Os hemangiomas são neoplasias vasculares benignas que podem acometer a cavidade oral. Podendo ser vistas, principalmente, nos lábios, língua, mucosa jugal e palato. Se caracterizam como lesões de coloração vermelho intenso ao aspecto arroxeado, de superfície lisa ou nodular, com extensão variável. Diversas modalidades de tratamento são descritas na literatura como escleroterapia, crioterapia, corticosteroides tópicos e/ou sistêmicos, remoção cirúrgica e terapia de fotocoagulação a laser. Este artigo tem como propósito relatar um caso de hemangioma em dorso de língua o qual foi optado por uma abordagem cirúrgica da lesão por apresentar interferência com as funções fisiológicas de deglutição, mastigação e fala da paciente. Após o período de controle de seis meses, manteve-se função habitual da língua, não havendo quaiquer sinal de recidiva da lesão.
Introdução: O fibroma ossificante juvenil psamomatoide é um neoplasma de tecido conjuntivo fibroso celularizado, tipicamente não encapsulados de limites bem definidos, de crescimento rápido e assintomático, acometendo principalmente maxila de pacientes jovens. O diagnóstico geralmente acontece pela observação clínica da expansão cortical e deformidade facial evidente. Radiograficamente apresentam-se como lesões radiolúcidas circunscritas, com possíveis áreas radiopacas centrais. O tratamento cirúrgico através da excisão cirúrgica e curetagem parece ser o mais adequado em vista da agressividade expansiva da lesão e da baixa taxa de recidiva. Objetivo: O objetivo desse trabalho é relatar o tratamento cirúrgico de um paciente do gênero masculino, jovem, diagnosticado com fibroma ossificante juvenil psamomatoide de grande dimensões em seio maxilar esquerdo. Caso clínico: Clinicamente assintomático, com expansão da cortical óssea em fundo de sulco maxilar esquerdo, divergência de raízes dentárias, estreitamento da fossa nasal e deformidade facial esquerda, o exame radiográfico panorâmico evidenciava lesão radiolúcida circunscrita com áreas radiopacas. O tratamento de escolha foi a excisão cirúrgica completa da lesão através do acesso único de Weber-Ferguson para prover adequado acesso a todas as regiões envolvidas e manutenção da morfologia da face por meio de reconstrução com malha de titânio. Este relato de caso ilustra a conduta frente a fibroma ossificante juvenil psamomatoíde de grande proporção. Em meio aos traumas que acometem a região facial, as fraturas faciais pediátricas são incomuns quando comparado aos adultos, não somente pelos aspectos anatômicos e fisiológicos, bem como por fatores sociais, o que os torna exposto a traumas de alto impacto durante a primeira infância. As fraturas de mandíbula são as mais comuns, aproximadamente metade das fraturas faciais, incluem o osso mandibular. O manejo do trauma maxilofacial nessa população além de controverso, é particularmente desafiador pois deve ser determinado a partir de diversos fatores. Para tanto se faz fundamental e imprescindível compreender e esquematizar as evidências científicas que envolvem esse assunto afim de selecionar a melhor terapia e gerenciar tais lesões. Desse modo, o presente trabalho tem por objetivo relatar uma série de casos de trauma de face no paciente pediátrico empregando tratamento cirúrgico por meio do sistema de fixação não-absorvível (titânio). Acreditamos que independente da terapia, o objetivo principal deve ser o reestabelecimento da função, estética e anatomia da área afetada sem prejudicar o crescimento fisiológico. Além disso, é imprescindível o acompanhamento a longo prazo a fim de prevenir complicações.
A pandemia pela COVID-19 promoveu o comprometimento dos sistemas de saúde de diversos países. Este estudo retrospectivo foi realizado para avaliar o efeito provocado pela pandemia no serviço de Cirurgia e Traumatologia Bucomaxilofacial do Hospital Universitário do Oeste do Paraná, em Cascavel. Foram comparados os pacientes atendidos no período de pandemia, entre 16 de março de 2020 até 15 de março de 2021, com o mesmo período, um ano anterior, denominado pré-pandemia. Os dados foram categorizados com base no total de atendimentos, horário e data de admissão e etiologias. Variações de idade e sexo também foram avaliadas. Estatísticas foram admitidas com significância em p <0,05. Os resultados demonstram que houve redução dos casos durante a pandemia (n=321) comparado com o período pré-pandemia (n=423 p=0,03). Também houve diminuição nas admissões noturnas entre os períodos (n=126 e n=181, respectivamente) com significância (p=0,037). Em relação ao sexo, o perfil de pacientes masculinos apresentou redução (n=220 e n=303, p=0,005). A etiologia de casos de agressão física apresentou redução significativa (n=38 e n=64, p=0,016) assim como os casos de multifatorial (n=49 e n=97, p=0,011). O período de pandemia mostrou associação com a redução de pacientes com trauma bucomaxilofacial. Concluiu-se que a redução de casos admitidos no período noturno assim como de pacientes do sexo masculino pode ser atribuída como resultado das restrições de circulação que a pandemia impôs. O efeito do consequente distanciamento social, sugere uma diminuição no trauma bucomaxilofacial devido à agressão física.
