This article presents the synthesis, self-assembly, and biological activity as transfection agents for pDNA, siRNA, and mRNA of novel pyridinium pseudogemini surfactants, interfacially engineered from the most efficient gemini surfactants and lipids generated in our amphiphile research program. Formulation of novel amphiphiles in water revealed supramolecular properties very similar to those of gemini surfactants, despite their lipidlike charge/mass ratio. This dual character was found also to enhance endosomal escape and significantly increase the transfection efficiency. We were also successful in identifying the parameters governing the efficient delivery of pDNA, siRNA, and mRNA, drawing valuable structure-activity and structure-property relationships for each nucleic acid type, and establishing DNA/siRNA/mRNA comparisons. Several supramolecular complexes identified in this study proved to be extremely efficient nucleic acid delivery systems, displaying excellent serum stability and tissue penetration in three-dimensional organoids.

The use of pharmacogenomics has become more prevalent over the past several years in treating many disease states. Several cytochrome P450 enzymes play a role in the metabolism of many pain medications including opioids and antidepressants. Noncytochrome P450 enzymes such as methylenetetrahydrofolate reductase (MTHFR) and catechol-O-methyl transferase (COMT) also play a role in the explanation of opioid dosage requirements as well as in response to certain antidepressants. We present the case of a patient with reduced COMT and MTHFR expression treated with leucovorin 10 mg daily for the management of chronic pain. The use of leucovorin in this patient decreased pain scores, which were clinically significant and increased functionality. This case demonstrates the importance of pharmacogenetics testing in patients, as this can help direct providers to better therapeutic options for their patients.

Controlled substances are among the most highly regulated and highly abused medications. The purpose of this study was to describe the classification systems for controlled substances in 23 countries and make comparisons. From this review, recommendations are provided for improving classification systems and their impact on medication use.   Type: Student Project

Pharmacists are the most accessible health care providers to the general public. Pharmacists are experts in helping patients get the most out of today’s complicated medications. Pharmacists ensure safe and effective use of medications. The American Pharmacists Association pitched a campaign, “Know your medicine, know your pharmacist.” Before one can know their medicine, one must know their pharmacist. This study was conducted to see whether older or younger and male verse female patients knew their pharmacist’s name better in the Philadelphia area, as a pilot study.   Type: Commentary
