Doença ultrarrara é definida como doença debilitante ouque ameace a vida, com incidência menor ou igual a 1 (um) casopara cada 50.000 (cinquenta mil) habitantes. Várias normativasnacionais e internacionais já discutiram a obrigação do fornecimentodo medicamento de estudo que se mostrou mais eficaz em testeclínico a todos que participaram do estudo é um tema que provocadiscussão. No Brasil, recentemente a Resolução CNS nº 563 foipublicada e regulamenta o direito do participante de pesquisa aoacesso pós-estudo em protocolos de pesquisa clínica destinadosaos pacientes diagnosticados com doenças ultrarraras por um prazode cinco anos. O objetivo do presente artigo de revisão é apresentaressa resolução. Primeiramente, foram contextualizadas outrasnormativas internacionais e nacionais quando ao pós-estudo e emseguida a discussão acerca deste tema. A Resolução nº 563, de10 de novembro de 2017, é um importante progresso na proteçãodos participantes de pesquisa portadores de doenças ultrarraras,mas considerar o tratamento de doenças ultrarraras somente pelofornecimento de medicamento pós-estudo não é o mais adequado. Énecessária uma melhora em toda a rede estruturada de tratamentocomo Centros de Tratamento Especializados e uma política públicaefetiva para doenças ultrarraras com cobertura nacional.
Introdução: A síndrome coronariana aguda é uma doença isquêmica do coração cuja etiologia é complexa e multifatorial e a alimentação tem ligação com muitos fatores de risco. Objetivo: Descrever a dieta e estado nutricional de uma população com síndrome coronariana aguda no estudo Estratégia de Registro de Insuficiência Coronariana (Projeto ERICO). Metodologia: Foram selecionados 290 pacientes de uma população de 841 com diagnóstico de síndrome coronariana aguda. Foram avaliados dados demográficos, IMC e variáveis de nutrição a partir de um Questionário de Frequência Alimentar. Resultados: A idade média dos pacientes foi de 61,8 ± 12,0 anos, com 62,8% de homens, 74,1% de brancos, 31,4% com infarto do miocárdio com elevação do segmento ST. A média de IMC foi de 27,1 ± 4,5 kg/m², consumo calórico total diário de 4311,8 ± 3043,1 kcal. A dieta em geral foi hipoglicídica, normoprotéica e hiperlipídica, com consumos elevados de ácidos graxos saturados, trans e colesterol, consumo insuficiente de ácidos graxos linolênico e linoleico e consumo adequado das demais variáveis. O Índice de Qualidade da Dieta-Revisado foi de qualidade regular. Conclusão: A dieta foi considerada de insatisfatória, sendo hipercalórica com má distribuição dos nutrientes, em especial, ácidos graxos saturados.Palavras-chave: Síndrome coronariana aguda, hábito alimentar, índice de qualidade da dieta revisado, projeto ERICO
Objetivo: investigar o efeito do consumo de álcool em geral e do tipo de bebida alcoólica consumida, em diferentes medidas lipídicas. Método: o efeito do consumo de álcool foi investigado, bem como do tipo de bebida alcoólica, em diferentes medidas lipídicas, em ambos os sexos de população do Estudo Longitudinal de Saúde do Adulto. Resultados: foram realizadas análises em relação ao tipo e quantidade de consumo de bebida alcoólica por sexo. O consumo baixo-amoderado de álcool, independentemente do tipo de bebida alcoólica consumida, resultou em níveis mais elevados de HDLcolesterol, HDL2-colesterol e HDL3-colesterol em homens e mulheres. Não houve participantes da pesquisa que consumiam quantidade baixa a moderada de bebidas alcoólicas destiladas. Os triglicerídeos tiveram efeitos inversos para homens e mulheres no perfil lipídico. Para homens, bebidas destiladas contribuíram para melhor perfil dos triglicerídeos, enquanto para mulheres foi o contrário. Homens que consumiam bebidas alcoólicas destiladas tiveram níveis menores de triglicerídeos e mulheres que consumiam bebidas alcóolicas destiladastiveram níveis maiores. Nossos resultados estão de acordo com os de estudos anteriores. Conclusão: o consumo de álcool resultou em níveis diferentes de medidas lipídicas séricas em homens e mulheres. Dessa forma, conclui-se que a resposta ao álcool tem diferenças biológicas.
