of 1 year 7 months who had recurrent nosebleeds, which led to a referral to a hematologist. Examination revealed hepatomegaly with impaired liver function (hypoproteinemia, long-term resistant hypoglycemia, coagulopathy) with the development of chronic liver failure, ascites and splenomegaly with signs of hypersplenism, ascites, and nephromegaly. Differential diagnostics was carried out between oncohematological process, myelodysplastic syndrome (MDS) and metabolic disease. Type 1 tyrosinemia (hereditary infantile tyrosinemia (HT-1)) was verified by a combination of clinical and biochemical, molecular genetic studies. Verification of the disease came from the spectrometry of amino acids, acylcarnitines, succinal acetates and molecular genetic studies. Molecular genetic studies in the INVITAE laboratory, USA revealed two pathogenic variants identified in the FAH gene c.1069G>T (p.Glu357*) and c.554-1G>T, which are associated with autosomal recessive tyrosinemia. The emphasis in the publication is on the differential diagnosis, the effectiveness of the treatment of this orphan disease. The method of pathogenetic therapy of HT-1 is described in detail, both with the use of the drug nitisinone (orphadin) registered in Ukraine, a special diet with a low content of phenylalanine / tyrosine, which have a pronounced positive clinical effect and prevent the formation of irreversible disabling disorders. We emphasize the need for early diagnosis of HT-1 and support the Ministry of Health of Ukraine in the initiative of routine neonatal screening for orphan diseases, which include HT-1, since timely treatment improves the quality of life in these patients.
