Between January 1975 and December 1983, 24 of 183 patients (13%) with familial adenomatous polyposis (FAP) seen at the Mayo Clinic had mesenteric fibromatosis (MF). MF was found most often in FAP patients with associated extra-colonic "Gardner" signs (19 patients) and those who had had previous abdominal surgery (20 patients). In 4 patients, MF appeared spontaneously. The male-to-female ratio was 0.4, with a median age of 31 years in women and 37 years in men. Ten of 24 patients (42%) had been asymptomatic prior to diagnosis at time of surgery for FAP. Complications of the disease included intestinal or urinary tract obstruction. Minimal surgical manipulation seemed to be associated with fewer postoperative complications and a lesser risk of regrowth of the tumor. Nonsurgical treatment, including tamoxifen and sulindac in combination, may be beneficial. Surgery should be reserved for relief of obstruction, and bypass is preferred to resection.

SouhrnVýchodiska: Nádorová onemocnění jsou ve většině případů multifaktoriální etiologie, na jejich vzniku se podílejí jak faktory vnějšího, tak vnitřního prostředí. Hereditární nádorové syndromy jsou převážně autozomálně dominantně dědičná onemocnění, s neúplnou, nicméně velmi vysokou penetrancí. Případ: Osmnáctiletá pacientka, virgo, přichází v červnu 2018 k ošetřujícímu gynekologovi pro metroragii. Magnetická rezonance pánve odhalí tumor velikosti 80 × 90 × 80 mm vycházející z oblasti děložního hrdla. Histologicky je potvrzen velmi vzácný, tzv. hyperkalcemický typ malobuněčného karcinomu děložního hrdla. Došetření germinálního exomu odhalilo patogenní variantu genů PALB2 a BRCA2 s doporučením podrobného vyšetření cestou lékařské genetiky. Závěr: Klinické zkušenosti s tímto typem nádoru jsou velmi omezené, ale nacházejí průnik v několika aspektech -gynekologické malobuněčné karcinomy jsou velice vzácná agresivní onemocnění s velmi špatnou prognózou, postihující převážně mladé ženy. Nejčastěji vycházejí z vaječníků, na tato onemocnění je relativně nejvíce dat, ale jsou popsány i případy lokalizace na cervixu, endometriu, v pochvě i vulvě. Jedná se o extrémně vzácný případ nádorového onemocnění, což dokumentuje i nedostatek klinických dat a popsaných kazuistik z celého světa, v našem případě spojeného s velmi málo pravděpodobnou genetickou mutační náloží, kterou pacientka získala od svých rodičů. Vzhledem k této zkušenosti doporučujeme při podobném nálezu pečlivě odebrat rodinou anamnézu a zvážit genetické testování.

The following main treatment approaches are currently used in locally advanced adenocarcinomas of the oesophagus and gastrooesophageal junction (GOJ): preoperative chemoradiotherapy and surgery, and perioperative chemotherapy and surgery. While preoperative chemoradiotherapy is used primarily in oesophageal tumours, perioperative chemotherapy is the treatment of choice in Western countries for gastric cancer. The optimal treatment strategy for GOJ adenocarcinoma is still not clear. In comparison to other malignancies, biomarkers are used as predictive factors in distal oesophageal and GOJ adenocarcinomas in a very limited way, and moreover, only in metastatic stages (e.g., HER2 status, or microsatellite instability status). The aim of the article is to provide an overview of current treatment options in locally advanced adenocarcinomas of oesophagus and GOJ based on the latest evidence, including the possible potential of predictive biomarkers in optimizing treatment.
