A prospective analysis of cytomegalovirus (CMV) glycoprotein B (gB) genotypes was conducted on 34 renal transplant recipients using peripheral blood leukocytes (PBLs) and urine specimens. The CMV gB genotypes were analyzed by polymerase chain reaction (PCR) followed by enzyme digestion. PBLs and urine samples showed almost equal proportions of the 4 known gB genotypes, as well as equal proportions of gB genotype mixtures. The gB genotypes 1, 2 and 3 were equally distributed in the patients. Twenty-four (70.6%) patients had more than one gB genotype during follow-up. There was no association of gB genotypes with the development of symptomatic CMV infection.
Since the emergence of West Nile virus (WNV) in North America in 1999, there have been several reports of WNV activity in Central and South American countries. To detect WNV in Brazil, we performed a serological survey of horses from different regions of Brazil using recombinant peptides from domain III of WNV. Positive samples were validated with the neutralisation test. Our results showed that of 79 ELISA-positive horses, nine expressed WNV-specific neutralising antibodies. Eight of the infected horses were from the state of Mato Grosso do Sul and one was from the state of Paraíba. Our results provide additional evidence for the emergence of WNV in Brazil and for its circulation in multiple regions of the country.
Purpose: To determine the incidence of Piry virus contamination among surgical instruments used with disposable accessories for phacoemulsification during se quential surgeries. Methods: An experimental model was created with 4 pigs' eyes that were conta minated with Piry virus and 4 pigs' eyes that were not contaminated. Phacoemul sification was performed on the eyes, alternating between the contaminated and noncontaminated eyes. From one surgery to another, the operating fields, gloves, scalpel, tweezers, needles, syringes, tips and bag collector from the phacoemulsi fi cation machine were exchanged; only the hand piece and the irrigation and aspira tion systems were maintained. Results: In the collector bag, three samples from the contaminated eyes (3/4) were positive, and two samples from the noncontaminated (2/4) eyes were also positive; at the tip, one sample from the contaminated eyes (1/4) and two samples of the non contaminated eyes (2/4) yielded positive results. In the irrigation system, one sample from a noncontaminated eye (1/4) was positive, and in the aspiration system, two samples from contaminated eyes (2/4) and two samples from noncontaminated eyes (2/4) were positive. In the gloves, the samples were positive in two samples from the noncontaminated eyes (2/4) and in two samples from the contaminated eyes (2/4). In the scalpel samples, three contaminated eyes (3/4) and none of the noncontaminated eyes (0/4) were positive; finally, two samples from the anterior chambers of the noncontaminated eyes gathered after surgery were positive. Conclusions: In two noncontaminated eyes, the presence of genetic material was detected after phacoemulsification surgery, demonstrating that the transmission of the genetic material of the Piry virus occurred at some point during the surgery on these noncontaminated eyes when the hand piece and irrigation and aspiration systems were reused between surgeries.
RESUMO:Neste trabalho, analisaram-se bactérias isoladas em 180 episódios de endocardite infecciosa, ocorridos em Ribeirão Preto, entre 1992 e 1997, e suas susceptibilidades a antimicrobianos. Hemoculturas foram realizadas em 93,8% dos casos, sendo que, em 60,7% deles, foi identificado, pelo menos, um agente etiológico. As bactérias mais comumente isoladas nos pacientes com EI foram Staphylococcus aureus, 31,0 % (52/168), Streptococcus viridans, 15,9 % (27/168), Streptococcus do Grupo D, 6,6% (11/168), Gram negativas, 5,3 % (9/168) e Staphylococcus epidermidis, 4,2 % (7/168). Dos Staphylococcus aureus, todos os testados mostraram-se sensíveis à vancomicina e à teicoplanina, 86,5%, mostraram-se sensíveis a aminoglicosídios (gentamicina e amicacina), 85% à cefalosporina de primeira geração (cefalotina) e 79% à oxacilina. Streptococcus viridans isolados, todos foram sensíveis ao cloranfenicol, à vancomicina e apenas dois foram resistentes à ampicilina e à penicilina G. Quanto aos Streptococcus do Grupo D, todos mostraram-se sensíveis à penicilina G, à ampicilina, ao cloranfenicol e à vancomicina. UNITERMOS:Endocardite. Infecção. Endocardite Bacteriana. Infecções Bacterianas. 1-INTRODUÇÃOA endocardite infecciosa (EI) é uma doença grave que acomete estruturas endocárdicas, principalmente as válvulas cardíacas e o endocárdio mural. Manifestações sistêmicas como febre, prostração, presença de sopro cardíaco, emagrecimento, hepatomegalia, esplenomegalia e alterações laboratoriais como anemia, leucocitose, proteinúria e hematúria, são freqüen-temente observadas. Essas alterações estão comumente relacionadas à virulência do agente etiológico, ao local de acometimento e à resposta imune do hospedeiro, bem como à efetividade do tratamento realizado (1) . A confirmação diagnóstica de EI, conforme recomendada nos critérios mais aceitos atualmente, como os de Duke, é feita com o isolamento do microorganismo causador, em hemoculturas, e pela visualização de vegetações relacionadas à presença bacteriana no endocárdio, em exames ecocardiográficos. Consideram-se, também, outros aspectos clínicos e patológicos nesses critérios (2) . and greater effectiveness (by 13.3%), with LZD being the 'dominating' treatment. Majority of treatment costs were related to hospital stay, primarily ICU (73% in private and 50% in public scenario). Several scenarios were tested by varying treatment duration (7or14 days), and varying discontinuation/ switch of therapy (at 5 or 10 days). Results for all scenarios were similar to the base case from public and private perspectives. CONCLUSIONS: From private perspective LZD is a cost-effective alternative to VAN and TEI for treatment of MRSA-confirmed NP, owing primarily to its higher clinical response rate. From public perspective, LZD can be considered cost effective since its ICER vs. VAN is within 2-3 times Brazil's GDP per capita. RESUMO:Citomegalovírus (CMV) é considerado o agente mais comum de infecção congêni-ta e perinatal em todas as regiões do mundo. Esta revisão visa apresentar os principais aspectos clínicos, epidemiológicos, diagnósticos e de tratamento desta infecção em recém-nascidos e lactentes. UNITERMOS:Infecções por Citomegalovírus. Infecção. Congênita. Perinatologia. INTRODUÇÃOCitomegalovírus são vírus DNA, da família Herpesviridae, subfamília Betaherpesvirinae e gê-nero citomegalovirus. Estudos soroepidemiológicos mostram que as infecções por estes vírus são muito freqüentes, ocorrendo em todas as regiões do mundo, sendo tanto mais comuns quanto menor o nível só-cio-econômico da população estudada (1) . Porém, observa-se que a doença clínica é rara em crianças e adultos imunocompetentes (1) . O único reservatório para a transmissão do CMV humano é o próprio homem. As fontes humanas de disseminação dos vírus incluem secreções respiratórias, saliva, sangue, urina, secreção de colo uterino, esperma, colostro e leite materno. A infecção primária pode ocorrer no perío-do pré-natal, perinatal ou pós-natal (2) .A infecção primária, em indivíduos imunocompetentes, geralmente, apresenta uma evolução benigna, mas, a exemplo de outros herpesvírus, o CMV pode permanecer latente no hospedeiro e ser reativado em decorrência de depressão da imunidade celular, que resulta em alterações no equilíbrio entre vírus e hospedeiro (3) . As células mononucleares do sangue periférico parecem ser os maiores sítios de latência do CMV (4) . Infecções recorrentes por CMV podem ser causadas por reativação do vírus causador da infecção primária ou por reinfecção. A reinfecção tem sido observada em casos de exposição a cepas diferentes de CMV (3) .
