Introdução: A avaliação fetal intraparto tem grande impacto na detecção de hipóxia fetal, que consiste em uma das principais causas de morbidade e mortalidade perinatal evitável. O método mais utilizado com essa finalidade é a monitorização eletrônica fetal contínua (MEFC). Em decorrência da sua baixa especificidade, são necessárias técnicas adicionais, como ausculta intermitente, estimulação digital no escalpe, oximetria de pulso, eletrocardiograma e amostras de sangue do couro cabeludo fetal. Objetivo: Analisar métodos de vigilância fetal intraparto para auxiliar na identificação dos fetos em estado de hipóxia, permitindo que medidas sejam tomadas para evitar complicações perinatais graves. Métodos: Revisão da literatura com base em pesquisa nas bases de dados Medical Literature Analysis and Retrieval System Online (MEDLINE) e Literatura Latino-Americana e do Caribe em Ciências da Saúde (Lilacs). Foram incluídos artigos originais e revisões bibliográficas atualizados, utilizando os descritores: “monitorização fetal”, “trabalho de parto” e “sofrimento fetal”. Resultados e conclusão: A ausculta intermitente é utilizada em gestações de baixo risco e, quando comparada à MEFC, apresenta menores taxas de intervenções desnecessárias. Entretanto, tem-se menor mortalidade quando a MEFC é utilizada. Dessa forma, a monitorização eletrônica fetal contínua é a base da avaliação do estado fetal em gestações de alto risco, sendo um bom preditor de resultados perinatais favoráveis, mas não de efeitos adversos. Os registros anormais não apresentam relação direta com a hipóxia, mas alertam para a possibilidade de tal gravidade, enquanto os achados normais indicam bem-estar fetal. Esse método posiciona o feto em três categorias: na I o traçado é normal, a II indica traçado anormal e na III o traçado é indeterminado. A estimulação digital do couro cabeludo, durante o exame vaginal, promove avaliação indireta do estado ácido-básico fetal, sendo recomendada quando o traçado é anormal. Caso o feto apresente aceleração da frequência cardíaca, isso indica pH sanguíneo acima de 7,2, no entanto a ausência dessa resposta não prediz o comprometimento fetal. A oximetria de pulso e o eletrocardiograma fetal não são utilizados na prática obstétrica em decorrência de resultados com benefícios parciais, sendo necessária melhor avaliação para o feto. A coleta de amostras de sangue do couro cabeludo fetal para a determinação de pH é capaz de reduzir taxas de intervenção cirúrgica e é recomendada em casos de traçado indeterminado ou anormal, com idade gestacional maior ou igual 34 semanas. Entretanto, esse método é um exame invasivo que necessita de conhecimentos técnicos, apresenta divergências na interpretação do pH obtido e o resultado negativo não assegura o bem-estar fetal. Assim, conclui-se que a monitorização eletrônica fetal contínua é obrigatória em gestações de alto risco, uma vez que resultados normais indicam manutenção da conduta e padrões anormais alertam para possível quadro hipoxêmico.
