Objectives. To study the prevalence of anxious-depressive disorders and sleep impairments in their structure among patients hospitalized with the new coronavirus infections (COVID-19) and to develop differential guidelines for their treatment in COVID-19 patients. Materials and methods. This report presents preliminary results from our own observations. We report here analysis of data from 119 patients (age 47-69 years, male and female) obtained at detailed interviews, including using telemedicine technologies, with evaluation on the following scales: the Hospital Anxiety and Depression Scale (HADS), the Multidimensional Fatigue Inventory (MFI-20) for subjective assessment of asthenia, and the Pittsburgh Sleep Quality Index (PSQI). Results. Results on the HADS indicated that clinically severe anxious-depressive symptomatology was seen in 33 of 119 patients (28%) hospitalized with diagnoses of COVID-19: of these, 11% of cases (n = 13) showed clinical signs of signifi cant anxiety only, while fi ve (4%) showed clinically signifi cant depression and 13% displayed increases on both the anxiety and depression subscales (n = 15). Increases on the MFI-20 scale (>20 points) were seen in 87 patients (73%) and sleep impairments on the PSQI were recorded in 32 patients (27%). Conclusions. The results of this study showed that most patients with COVID-19 had not only depressive symptomatology, but also anxious and hypochondriac disorders, asthenic symptom complex, and sleep impairments with diffi culty going to sleep and poor sleep quality. Differential guidelines were developed for the treatment of these states taking account of the side effects of the drugs prescribed, interactions between drugs, and the features of the patients' somatic condition. Drug selection must be based on the severity of the impairments found.
BackgroundCHRONOS was a large naturalistic study designed to evaluate the effectiveness and safety of agomelatine in the management of patients with major depression in routine clinical practice.MethodsPatients (n=6,276) with a moderate or severe major depressive episode without psychotic symptoms were treated initially as outpatients (80.2%) or in psychiatric facilities (19.8%) in 54 regions of the Russian Federation. Patients received a flexible-dosing regimen of agomelatine 25 mg or 50 mg once daily for 8 weeks, with frequent study visits (weeks 1, 2, 3, 4, 6, and 8).ResultsPatients (mean age 44 years, 72.6% female) showed progressive improvement on the 17-item Hamilton Rating Scale for Depression (HAMD-17) total score from 22±6.9 at baseline to 4.7±4.7 at week 8 (P<0.0001). The proportion of responders (HAMD-17 decrease of ≥50%) was 90.1% and the proportion of remitters (HAMD-17 <7) was 79.1% at week 8. All individual HAMD-17 item scores improved rapidly, and the change relative to baseline was significant (P<0.0001) at week 1 and at each subsequent visit in all cases. There were corresponding rapid improvements in Clinical Global Impression Severity and Improvement scores. In the subgroup of patients with more severe illness (HAMD-17 ≥21 at baseline; n=3,478), the proportions of responders and remitters were 92.4% and 72.8%, respectively, at week 8.ConclusionAgomelatine was effective and well tolerated in a large sample of depressed patients in an observational treatment setting, and showed a rapid onset of benefit across all HAMD-17 items.
Background. The COVID-19 pandemic has a significant impact on the mental state of not only quarantined citizens and patients, but also health workers.
Aims asessment of the mental health of doctors involved in work in the red zone during the COVID-19 pandemic.
Materials and methods. 77 respondents were interviewed using the HADS questionnaires and the Maslach burnout test. For statistical data processing Microsoft Office Excel 2016, IBM SPSS were used.
Results. An increase on the depression scales was noted in 7%, anxiety in 23%, and anxiety and depression together 27%. According to the Maslach questionnaire, 32 doctors (41.5%) noted a reaction of the type of emotional devastation. 10 doctors (12.9%) noted a reaction reduction of professional achievements. Three doctors (3.8%) had a dehumanization reaction in the form of dull emotions to colleagues and patients.
Conclusions. Work in the red zone has a significant negative impact on the mental health of doctors and medical personnel.
