INTRODUÇÃOOs curativos são uma forma de tratamento das feridas cutâneas e sua escolha depende de fatores intrínsecos e extrínsecos. O tratamento das feridas cutâneas é dinâmico e depende, a cada momento, da evolução das fases de cicatrização. Atualmente são inúmeras as opções de curativos existentes no mercado. Os recursos financeiros do paciente e/ou da unidade de saúde, a necessidade de continuidade da utilização do curativo, inclusive com visitas domiciliares, e a avaliação de benefícios e custos são alguns dos aspectos a serem considerados no momento da escolha do tipo de curativo, que devem ser adequados à natureza, à localização e ao tamanho da ferida. Embora haja uma grande variedade de curativos, um só tipo de curativo não preenche os requisitos para ser aplicado em todos os tipos de feridas cutâneas.Para incisões cirúrgicas, a oclusão deverá ser por 24 a 48 horas mantendo o curativo seco.Nas feridas abertas, a antiga controvérsia entre curativo seco e curativo úmido deu lugar a uma proposta atual de oclusão e manutenção do meio úmido.A cicatrização através do meio úmido tem as seguintes vantagens quando comparadas ao meio seco [1][2][3][4][5][6][7] : prevenir a desidratação do tecido que leva à morte celular; acelerar a angiogênese; estimular a epitelização e a formação do tecido de granulação; facilitar a remoção de tecido necrótico e fibrina; servir como barreira protetora contra microorganismo; promover a diminuição da dor; evitar a perda excessiva de líqui-dos; e evitar traumas na troca do curativo.CURATIVOS COM POMADAS OU ÓLEOS -têm como exemplos comumente utilizados os seguintes curativos 1,2,5,8,9 . Curativo com Sulfadiazina de PrataComposição: sulfadiazina de prata a 1% hidrofílica. Mecanismo de ação: o íon prata causa a precipitação de proteínas e age diretamente na membrana citoplasmática da célula bacteriana, exercendo ação bactericida imediata, e ação bacteriostática residual, pela liberação de pequenas quantidades de prata iônica.Indicação: feridas causadas por queimaduras ou que necessitem ação antibacteriana.Contra indicação: hipersensibilidade a sulfas. Modo de usar: remover o excesso de pomada e tecido desvitalizado. Lavar a ferida e aplicar o creme, assepticamente, em toda extensão da lesão (5 mm de espessura). Colocar gaze de contato úmida. Cobrir com curativo estéril.Periodicidade de troca: no máximo a cada 12 horas ou quando a cobertura secundária estiver saturada. No momento da troca a pomada pode apresentar aspecto purulento devido a sua oxidação sem, contudo apresentar infecção real.Exemplo comercial: Dermazine®; Pratazine® Curativo com Pomada Enzimática -ColagenaseComposição: colagenase clostridiopeptidase A e enzimas proteolíticas.Mecanismo de ação: age degradando o colágeno nativo da ferida.Indicação: feridas com tecido desvitalizado. Contra indicação: feridas com cicatrização por primeira intenção.Modo de usar: aplicar a pomada sobre a área a ser tratada. Colocar gaze de contato úmida. We aimed to evaluate the treatment protocols for cleft lip and palate that are used in Brazil, to compare them with the ones proposed from elsewhere, and to discuss the official data about admission for treatment of cleft lip or palate, or both, in Brazil. We also assessed the importance of integrated action of different specialities to treat this condition. A questionnaire related to attendance protocols was developed and sent out to all Brazilian Plastic Surgery Services connected to the Brazilian Society of Plastic Surgery, and to other units involved in such treatment. We also studied the data produced by the Brazilian Department of Health about the operations done during the past five years. Many protocols were identified, but despite much controversy in many areas, a consensus was reached about the surgical techniques, the age group most suitable to be operated on, and there was total agreement about the need for multidisciplinary management. According to the State Department of Health, the number of operations done in 1995 was 9696, and this had increased to 21 022 in 1999. The state of São Paulo had done 17636 (84%) of all procedures in 1999.
This paper addresses the problem of automatically learning to classify texts by exploiting information derived from meta-level features (i.e., features derived from the original bag-of-words representation). We propose new meta-level features derived from the class distribution, the entropy and the within-class cohesion observed in the k nearest neighbors of a given test document x, as well as from the distribution of distances of x to these neighbors. The set of proposed features is capable of transforming the original feature space into a new one, potentially smaller and more informed. Experiments performed with several standard datasets demonstrate that the effectiveness of the proposed meta-level features is not only much superior than the traditional bagof-word representation but also superior to other state-ofart meta-level features previously proposed in the literature. Moreover, the proposed meta-features can be computed about three times faster than the existing meta-level ones, making our proposal much more scalable. We also demonstrate that the combination of our meta features and the original set of features produce significant improvements when compared to each feature set used in isolation.
