BackgroundThe prevalence of obesity and obesity-related disorders is rapidly increasing among the Chinese populations. Bariatric surgery is becoming more and more popular in China, yet little cases were performed compared with western countries. The acceptance of this new treatment modality in Chinese bariatric surgery candidates was seldom studied.ObjectiveTo investigate the factors affecting the choice of bariatric surgery in Chinese patients with obesity and metabolic disorders, so as to promote the popularization of bariatric surgery in developing countries like ChinaMethodsPatients with obesity and related metabolic disorders meet the indications for bariatric surgery in the Department of Metabolic and Bariatric Surgery in the First Affiliated Hospital of Jinan University between January 2016 and April 2017 were asked to answer a questionnaire about the demographics of the patients, social economic status, present and past history, family history, etc. The data collected and the relationship of the acceptance of bariatric surgery were analyzed.ResultsThere were 157 patients (51 males, 32.5%; 106 females, 67.5%) with mean BMI 38.7 ± 8.1 kg/m2 answered the questionnaire. One hundred twenty-three of them (78%) accepted bariatric surgery. By univariate analysis, it was found that patients’ weight, BMI, family support, medical insurance, past surgical history, family history of T2DM, and obesity-related comorbidities and symptoms are correlated with the acceptance of bariatric surgery. By multivariate analysis, it was found that patients’ weight (P = 0.024), BMI (P = 0.007), family support (P < 0.001), medical insurance (P < 0.001), past surgical history (P = 0.011), family history of T2DM (P = 0.020), and obesity-related comorbidities and symptoms (P = 0.030) are statistically significant and were positively correlated with the acceptance of bariatric surgery. Age, height, gender, history of smoking and alcohol consumption, family history of obesity, history of hypertension and T2DM, education level, and marital status were not statistically significant (P < 0.05).ConclusionsPatients with heavier weight, higher BMI, family support, medical insurance reimbursement, past surgical history, family history of T2DM, and obesity-related comorbidities and symptoms are more likely to consider bariatric surgery in Chinese bariatric surgery candidates. It will be important to provide appropriate healthcare education and support to patients focusing on both obesity-related health risks and options of surgical treatment so to improve their acceptance of bariatric surgery.

Purpose: To explore the effect of using WeChat platform to assist extended care on the compliance behaviors, health locus of control and quality of life of patients who were undergoing laparoscopic sleeve gastrectomy (LSG) bariatric surgery. Methods: A total of 120 patients from July 2018 to June 2019 in the First Affiliated Hospital of Jinan University were enrolled in the study. 120 patients were randomly divided into control and intervention groups containing 60 patients respectively. In the control group, patients were given routine extended care. In the intervention group, WeChat platform extended care was adopted in addition to routine extended care. The compliance behaviors of patients in the two groups were collected and compared, and health locus of control and quality of life of patients in the two groups before the intervention and 3 months after the intervention were evaluated using the multidimensional health locus of control scale (MHLC) and 36 item short form health survey (SF-36). Results: The compliance rates of the intervention group were higher than those of the control group (P<0.05). Before the intervention, there were no significant differences between the two groups of patients' health locus of control and quality of life (P>0.05), but 3 months after the intervention, compared with the control group, the health locus of control and quality of life of the intervention group were significantly improved (P<0.05). Conclusions: Application of WeChat platform to assist extended care is beneficial to the enhanced compliance behaviors, improved health locus of control and quality of life of patients undergoing LSG bariatric surgery.

Purpose: To explore the effect of medical nursing integrated nursing mode on the quality of care of patients undergoing laparoscopic Roux-en-Y gastric bypass (LRYGB) bariatric surgery. Methods: A total of 60 patients from January 2018 to December 2018 were enrolled as the control group, and 60 patients from January 2019 to December 2019 were enrolled as the intervention group. In the control group, patients were cared by the routine nursing mode. In the intervention group, medical nursing integrated nursing mode was adopted in addition to the routine LRYGB nursing process. The waiting time of preoperative examination, preoperative HAMA and SDS scores, postoperative complication rates, time indicators of nursing quality and patient satisfaction in the two groups were collected and compared. Results: The waiting time of preoperative examination was shortened (P<0.05), preoperative anxiety and depression were reduced (P<0.05), the complication rates of postoperative bleeding, infection, wound pain and abdominal distension were reduced (P<0.05), the time indicators of nursing quality were shortened (P<0.05) and patient satisfaction was improved in the intervention group compared to the control group (P<0.05). Conclusions: Application of medical nursing integrated nursing mode in patients undergoing LRYGB bariatric surgery can alleviate the negative emotions of anxiety and depression before surgery, reduce the postoperative complication rates, improve the quality of care and patient satisfaction.

In order to accurately calculate the fatigue life of the sub-frame weld of the mixer truck during the design stage, fatigue analysis of the weld is carried out by equivalent structural stress method. Through the finite element analysis of the frame, the key weld position is researched. By using the inertial release method, the stress response of the frame under the single excitation of the unit load is obtained. Combined with the basic principle of equivalent structural stress, the stress at the weld position is calculated, and the main S-N curve with a survival rate of 50% is selected to predict the fatigue life of the weld. The results show that the fatigue life of the key weld is 105.803 cycles, which fails to reach the design target value. The equivalent structural stress method has the characteristics of grid insensitivity and can better predict the fatigue life of the sub-frame weld of the mixer truck.
