Linear growth retardation and anemia are the most prevalent nutritional problems in the world; effective interventions are urgently needed. We evaluated Ecuador's National Food Nutrition Program (PANN 2000) that included a micronutrient-fortified complementary food (FCF), Mi Papilla, in poor periurban and rural communities of Ecuador. The program is preventive and targeted to all infants and young children living in poor communities and receiving government health services. We compared dietary intake, micronutrient status, and growth over 11 mo in a cohort of children from the catchment areas of the PANN 2000 with same-age control children in nearby communities eligible to enter the program 1 y later. PANN 2000 children enrolled in the program when they were age 9-14 mo and were age 20-25 mo at the final survey. They consumed significantly more energy, protein, fat, iron, zinc, vitamin A, and calcium than control children because of their FCF consumption. Anemia, 76% in both groups at baseline, fell to 27% in PANN 2000 children but only to 44% in control children (P < 0.001). The odds of being anemic were 58% lower for PANN 2000 children (P = 0.003). The effects on linear growth and weight were limited to children who were older when the program began (12-14 mo) and were significant for weight (interaction with age, 0.38 kg; P = 0.029) and positive but not significant for length (0.66 cm; P = 0.08). An FCF, including ferrous sulfate, delivered through public health services, is highly effective in improving weight and hemoglobin and reducing anemia.
O Objetivo: evaluar si en la literatura científica se describe una relación entre obesidad y dislipidemia con progresión a deterioro cognitivo leve (DCL) y demencia en adultos mayores de 65 años. Métodos: búsqueda en la literatura científica publicada en PubMed, EbscoHost, ScienceDirect ClinicalKey y Registro Cochrane entre 2009 y 2017, de estudios prospectivos de cohortes, casos y controles de sujetos adultos con diagnóstico de obesidad o dislipidemia y con valoraciones de la función cognitiva a través del tiempo, con el fin de determinar la incidencia de DCL y demencia, y su relación con los factores de riesgo mencionados. Resultados: la revisión sistemática arrojó un total de 13 artículos de alta calidad según SIGN que cumplieron con los criterios de selección. De estos 7 incluyen la obesidad como factor de riesgo y 6 la dislipidemia. En los diferentes artículos relacionados con obesidad en adultos mayores no existió relación directa entre obesidad y deterioro cognitivo, de igual forma en los relacionados con dislipidemia no se reportó relación directa con deterioro cognitivo. Conclusión: los pacientes adultos mayores con obesidad podrían tener un factor protector frente al desarrollo de deterioro cognoscitivo, lo que no ocurre con adultos jóvenes. Por su parte, la fisiopatología descrita en la dislipidemia sugiere que el colesterol alto o bajo pueden llevar a DCL y a demencia.
INTRODUCCIÓN: La prevalencia de deterioro cognitivo leve (DCL) y demencia se ha descrito recientemente como alta en Colombia, debido al aumento de la población adulta mayor y a su baja escolaridad. Aun no es claro el mecanismo por el cual la diabetes mellitus (DM) o la hipertensión arterial (HTA), en ausencia de accidente cerebrovascular (ACV), aumentarían el riesgo de DCL o demencia. OBJETIVO: Revisar en la literatura la posible asociación de HTA y DM con el riesgo de progresión a DCL. MÉTODOS: Se realizó una búsqueda en las bases de datos de las publicaciones entre los años 2007 y 2017, de cohortes y casos y controles de adultos de 65 años o más con HTA o DM y que evaluaran como desenlace DCL o demencia. RESULTADOS: La revisión arrojó doce artículos que cumplieron con los criterios de selección. Seis artículos reportaban relación entre HTA y riesgo de presentar deterioro cognitivo; de estos, cinco artículos presentaban como desenlace demencia vascular (DVa) y el otro tanto demencia tipo Alzheimer (DTA) como DVa. Además, informan sobre el riesgo de presentar demencia según las cifras de presión arterial sistólica y diastólica. De los seis estudios de cohortes incluidos, cinco encontraron una relación significativa entre DM y progresión a DCL o DTA. CONCLUSIÓN: Los pacientes con HTA tienen mayor riesgo de DVa, seguido por demencia mixta. La DM se asocia con una mayor frecuencia de DTA, en especial si la DM aparece en edades tempranas.
