A alteração no perfil demográfico da população mundial é notória, com destaque para o envelhecimento. Nesse sentido e, em decorrência disso, questões como a da ocorrência de polifarmácia nessa população causam grande impacto. A classe dos Inibidores de Bomba de Prótons (IBPs) é utilizada indiscriminadamente, podendo ocasionar potenciais efeitos adversos diante do seu uso prolongado. DISCUSSÃO: Há diferentes mecanismos dos IBPs que podem ocasionar efeitos adversos, com seu uso a longo prazo. As informações diferem conforme o ensaio clínico analisado. METODOLOGIA: Trata-se de um estudo de revisão narrativa, o qual objetiva transcorrer sobre os efeitos do uso prolongado dos IBP em idosos. As buscas foram realizadas nas bases de dados PubMed, EBSCOhost e SciELO, e foram aplicados os descritores: Inibidores das bombas de prótons, idoso, uso inadequado de medicamentos, osteoporose e fraturas ósseas. RESULTADOS: Foram contemplados 24 artigos com diversas perspectivas acerca dos prejuízos causados pelo uso prolongado de IBPs na população idosa, resultando na divergência de resultados. CONCLUSÃO: Não há consenso sobre os efeitos da abordagem terapêutica em questão, estudos evidenciam a relação entre o uso prolongado dos IBPs e efeitos adversos, todavia, são necessários estudos sistemáticos e ensaios clínicos randomizados. A classe deve ser prescrita com cautela, somente quando necessário.
Resumo Com o aumento da duração e frequência das viagens aéreas, observou-se um aumento da prevalência de tromboembolismo venoso nos passageiros. Este estudo avaliou a eficácia do uso de meias elásticas de compressão graduada para a prevenção de tromboembolismo venoso desencadeado por viagens aéreas com duração maior que 3 horas de voo. Trata-se de uma revisão sistemática de ensaios clínicos. A qualidade metodológica dos estudos e o nível de evidência científica foram avaliados pelo Consolidated Standards of Reporting Trials e Grading of Recommendations Assessment, Development and Evaluation. Foram identificados 34 artigos, entretanto apenas oito atenderam aos critérios de elegibilidade. Os desfechos incidência de tromboembolismo venoso e edema foram avaliados em 2.022 e 1.311 passageiros, respectivamente. Os estudos demonstraram evidências de alta qualidade para a prevenção de edema e de moderada qualidade para a redução da incidência de tromboembolismo venoso com o uso de meias elásticas de compressão graduada durante viagens aéreas.
INTRODUÇÃO: A Doença de Crohn (DC), uma Doença Inflamatória Intestinal (DII) cuja patogênese é caracterizada por um desequilíbrio imunológico, é associada a fatores genéticos, ambientais e à microflora intestinal (MI). Isto posto, o presente artigo objetiva elucidar a relação entre MI e etiologia da DC. METODOLOGIA: Trata-se de uma Revisão Integrativa, com análise de 22 estudos publicados entre 2016 e 2020, disponíveis no portal da PubMed. RESULTADOS: A busca foi constituída por 4.782 artigos, com seleção de 22 estudos. A população amostral variou de 19 a 895 participantes. Observou-se que uma DII pode ser precedida por disbiose da MI, corroborando uma reação inflamatória local. Assim, determinou-se excesso de Enterococcus e Escherichia, associado a diminuição de Faecalibacterium, Gemmiger, Rominocuccus e Dialister. Ademais, dieta rica em sacarídeos, uso de antibióticos na infância e industrialização elevaram a incidência em ambientes urbanos. DISCUSSÃO: Na DC, a interação entre suscetibilidade genética, fatores ambientais e disbiose intestinal, resulta em comprometimento da barreira epitelial e consequente resposta imune anormal. CONCLUSÃO: Conclui-se que há uma forte relação entre disbiose intestinal e DC. Contudo, apesar das pesquisas pré-existentes, os fatores etiopatogênicos da DC não são bem elucidados. Portanto, ressalta-se a necessidade de estudos mais aprofundados acerca da temática proposta.
