Objective: To discuss the potentialities of using the concept of vulnerability to support measures for preventing and controlling healthcare-associated infections (HAIs). Methods: This theoretical study was conducted in steps: 1) presentation of markers that frame the concept of vulnerability; 2) presentation of the characteristics of the health events to which the concept of vulnerability is intended to be applied; 3) identification of research gaps that could be potentially filled by using the concept of vulnerability; 4) identification of the potentialities of using the concept of vulnerability to deal with HAIs. Results: Proposal of a framework for analyzing HAIs from a vulnerability perspective, including the individual and collective dimensions. Conclusion: Using the concept of vulnerability to study and deal with HAIs favors a new approach to an old problem, unlike the dominant studies that highlight the individual aspects of the practices in healthcare services.
O objetivo deste estudo foi desenvolver e validar o conteúdo do roteiro educativo para indivíduos em precauções específicas para evitar a transmissão de microrganismos. Estudo metodológico, desenvolvido em três fases,em dois hospitais (público e privado) na cidade de São Paulo, no período de maio a julho de 2015, utilizando o referencial teórico de Vulnerabilidade. Na primeira fase, aplicou-se questionário para captação das percepções dos indivíduos adultos sobre precauções específicas. Na segunda fase, elaborou-se o roteiro educativo. Na terceira fase, foi submetido a especialistas em precauções específicas e vulnerabilidade para validação de conteúdo. Foram entrevistados39 indivíduos em média sete dias após a instituição das precauções específicas, 32 (82%) em precaução para contato. O roteiro foi desenvolvido para proporcionar maior conhecimento nos aspectos usualmente negligenciados pelos profissionais e estimular o cuidado centrado no indivíduo. Todos os itens tiveram um índice de validade de conteúdo acima de 75%. O roteiro apresenta potencial para instrumentalizar profissionais na elaboração de ações educativas para pacientes adultos em precauções específicas.
Background: In healthcare implementation research, there is little discussion on researcher experiences using frameworks including the Consolidated Framework for Implementation
Research (CFIR).Aim: To identify and discuss the benefits, challenges and lessons learnt from researchers' experiences using the CFIR in different contexts and phases of research.Discussion: The article synthesised the reflections of nursing and public health researchers' on their experiences using the CFIR across four separate healthcare-associated infection prevention and control implementation studies. Benefits, challenges, resolutions to these challenges and lessons learnt from the application of the framework were discussed.Conclusions: Identified benefits included the framework's adaptability and flexibility, provision of structure and shared language for research. Translation to another language, and differentiating between domains and constructs were challenges.Implications for practice: Healthcare researchers may find this article useful when considering use of the CFIR, or to anticipate and prepare to overcome the challenges highlighted when using the framework.
