The risk of mycobacterial disease is significantly increased in drug abusers as well as in immunocompromised HIV-1-infected individuals. The essential trace element selenium has an important function in maintaining immune processes and may, thus, have a critical role in clearance of mycobacteria. The impact of selenium status on the development of mycobacterial diseases in HIV-1-seropositive drug users was investigated over a 2-year period (1999-2001). Twelve cases of mycobacterial disease (tuberculosis, 9; infection due to atypical Mycobacterium species, 3) occurred; these 12 cases were compared with 32 controls with no history of respiratory infections who were matched on age, sex, and HIV status. IntroductionWe assessed HIV drug resistance (DR) in individuals failing ART (acquired DR, ADR) and in ART-naïve individuals (pre-ART DR, PDR) in Honduras, after 10 years of widespread availability of ART.Methods365 HIV-infected, ART-naïve, and 381 ART-experienced Honduran individuals were enrolled in 5 reference centres in Tegucigalpa, San Pedro Sula, La Ceiba, and Choluteca between April 2013 and April 2015. Plasma HIV protease-RT sequences were obtained. HIVDR was assessed using the WHO HIVDR mutation list and the Stanford algorithm. Recently infected (RI) individuals were identified using a multi-assay algorithm.ResultsPDR to any ARV drug was 11.5% (95% CI 8.4–15.2%). NNRTI PDR prevalence (8.2%) was higher than NRTI (2.2%) and PI (1.9%, p<0.0001). No significant trends in time were observed when comparing 2013 and 2014, when using a moving average approach along the study period or when comparing individuals with >500 vs. <350 CD4+ T cells/μL. PDR in recently infected individuals was 13.6%, showing no significant difference with PDR in individuals with longstanding infection (10.7%). The most prevalent PDR mutations were M46IL (1.4%), T215 revertants (0.5%), and K103NS (5.5%). The overall ADR prevalence in individuals with <48 months on ART was 87.8% and for the ≥48 months on ART group 81.3%. ADR to three drug families increased in individuals with longer time on ART (p = 0.0343). M184V and K103N were the most frequent ADR mutations. PDR mutation frequency correlated with ADR mutation frequency for PI and NNRTI (p<0.01), but not for NRTI. Clusters of viruses were observed suggesting transmission of HIVDR both from ART-experienced to ART-naïve individuals and between ART-naïve individuals.ConclusionsThe global PDR prevalence in Honduras remains at the intermediate level, after 10 years of widespread availability of ART. Evidence of ADR influencing the presence of PDR was observed by phylogenetic analyses and ADR/PDR mutation frequency correlations.
Antecedentes. El Trastorno de déficit de atención con hiperactividad (TDAH) define un patrón persistente de inatención y/o hiperactividad/impulsividad, incoherente con el nivel de desarrollo del individuo. Inicia en la infancia e interfiere en la adaptación, funcionamiento social y rendimiento académico. Objetivo. Determinar el rendimiento académico en los escolares asociado a trastorno de déficit de atención con hiperactividad en escuelas públicas de El Progreso, Yoro durante el período de enero a junio 2018. Pacientes y métodos. Estudio descriptivo transversal analítico. Universo de 12,440 escolares de escuelas públicas. Se estimó un tamaño muestral de 300 (10.8%, IC95%). Se incluyeron escolares de 6-12 años. Se excluyeron patologías congénitas o adquiridas con afectación directa en personalidad/conducta, fracaso escolar secundario a estresores ambientales. Las variables incluyeron datos del escolar/ padre/tutor, rendimiento académico, antecedentes familiares/personales patológicos. Se utilizó el test de Conners para padres/maestros, así como el cuestionario DSM-V para diagnóstico TDAH. La información recolectada fue ingresada en Epiinfo versión 7.2.2.6 (CDC, Atlanta). Los resultados se presentan como frecuencias, porcentajes, valor de P, OR y chi2 La información personal de los casos se manejó confidencialmente. Resultados. La prevalencia de TDAH fue del 36% (108/300). Se encontró asociación estadísticamente significativa (P<0.05) en la edad del escolar, estado civil del padre/ tutor, antecedentes familiares, persona que vive con él. El rendimiento escolar fue satisfactorio ≤70-80% (53%). No hubo diagnóstico de TDAH en 64%. Conclusión. Es importante realizar una intervención oportuna e integral para prevenir las complicaciones del TDAH y la afectación directa en el rendimiento académico de los escolares.
Antecedentes: Se considera una Infección Asociada a la atención en Salud (IAAS) aquella que no está presente ni incubándose en el momento de la admisión, pero que se observa 72 horas después de la estadía hospitalaria al alta del paciente. Objetivo: Determinar la relación entre las características del huésped, procedimientos invasivos y las infecciones asociadas a la atención en salud en la sala de lactantes del Hospital Nacional Dr. Mario Catarino Rivas (HNMCR), durante el período de enero a diciembre 2017. Pacientes y Métodos: Estudio descriptivo, retrospectivo de 300 pacientes lactantes de 1 mes a 1 año 364 días que fueron ingresados en la sala de lactantes del HNMCR. El tamaño de la muestra fue de 45 pacientes. Resultados: La prevalencia IAAS fue de 15%. El 62.2% de los pacientes presentaron alguna enfermedad de base asociada, representando la desnutrición un 64.4%. Conclusiones: Los procedimientos invasivos y las enfermedades de base asociadas son un factor de riesgo para el desarrollo de una IAAS.
