Новокузнецкий государственный институт усовершенствования врачей, г. Новокузнецк Изучена чувствительность 457 госпитальных штаммов микроорганизмов, выделенных от больных отделения реанимации и интенсивной терапии «ГКБ №1», к антибиотикам диско-диффузионным методом. Изоляты Acinetobacter spp. и Pseudomonas aeruginosa обладали высокой резистентностью к большинству антибактериальных препаратов, но были чувствительны к нетилмицину, меропенему и полимиксину В. При инфекциях, вызываемых представителями семейства Enterobacteriaceae, для лечения можно использовать карбапенемы (меропенем и имипенем), цефалотин и фторхинолоны. Enterococcus spp. сохраняли 100% чувствительность к линезолиду, в 64,7%-к офлоксацину, в 61,8%-к ванкомицину и в 54,3%-к ципрофлоксацину. Staphylococcus aureus оксациллинорезистентные составили 72,6%, они в 100% случаев были чувствительны только к линезолиду, в 96,7%-к фузидиевой кислоте и новым аминогликозидным антибиотикам-нетилмицину (79,3%) и амикацину (65,2%). Среди коагулазоотрицательных стафилококков не было выделено резистентных штаммов к гликопептидным антибиотикам и цефалотину, оксациллиноустойчивыми оказались 38,5% культур. Ключевые слова: госпитальные инфекции, микробиологический мониторинг возбудителей, антибиотикорезистентность Госпитальные инфекции (ГИ) и гнойно-воспалительные осложнения в отделении реанимации и интенсивной терапии (ОРИТ) остаются одной из существенных причин неблагоприятных исходов, удлинения сроков госпитализации и увеличения финансовых затрат на лечение. По данным многоцентрового когор-тного исследования [5], частота инфекций различной локализации в ОРИТ составила 18,9%, при этом в 65% случаев инфекции относились к госпитальным. Летальность среди пациентов без признаков инфекции в приведенном исследовании составляла 16,9%, а среди пациентов с инфекциями достигала 53,6%. Большинство инфекций в ОРИТ имеет полимикробный характер и вызывается «проблемными» полирезистентными штаммами микроорганизмов-энтеробактериями, синегнойной палочкой, ацинетобактериями, золотистым и коагулазонегативным стафилококками и энтерококками [1,2,4,5,6]. Наиболее опасным и проблемным госпитальным патогеном является Pseudomonas aeruginosa, имеющим множественные и быстро формирующиеся (на этапе первой линии антимикробной терапии) механизмы резистентности, в том числе и к препаратам, обладающим антисинегнойной активностью [1].
During the period 2006-2007 there were allocated and analysed 457 clinical shtamms of activators of heavy nozocomial infections. The leaders of pathogens at various forms of intrahospital complications are the specimens of gramm negative microflorae - Acinetobacterses spp. (16,63 %) and Pseudomonas aeruginosa (14,44 %), the enterococcus were allocated in 13,35 % of cases, the clebsiels – in 11,6 %, the intestinal stick and coagulazo-positive staphylococcus were met practically with identical frequency (9,63 % and 8,75 % accordingly). It is necessary to note enough the high densities of mushrooms of sort Candida (6,35 %). It was noted the statistically significant prevalence of microbic associations among the bacteria grammpositive and grammnegative, the hospital infectious process was confirmed. The seasonal prevalence of shtamms allocation is analysed: the peak of purulent – septic complications in ОRIT is in years months - June (17,3 %) and August (10,0 %), and also for April and October (9,8 % and 9,5 % accordingly); the minimal diseases is registered in winter months. It is more often the nozocomial infection among the young men and women of the most efficient age (20-45 years).
