Introducción: en Colombia no se dispone de datos suficientes y significativos sobre la caracterización del carcinoma de amígdala. Por esta razón, el propósito del presente estudio es describir una cohorte de pacientes de una institución local con disponibilidad de grupo multidisciplinar para el tratamiento del cáncer de cabeza y cuello. Materiales y métodos: estudio observacional descriptivo de una cohorte de pacientes con cáncer de amígdala, realizado desde agosto de 2015 hasta febrero de 2020. Se incluyeron variables clínicas y moleculares y los desenlaces clínicos respectivos. Se realizó un análisis descriptivo con reporte de frecuencias absolutas y posteriormente, se efectuó tanto un análisis de supervivencia mediante el método de Kaplan-Meier, como una comparación entre factores mediante un Log-rank Test con significancia menor a 0.05.
Introducción: la evidencia mundial demuestra que en el CPCNP metastásico con una mutación sensibilizante de EGFR, los inhibidores de EGFR son superiores a la quimioterapia citotóxica en el tratamiento de primera línea, sin embargo, en algunos países como Perú y Colombia hay un acceso limitado a las pruebas moleculares para definir inicio de estas terapias dirigidas. En este escenario, es frecuente que los oncólogos inicien la quimioterapia hasta que estén disponibles los resultados de la prueba de mutación de EGFR. No hay datos del mundo real para medir el impacto de este comportamiento en la supervivencia general de estos pacientes. Materiales y métodos: se realizó un estudio natural experimental, utilizando la situación natural del Perú, en la que algunos pacientes inician quimioterapia citotóxica de primera línea antes de la ITK por mal acceso o retraso en la obtención de resultados para el estado de mutación del gen EGFR. Para ello, se dividió una cohorte del mundo real en dos grupos, Grupo 1: pacientes con pruebas moleculares tardías que iniciaron quimioterapia (dupleta basado en platinos) y posterior cambio a ITK EGFR y, Grupo 2: pacientes con resultados oportunos del estado mutacional para gen EGFR que inició EGFR ITK en la primera línea, en este caso Erlotinib. Para estos análisis desarrollamos una metodología predictiva, utilizando modelos de redes neuronales; estos son modelos multicapa que permiten realizar predicciones según el peso de cada variable y utilizamos modelos de regresión Cox en los que hacemos predicciones en función del tiempo. Los modelos se ejecutaron en el software Orange®.
Introducción: el presente estudio describe las características demográficas y los desenlaces clínicos de los pacientes con cáncer de pulmón de célula no pequeña (CPCNP) para un centro oncológico de referencia nacional. Materiales y métodos: se realizó un estudio observacional descriptivo, retrospectivo en una cohorte de pacientes con CPCNP. Se realizó un seguimiento de 50 meses desde agosto de 2015 a octubre de 2020, analizando variables clínicas, tiempo libre de progresión, supervivencia global y complicaciones relacionadas al tratamiento. Para el análisis de supervivencia se realizó un modelo de Kaplan-Meier por el tamaño de muestra. Resultados: 68 pacientes fueron incluidos, 57.3 % corresponden a estadios IV, 16.1 % a estadios III, 11.7% a estadios II y 14.7 % a estadio I. En los estadios I ningún paciente ha progresado, en los estadios II, los estadios IIA el 42 % de los pacientes ha progresado, con medianas de supervivencia de 26 meses. En los estadios III aproximadamente el 33-36 % de los pacientes ha progresado, pero sin ningún fallecimiento este grupo. En los estadios avanzados las medianas de supervivencia según grupofueron, EGFR 25.6 m, ALK 24 m, PDL-1 >50 % 12.5 m, PDL-1 1-49 % manejados con quimioinmunoterapia 36.5m, PDL-1 <1 % sin adicción oncogénica manejados con quimioterapia 10.8m y PDL-1 <1 % sin adicción oncogénica manejados con quimioinmunoterapia 21.6m. El efecto adverso más relacionado a la inmunoterapia fue hipotiroidismo 21.7 %. En la toxicidad grado 3 relacionada a la terapia dirigida en enfermedad metastásica se presentó un 13 % de diarrea y un 13 % de hepatotoxicidad, seguido de un 8.3 % de hemoptisis por bevacizumab y un 8.6 % de complicaciones dermatológicas. La toxicidad grado 3 con quimioterapia en el escenario metastásico el evento más frecuente fue emesis en un 9 % seguido de falla renal, diarrea e hiporexia en un 4.5 %. La quimioterapia adyuvante presentó un 14.2 % de diarrea grado 3, los demás eventos adversos se presentaron en menos de dos casos. En la toxicidad severa por inmunoterapia solo se presentó un caso de colitis autoinmune y un caso de un síndrome miofascial.
