BACKGROUND: Adherence to TB treatment and therefore treatment success could be improved using digital adherence technology.OBJECTIVE: To evaluate the effectiveness of a medication event reminder monitor system (MERM) on treatment success and treatment adherence in patients with drug-susceptible pulmonary TB in Perú.METHODS: This was an experimental, randomised, open-label, controlled study conducted among patients in the second phase of TB treatment. The intervention group received their medications through MERM with the support of a treatment monitor, whereas the control group used the usual strategy. Participants were followed until they completed the 54 doses of the second phase of treatment.RESULTS: The study included 53 patients in each group; four in the intervention group withdrew from the study. Treatment success was significantly more frequent in the MERM group (RR 1.15, 95% CI 1.02–1.30; P = 0.0322). There was no significant difference in the adherence outcomes; however, the percentage of patients who missed at least one dose and patients with more than 10% of total doses missed were lower in the intervention group.CONCLUSION: The use of MERM in the second phase of treatment showed a significant improvement in the treatment success rate in patients with drug-susceptible pulmonary TB.
Objetivos. Determinar el efecto gastroprotector y antisecretor del extracto etanólico de las hojas de matico (Piper aduncum) en modelos animales. Materiales y métodos. Para la evaluación del efecto gastroprotector se utilizó 220 ratones de la cepa Balb C57, los cuales fueron aleatorizados en 22 grupos de diez animales, a los cuales se les indujo la formación de úlceras gástricas con indometacina, la gastroprotección se determinó a través de tres aspectos: inflamación, número de bandas hemorrágicas y número de úlceras. Para evaluar el efecto antisecretor se utilizó 64 ratas albinas machos Holtzman, los cuales fueron aleatorizados en ocho grupos de ocho animales, un control y siete grupos de tratamiento con un nivel de dosis de los extractos y dos niveles de dosis en los fitofármacos; la antisecreción se realizó con el ensayo de ligazón pilórica. Resultados. Para la gastroprotección, los extractos de diclorometano, cloroformo, hexano y metanol, lograron una disminución de la inflamación de más del 66% (p<0,05); el extracto etanólico presenta una actividad de 100% para disminuir el número de bandas hemorrágicas (p<0,05); el extracto clorofórmico presenta una actividad antiulcerosa de 75% (p<0,05). Respecto a la antisecreción, el fitofármaco en cápsulas conteniendo el extracto etanólico logró un 72% de reducción del volumen de la secreción gástrica (p<0,01) y un incremento del pH en 104,3% (p<0,01). Conclusiones. En condiciones experimentales los extractos etanólico, sus fracciones y su fitofármaco son gastroprotectores en ratones y antisecretores en ratas. To determine the gastroprotective and antisecretory effect of ethanol extract from matico leaves (Piper aduncum) in animal models. Materials and methods. To evaluate the gastroprotective effect, 220 mice of the Balb C57 strain were used. They were randomized in 22 groups of ten animals each, in which the formation of gastric ulcers was induced with indomethacin. Gastroprotection was determined by evaluating three aspects: inflammation, number of hemorrhagic shocks and number of ulcers. This study aimed to determine the therapeutic equivalence of four multi-source drugs containing moxifloxacin (400 mg tablets) in vitro studies to establish their interchangeability with the reference product. Four multi-source products were acquired in pharmaceutical establishments in metropolitan Lima, each from different manufacturing sites (two products from India, one from Brazil, and one from Peru). The reference product was Avelox (400 mg) coated tablets (Bayer AG, Germany). Quality control and dissolution profile tests were performed. For dissolution tests, a validated ultraviolet-visible spectrophotometry method was used to determine the percentage of drug released. The similarity factor (f2) analysis was used to establish therapeutic equivalence of the drug release curves. The dissolution rates were considered equivalent if the values of f2 were between 50 and 100. Concerning the quality control tests, the moxifloxacin content was 98.5% in the reference product and 97.1-100.0% in the multisource products. Three out of four multi-source products passed the f2 test at pH 1.2. Therefore, there is at least one moxifloxacin multi-source product circulating in Peru, manufactured in India, that does not is not interchangeable with the reference product.
El objetivo del estudio fue determinar la equivalencia terapéutica evaluada mediante estudios in vitro de cuatro marcas de medicamentos conteniendo amoxicilina, doxiciclina y fluconazol adquiridos en establecimientos farmacéuticos de Lima Metropolitana y establecer su intercambiabilidad con un producto de referencia (PR). Se empleó un método validado de espectrofotometría ultravioleta visible para determinar el perfil de disolución. El factor de similitud (f2) se utilizó para establecer la equivalencia terapéutica, considerándose equivalentes si los valores de f2 se encontraban entre 50 y 100. Para doxiciclina los cuatro medicamentos fueron equivalentes in vitro al PR, para amoxicilina sólo dos medicamentos fueron equivalentes in vitro al PR y para fluconazol ninguno fue equivalente in vitro al PR. Se concluye que algunos medicamentos de amoxicilina y fluconazol que circulan en el mercado nacional no cumplen con la equivalencia terapéutica evaluada mediante estudios in vitro; es decir, no son intercambiables.
Objetivo: Determinar el porcentaje de medicamentos de mala calidad: Falsificados y sub estándar, analizados en el “Centro Nacional de Control de Calidad (CNCC)” del Instituto Nacional de Salud (INS) en el período 2005-2014 y determinar sus tipos y características. Material y Métodos: Se elaboró una ficha para recoger los datos, los cuales fueron tomados directamente de los informes emitidos por el CNCC y del Sistema Integrado de Gestión de Laboratorios (SIGEL). Resultados: El porcentaje de medicamentos falsificados con relación al total de medicamentos analizados fue 3,0 % en 2005, 5,0 % en 2006, 7,3 % en 2007, 9,2 % en 2008, 11,4 % en 2009, 6,7 % en 2010, 9,9 % en 2011, 10,6 % en 2012, 8,4 % en 2013 y 7,6 % en 2014. Los principales grupos de medicamentos falsificados según la clasificación ATC de la OMS fueron: sistema nervioso (21,9 %), antiinfecciosos para uso sistémico (20,8 %) y sistema musculo esquelético (18,4 %). Los tipos de falsificación predominantes fueron: Aquellos que contienen la dosis correcta del principio activo pero el fabricante es distinto al declarado (44,3 %) y aquellos que no contienen ningún principio activo a pesar de lo señalado en el rotulado (37,4 %). El porcentaje promedio de medicamentos de calidad subestandar fue 6,8 %, entre el 2005-2014. En los tres últimos años, los ensayos críticos de los medicamentos de calidad subestandar representaron el 73 %. Conclusión: Entre el 2005 y 2009 hubo un aumento en el porcentaje de medicamentos falsificados, y en el período del 2010 al 2014, este porcentaje disminuyó. Respecto a los medicamentos de calidad sub estándar la mayor proporción corresponde a ensayos que son críticos para la calidad de un medicamento.
