Background: Multiple sclerosis (MS) is an incurable chronic disease that predominantly affects young adults. It has a high socio-economic impact which increases as disability progresses. An assessment of the real costs of MS may contribute to our knowledge of the disease and to treat it more efficiently. Our objective is to assess the direct and indirect costs of MS from a societal perspective, in patients monitored in our MS Unit (Baix Llobregat, Catalonia) and grouped according to their disability (EDSS).

En el presente trabajo se propone indagar sobre las prácticas alimentarias de los grupos cazadores-recolectores y pescadores que habitaron el litoral fluvial bonaerense hace ca. 2000 años. Estos hábitos han sido estudiados desde diferentes metodologías, tales como el análisis de microrrestos vegetales preservados en tiestos cerámicos e instrumentos de molienda, que han permitido identificar tareas de procesamiento de vegetales silvestres. También se han trabajado aspectos morfo-funcionales a través de técnicas arqueométricas, reconociendo diferentes tipos de vasijas con funciones análisis de ácidos grasos absorbidos y adheridos en las paredes de contenedores cerámicos arqueológicos y experimentales. Estos datos han sido interpretados en forma conjunta con la información procedente del registro arqueofaunístico, arqueobotánico y tecnológico y han permitido avanzar en la interpretación de estas prácticas. Para tal fin se han seleccionado dos sitios arqueológicos: Las Marías, correspondiente a las primeras ocupaciones registradas en la zona, y San Clemente VI, perteneciente a un momento más tardío. La elección de los sitios con cronologías diferentes se fundamenta en el interés de analizar los cambios y/o continuidades en las prácticas vinculadas al procesamiento de alimentos y manejo del entorno.

Multiple sclerosis (MS) is a chronic demyelinating disease, which represents a great economic burden to society. Cost-of-illness studies of MS tend to underestimate the intangible costs related to pain, anxiety and helplessness. The purpose of this study was to estimate the intangible costs of MS, and determine whether these costs increase as disability progresses. We studied 211 consecutive patients with MS who attended our MS unit. Patients mean age was 41.6 (SD: 10.7) years, 69% were female, and their mean Expanded Disability Status Scale (EDSS) score was 2.47 (SD: 2.05). Quality-of-life was measured with the EuroQoL visual analogue scale. Quality-adjusted life year (QALY) was calculated for each patient. Patients were grouped into five disability stages according to their EDSS, and QALY was compared between patients and a group of healthy controls matched by age and sex. A benchmark value was ascribed to each QALY lost, and the intangible costs per patient-year were calculated as Euros 0 (EDSS =0), Euros 1100 (EDSS =1-3), Euros 8250 (EDSS =3.5-5.5), Euros 9900 (EDSS =6-7) and Euros 11,000 (EDSS >7.5). Sensitivity analysis showed a similar progression of costs. We conclude that intangible costs are relevant in MS, especially when disability increases. Although the method to calculate the costs remains controversial, we consider that they should be included in cost analysis of MS.

SUMMARY:Pecan nut expeller cake (PNEC), a by-product of pecan oil extraction, was characterized and the effect of packaging and storage conditions was investigated. Samples were packaged under vacuum or in normal atmosphere and stored (10 or 20 ºC) for 12 months. Its composition, water and oil absorption, fatty acids profile, lipid oxidation (thiobarbituric acid reactive substances, TBARS), antioxidant activity [Diphenyl-1-(2,4,6-trinitrophenyl)hydrazyl] radical, DPPH• and [(2,2′-azinobis-(3-ethylbenzothiazoline-6-sulfonic acid)] radical cation, ABTS•+ ), and microbiological quality were analyzed. PNEC showed high lipid and fiber contents (51.4 and 21.6 g/100g, respectively) with good water and oil absorption capacity. Vacuum storage at 10 ºC preserved antioxidant capacity (TBARS: malondialdehyde= 0.23 mg/kg, IC 50 DPPH = 9.0 mg/mL, and IC 50 ABTS = 21 mg/mL after one year storage). Extremely low mold and yeast levels were found in PNEC in low permeability vacuum packages, regardless of the storage temperature. Thus, PNEC could retain suitable quality attributes for up to 12 months using the right combination of packaging and temperature.KEYWORDS: Antioxidant capacity; Fatty acid profile; Pecan nut expeller cake; Storage; TBARS RESUMEN: Caracterización de la torta prensada de nuez de pecan y efecto del almacenamiento sobre su calidad microbiológica y oxidativa. La torta de prensada de nuez de pecan (PNEC), un sub-producto dela extracción de aceite de pecan, fue caracterizado y se investigó el efecto del embalaje y condiciones de almacenamiento. Las muestras fueron envasadas con y sin vacío y almacenadas (10 o 20ºC) durante 12 meses. Se analizó la composición, absorción de agua y aceite, perfiles de ácidos grasos, oxidación lipídica (sustancias reactivas al ácido tiobarbitúrico, TBARS), actividad antioxidante [radical (difenil-1-(2,4,6-trinitrofenil) hidracilo] DPPH• y catión radical [ácido (2,2′-azinobis-(3-etilbenzotiazolin-6-sulfónico)], ABTS•+ ) y calidad microbiológica. El PNEC mostró altos contenidos de lípidos y fibra (51.4 y 21.6 g/100g, respectivamente) con buenas capacidades de absorción de agua y aceite. El almacenamiento bajo vacío a 10ºC preservó la capacidad antioxidante (TBARS: malondialdehído, MDA = 0.23 mg/kg, IC 50 DPPH = 9.0 mg/mL, y IC 50 ABTS = 21 mg/mL luego de un año de almacenamiento). Los recuentos de hongos y levaduras fueron extremadamente bajos en los tratamientos con vacío y envases de baja permeabilidad, para ambas temperaturas. Por lo tanto, el PNE pudo retener adecuados atributos de calidad hasta 12 meses empleando la adecuada combinación de envase y temperatura.
