Zusammenfassung:Gegenstand und Ziel: Luft und Gas wirken sich bei der sonographischen Untersuchung des Abdomens in der Regel negativ auf die diagnostische Wertigkeit des Verfahrens aus. In bestimmten Fällen kann das Gas jedoch die Diagnosestellung erheblich erleichtern oder sogar erst möglich machen. Diese Arbeit bietet eine Zusammenstellung der pathologischen Befunde, die sich durch das Vorhandensein von Gas besser diagnostizieren lassen. Material und Methode: Anhand klinischer Beispiele wird beschrieben, welche Vorund Nachteile sich aus der Anwesenheit der Luft für die Abdominalsonographie ergeben. Ergebnisse: Bei Magenund Darmtumoren erlaubt die intraluminale Gasansammlung eine eindeutige Organzuordnung zum Magen bzw. Darm. Gasbläschen in der Gallenblase sind ein Anzeichen für eine hochgradige entzündliche Veränderung und erfordern in der Regel ein chirurgisches Eingreifen. Gaseinlagerungen in den Wänden von Hohlorganen weisen auf eine Infiltration der Wandung hin (Darm: Ulkus, Harnblase: emphysematöse Zystitis). Der Nachweis von freiem Gas in der Bauchhöhle stellt bei nichtiatrogenem Ursprung eine Indikation für eine diagnostische Laparotomie dar. Durch den Einsatz von Gas als Kontrastmittel kann eine Harnblasenruptur einfach diagnostiziert werden. Schlussfolgerung: Das Vorhandensein von Gas im Abdomen lässt sich diagnostisch nutzen. In einigen Fällen beeinflusst der Nachweis von Gas die Therapie.
Zusammenfassung Gegenstand und Ziel: Das postanästhetische Nierenversagen ist eine gefürchtete Komplikation einer Narkose. Nicht wenige Patienten leiden an einer kompensierten Niereninsuffizienz, die durch zusätzliche Belastungen wie Operation und Narkose in eine akute Niereninsuffizienz übergehen kann. Ziel der Studie war, die Auswirkung einer Neuroleptanalgesie mit Acepromazin auf sonographisch erfassbare Durchblutungsparameter zu untersuchen. Material und Methoden: Bei 19 klinisch gesunden Hunden der Rassen Foxhound und Beagle erfolgte unter Acepromazin/l-Methadon-Narkose (0,1 mg/kg KM Acepromazin; 0,5 mg/kg KM l-Methadon) eine sonographische Untersuchung der linken Niere. Alle 5 Minuten wurden Resistance-Index und Pulsatilitätsindex bestimmt und digital aufgezeichnet, ebenso der invasiv gemessene Blutdruck. Ergebnisse: Fünf Minuten nach Narkoseeinleitung stieg der mittlere arterielle Blutdruck an, fiel aber bereits nach 10 Minuten wieder auf den Ausgangswert zurück und änderte sich im weiteren Verlauf nur noch unwesentlich. Resistance-Index und Pulsatilitätsindex verhielten sich vergleichbar: Nach einem geringgradigen Abfall nach 5 Minuten stiegen die Werte beider Parameter signifikant an. Dieser Anstieg war bis 15 Minuten nach Narkoseeinleitung sehr stark und verlief dann bis zum Ende der Untersuchung nach 30 Minuten deutlich flacher. Die Werte entsprachen denen einer Gefäßstenose. Schlussfolgerung und klinische Relevanz: Aufgrund der erhaltenen Messwerte wird davon abgeraten, diese Form der Anästhesie beim nierenkranken Hund einzusetzen, wenngleich sich die Ergebnisse dieser experimentellen Studie nicht auf den klinischen Patienten übertragen lassen.
Zusammenfassung: Gegenstand und Ziel: Beschreibung der Diagnostik und Therapie eines Nierenabszesses bei einer Katze. Material und Methoden: Fallbericht einer neun Monate alten Europäischen Kurzhaarkatze, die mit einer Fistel an der linken Flanke vorgestellt wird. Ergebnisse: Etwa drei Monate nach einem Hundebiss in die Bauchregion wird die Katze mit einer faustgroßen Umfangsvermehrung im Abdomen vorgestellt, die in der bildgebenden Diagnostik (Röntgen und Ultraschall) als Nierenabszess diagnostiziert wird. Dabei ist die dopplersonographisch erfasste Gefäßarchitektur für die Organzuordnung von großer Bedeutung. Aufgrund der hochgradigen Zerstörung des Parenchyms muss die linke Niere entfernt werden. Die Katze wird mit gutem Allgemeinbefinden entlassen und weist auch sechs Monate nach der Operation ein ungestörtes Allgemeinbefinden auf. Schlussfolgerung: Bei hochgradig veränderten Parenchymen kann die Organzuordnung durch die Dopplersonographie ermöglicht werden. Die Nephrektomie vermag als Therapie eines Nierenabszesses in der Tiermedizin durchaus sinnvoll erscheinen, auch wenn in der Humanmedizin das Bestreben nach weniger invasiven Methoden im Vordergrund steht. Für die Therapieentscheidung spielt der Grad der Organzerstörung eine erhebliche Rolle.
