RESUMEN Introducción:Casos Clínicos: Discusión: Palabras clave:La Hepatitis autoinmune (HAI) en niños se constituye en una enfermedad inflamatoria del hígado, caracterizada histológicamente por un denso infiltrado de células mononucleares en el sistema porta, y serológicamente por la presencia de auto-anticuerpos específicos del hígado y aumento de los niveles de transaminasas e IgG, en ausencia de una patología conocida que pueda afectar el hígado. Se presenta un total de 10 casos con diagnóstico de HAI, la media de edad es de 12 años, con 2 de sexo femenino (20%) y 8 masculino (80%); 4 casos fueron de tipo 1, un caso de tipo 2, y a 5 pacientes no se los pudo clasificar. La elevación de transaminasas (> x10 su valor normal) se observó en el 70% de los casos y la hipergammaglobulinemia en todos los casos. El tratamiento instaurado consistió en corticosteroides y azatioprina en todos los pacientes, con un tiempo medio de remisión de 4-6 semanas. Se observó abandono de tratamiento en 3 casos de los 10 casos, 2 de ellos con desenlace fatal.La HAI presenta manifestaciones clínicas generales y específicas en etapas tempranas de la enfermedad. El alto porcentaje de respuesta al tratamiento médico nos hace considerar que el diagnóstico y tratamiento precoz son muy importantes, con el objetivo de detener la evolución a la cirrosis.Hepatitis autoinmune, enfermedad hepática crónica, hipergammaglobulinemia. Introducción: En países en desarrollo se describen prevalencias de Helicobacter pylory (HP) de hasta 80%. Su impacto en los enfermos celiacos es controversial, pero existen indicios que podría agravar la condición celiaca. No se ha confirmado la asociación entre ambas. En nuestro medio, factores socio ambientales pudieran favorecer una elevada prevalencia del HP. Objetivos: Conocer la prevalencia de infección por HP en una población pediátrica portadora de enfermedad celiaca (EC). Materiales y Métodos: estudio observacional, retrospectivo de corte transversal revisando fichas clínicas y resultados biópsicos de pacientes sometidos a endoscopia digestiva alta por la sospecha clínica y laboratorial (marcadores serológicos positivos) de enfermedad celiaca en el Departamento de Gastroenterología del Servicio de Pediatría del Hospital de Clínicas de la Facultad de Ciencias Médicas. Resultados: Se incluyeron en el estudio 50 muestras de biopsias gástricas y duodenales. Todos reunieron criterios histológicos de EC, en 9 pacientes (18%) se observó la presencia de HP. No se encontraron diferencias significativas en aspectos socioambientales y tampoco en cuanto a prevalencia de desnutrición y anemia comparando EC con y sin HP. Llamo la atención la presencia de distensión abdominal como dato significativo en los EC portadores de HP. La gastritis crónica activa fue el hallazgo histológico prevalente (89%) en los EC con HP+. Conclusión: La prevalencia de HP en niños celiacos fue del 18%, inferior al esperado, aunque coincidente con un reporte nacional previo. Introducción: El estreñimiento funcional (EF) en niños es un problema común, constituyendo un importante problema de salud. Objetivo: Caracterización clínico-epidemiológica y percepción de padres y/o cuidadores de niños de una consulta pediátrica sobre el EF en la infancia. Materiales y Métodos: Estudio observacional descriptivo de corte transverso, utilizando la entrevista con encuesta semiestructurada a cuidadores de niños de 2 a 10 años de una Unidad Pediátrica Ambulatoria. Resultados: sobre 374 encuestados 251(67%) niños estaba al cuidado de sus padres y provenían del departamento central. La frecuencia de EF fue de 4,8% (4.4% en preescolares y 5,1% en escolares). En escolares, predominó en varones. En niños con EF se refirió defecación dolorosa en 66,6 %, antecedente familiar de estreñimiento en 61%, ingesta excesiva de lácteos en 39% y 17% no consumía ninguna fruta o vegetal. El 55,5% de niños inició entrenamiento esfinteriano antes de los 24 meses. Se observó 11% de cuidadores sin escolaridad. Desde la percepción de los encuestados se consideró estreñimiento: esfuerzo defecatorio (27%), ausencia de defecación diaria (19,7%), deposiciones duras y dolorosas (34,4%) y necesidad de uso de laxantes (18,9%). 367/374 (98.1%) conocía los hábitos intestinales de los niños a su cargo en frecuencia, características y síntomas. Refirieron utilización de supositorios, laxantes y enemas en 100% en EF. Conclusiones: La frecuencia del EF fue inferior al promedio mundial reportado de 12%, con predominio en varones en edad escolar. Se constató antecedente familiar de estreñimiento, conceptualización poco clara sobre EF, prácticas inapropiadas sobre prevención y manejo del EF.
