A sialometaplasia necrosante (SN) é doença inflamatória de natureza benigna que se manifesta como úlceras profundas e irregulares de crescimento rápido, acometendo glândulas salivares menores, simulando neoplasias malignas. Relata-se caso de SN que se destaca pela forma de apresentação cística em assoalho da boca de mulher de 70 anos, com boa evolução pós-operatória e sem evidências de recidiva um ano após a ressecção.
