Introdução: Em 2011, cerca de 453.000 casos iniciais de infecção por Clostridium difficile ocorreram nos Estados Unidos, houve também uma estimativa de 83.000 primeiras recorrências e 29.300 óbitos. Estima-se que 2 a 3% destes paciente desenvolverão complicações graves e apenas 1 em 100 necessitarão de abordagem cirúrgica, a qual é reservada a casos de complicações como perfuração ou não resposta ao tratamento clínico otimizado. Assim a colite fulminante ou megacólon tóxico por Clostridium difficile se torna raramente de abordagem cirúrgica e ainda esta é realizada em uma situação de urgência e emergência, com uma mortalidade estimada entre 20 a 85% dos casos. Relato do caso: Trata-se de caso com abordagem cirúrgica inovadora, seguindo os protocolos de Pittsburgh, em colite fulminante por C.difficile cuja literatura principal considera como estado da arte a colectomia total com ileostomia definitiva. Contudo centros de referência internacional, e estudos em sua maioria retrospectivos vem utilizando como boa alternativa a confecção de ileostomia em alça com lavagem intestinal utilizando o polietilenoglicol (PEG) e infusão de vancomicina via ileostomia. Conclusão: Trata-se de paciente jovem, oncológica, com rápida progressão infecciosa, com critérios de gravidade, onde a técnica cirúrgica teve boa eficácia e principalmente foi possível a preservação colônica.
