Em julho de 1990, foi registrado na cidade de Bauru, Estado de São Paulo, Brasil, um surto de malária envolvendo usuários de cocaína injetável. Uma ampla investigação epidemiológica, conduzida de 19 de julho a 13 de setembro, revelou que pelo menos 119 pessoas estavam envolvidas no surto, uma vez que haviam compartilhado seringas e agulhas com um ou mais casos confirmados nos 3 meses anteriores à ocorrência. Cento e duas dessas pessoas foram localizadas e entrevistadas, e destas, 99 foram submetidas a exame de gota espessa e 91 a exames sorológicos para malária. Foram confirmados por exame hemoscópico 21 casos de malária por P. vivax, e 3 outros tiveram exame sorológico positivo para P. vivax. O controle da transmissão foi obtido fornecendo-se cloroquina aos envolvidos no surto, numa dose inicial de 10 comprimidos, seguida de doses supressivas semanais de 2 comprimidos até que fosse identificado o último comunicante. Amostras de soro coletadas na ocasião revelaram, ao lado da malária, uma alta prevalência de infecções pelo HIV (58%) e pelo vírus da hepatite B (40%). Foram discutidas as dificuldades para o controle do surto e a possibilidade da malária vir a se tornar uma doença endêmica entre usuários de drogas injetáveis, no Estado de São Paulo.
Cento e dez pacientes com malária falciparum não complicada, provenientes da Amazônia brasileira, foram tratados com cloridrato de tetraciclina, na dose de 250 mg a cada 6 horas por 7 dias, associado ao sulfato de quinino na dose de 1,5 a 2,0 gramas/dia nos 3 ou 4 dias iniciais de tratamento. Os pacientes tiveram acompanhamento clínico e parasitológico por pelo menos 7 dias e em todos observou-se a negativação da parasitemia após o tratamento. Setenta e cinco pacientes foram acompanhados por 28 dias ou mais e destes 71 tiveram resposta do tipo Se4do tipo RI. O cloridrato de tetraciclina associado ao sulfato de quinino mostrou-se altamente eficaz no tratamento de malária falciparum não complicada, sendo bem tolerado e portanto útil na terapêutica das infecções por P. falciparum multirresistentes contraídas na Amazônia.
Relata-se surto de malária por Plasmodium vivax entre usuários de drogas injetáveis, detectado na cidade de Bauru, Estado de São Paulo, Brasil. Até julho de 1989 12 casos haviam sido confirmados pelo exame de gota espessa, e 20 outros contactantes suspeitos estavam sendo investigados. Todos os casos relataram uso freqüente de cocaína injetável, compartilhando seringas e agulhas, e negaram viagens por áreas endêmicas de malária.

O sistema de vigilância da doença de Chagas no Estado de São Paulo propõe investigação minuciosa da presença de triatomíneos nos domicílios, que inclui o controle sorológico de populações moradoras em unidades domiciliares associadas a focos potenciais de triatomíneos vetores. Nos últimos anos tem-se observado que os indivíduos sorologicamente reagentes distribuem-se em faixas etárias acima de 19 anos, sendo que as investigações de casos mostraram que estes adquiriram a infecção no Estado de São Paulo, no passado ou em outros Estados onde a endemia ainda ocorre. Recentemente, um caso de uma criança de oito anos de idade, residente na Região de Sorocaba (SP), mostrou-se sorologicamente reagente (título igual a 128 -IgG -por meio da Reação de Imunofluorescência Indireta). Pela investigação epidemiológica revelou tratar-se de caso transfusional, cujo doador, sorologicamente reagente, forneceu elementos suficientes para explicar a origem da infecção. Observou-se que este doador já havia doado sangue em mais de uma oportunidade, sem que se tivesse descoberto tratar-se de portador de infecção chagásica. Concluiu-se pela necessidade de implantação de sistema de atendimento de pacientes sorologicamente reagentes, tendo em vista horizontalizar atividades de saúde pública.Descritores: Tripanossomose sul-americana, transmissão. IntroduçãoO controle da transmissão vetorial da doença de Chagas no Estado de São Paulo teve iní-cio em 1950 e após duas décadas de ações contínuas, os indicadores entomológicos e de prevalência da endemia na população humana apontavam êxito nesse sentido. Com o controle dos vetores domiciliados -Triatoma infestansespécies ditas "secundárias" na transmissão como T. sórdida e Panstrongylus megistus passaram a assumir importância crescente à medida que se constatava o crescimento destas espécies nas coletas efetuadas no ambiente domiciliar.Assim, foi proposto para a zona rural do Estado de São Paulo um sistema de vigilância da doença de Chagas, que visasse não só a garantir os resultados alcançados pelas medidas de controle, como também aumentar a eficiência das ações de intervenção.Desta forma, qualquer presença de triatomí-neos nos domicílios se constitui num foco e como tal deve ser investigada. Neste particular, tem sido realizada pesquisa de anticorpos antiTrypanosoma cruzi no sangue dos moradores das casas com presença demonstrada de: a) Triatoma infestans; b) triatomíneos hematófagos em que se tivesse detectado a presença de sangue humano; c) triatomíneos hematófagos positivos para T. cruzi. Todos os pacientes sorologicamente positivos devem ser investigados epidemiologicamente devendo ser determinado o possível local de transmissão.
