ResumenIntroducción: el cáncer de páncreas se caracteriza por su agresividad biológica, mal pronóstico oncológico y diagnóstico tardío. La mayoría con baja tasa de resecabilidad (10% a 20%), por la presencia de factores de irresecabilidad al momento del diagnóstico.Objetivo: descripción epidemiológica de las características del cáncer de páncreas irresecable, los tratamientos paliativos realizados, la sobrevida global y en relación con los procedimientos realizados.Material y método: estudio descriptivo, multivariable, retrospectivo de los cánceres de páncreas irresecables, de enero de 2010 a diciembre de 2015, en la Clínica Quirúrgica 3 del Hospital Maciel.Resultados: de los 30 pacientes irresecables: igual distribución por sexo; edad promedio: 61 años, cefálicos en 67% de los casos. El 53% adelgazados al momento de la primera consulta; 52% presentaban compromiso ganglionar locorregional, y 50% presentaba metástasis hepáticas o ascitis. A 52% de los pacientes se les realizó algún tipo de derivación biliar; quirúrgi-ca en 53% (63% coledocoduodenostomías) y el resto percutánea o endoscópica. El 31% de los pacientes, por lo avanzado de la enfermedad, no recibió tratamiento quirúrgico ni endoscópico. La sobrevida media del tratamiento paliativo quirúrgico o endoscópico fue de 3,7 meses.Conclusiones: al momento del diagnóstico, la mitad de los casos tenían metástasis y repercusión nutricional severa, con indicadores locorregionales de irresecabilidad o incurabilidad. Un tercio de los casos no recibió tratamiento oncológico por lo avanzado de la enfermedad. La sobrevida global fue escasa. Esto permite sugerir que son imprescindibles medidas sanitarias para su diagnóstico precoz e incremento de la tasa de resecabilidad.
ResumenLa hidatidosis o equinococosis quística es una enfermedad zoonótica que tiene presentaciones clínicas muy heterogé-neas. Los quistes sintomáticos y complicados habitualmente son resueltos mediante cirugía siguiendo las reglas de la WHO-IWGE. Sin embargo, en las formas transicionales (CE3) los criterios terapéuticos continúan en discusión. Asimismo, en quistes asintomáticos y en pacientes con alto riesgo quirúrgico, la opción del tratamiento médico con benzoimidazólicos y su combinación con la estrategia de vigilar y esperar puede ser una alternativa terapéutica segura. Se presenta un caso de hidatidosis hepática CE3b con características clínico-evolutivas excepcionales y alto riesgo quirúr-gico en que se adoptó un manejo de vigilancia clínica y tratamiento con albendazol. Se analizan los beneficios y riesgos de esta terapéutica. IntroducciónLa equinococosis quística o hidatidosis es una enfermedad zoonótica causada por el parasitismo de la larva de Echinococcus granulosus, que continúa siendo un grave problema sanitario en el mundo, en la región y en Uruguay (1) . El hígado es la localización más frecuente; según nuestra casuística representa el 59% de los casos, con una gran variabilidad en su presentación clínica y sintomatología (2) . El diagnóstico se basa en la presentación clínica, factores de riesgo epidemiológicos, imagenología y serología, teniendo esta última un rol complementario (3) . Actualmente los criterios generales de tratamiento se ajustan a los criterios establecidos a partir de la clasificación WHO-CE (4) y la clásica de Gharbi (5) , los que dependen de la localización, tamaño, presencia de complicaciones, quistes y riesgos de diseminación. Sin embargo, existe una población de casos asintomáticos y no complicados en los cuales se discuten los resultados de la cirugía (abierta o laparoscópica) frente a otras alternativas de tratamiento médico, percutánea o una estrategia de vigilancia y espera, o ambas (6,7) . En algunos casos la combinación de terapéutica mé-dica con albendazol (ABZ) en series (3 a 6) de 30 días con descanso de 15 días entre ellos es recomendada, continuando con un seguimiento regular por imagenología anual (6)(7)(8) . El propósito de este trabajo es presentar un caso de equinococosis quística hepática CE3b, con alto riesgo quirúrgico, tratado mediante vigilancia/esperar y alben-269
La adenomiomatosis vesicular (AV)se observa entre 1%y8,5 % de las colecistectomías(1,2). Se asocia a colelitiasis en 80 % de los casos. (3) Es una neoplasia benigna, caracterizada por hiperplasia del epitelio vesicular con invaginaciones hasta la subserosao serosa formando divertículos intramurales, los senos de Rokitansky-Aschoff.(4) (Figura 1) Se observa la hipertrofia muscular asociada a invaginación de la mucosa de diversa profundidad. La ecografía es relativamente inespecífica; pero la TC contrastada incrementa el diagnóstico .La colangioRNM es studio más eficaz con una sensibilidad de 81% a 93 % y especificidad de 93% a 100 % ; porque las cavidades intraluminares hiperintensas en T.2 son sugestivas de ser senos de Rokitansky-Asffof y excluyen el diagnóstico de cáncer de vesícula. (2,5,6).
