Background and Purpose: The Challenging Behavior Scale (CBS) measures the behavior of individuals with dementia. The study aims to translate the English CBS into German (CBS-G) and test its linguistic validity. Methods: The two-panel approach was used to translate the CBS. Nursing home staff reviewed the unambiguity and familiarity of the CBS-G items and the adequateness of the examples used to describe the items. Content validity indexes (CVI) and modified kappa (k) coefficients were calculated. Results: Most of the CBS-G items had excellent CVI and k values for both unambiguity and familiarity. All examples were viewed as adequate. Conclusion: A German version of the CBS that is linguistically equivalent to the original CBS is available but needs further testing of its psychometric properties.

Terms of use: Documents in Employment effects of thresholds in labour lawAbstract In Germany, there exist around 160 threshold values stating that certain labour laws and regulations apply only in establishments above a critical size (usually defined by the number of employees). As crossing a threshold may result in additional costs for the firm, it is often hypothesized that firms try not to exceed thresholds, in such a way dampening employment growth. Similarly, firms might reduce employment in order to get rid of certain regulations applying above a threshold. In our project, we have tested these hypotheses using different data sets and investigating several labour laws (such as the Disabled Persons Act, the Works Constitution Act, and various regulations creating thresholds around the size of 20 employees in a firm). In contrast to expectations, very few effects on employment dynamics have been found. Since thresholds are defined inconsistently, we have developed several suggestions for simplifying and unifying threshold regulations, which should reduce transaction costs and side effects.
Depressionen treten im höheren Lebensalter gehäuft auf und weisen als eigenständige Krankheitsentitäten verschiedenartigeAuftretensformen und klinische Manifestationen auf. Bei älteren Patienten stehen häufig somatische Beschwerden im Vordergrund, die die rechtzeitige Diagnosestellung und Therapie erschweren. Therapieverfahren sollten unter Berücksichtigung der besonderen Anforderungen des älteren Patienten ausgewählt werden. Die leichte kognitive Beeinträchtigung (LKB) ist mit einem erhöhten Risiko für die spätere Manifestation einer Demenzerkrankung assoziiert. Die unterschiedlichen Konzepte lassen sich hinsichtlich Ätiologie, symptomatischen Schwerpunkten und Verlauf unterteilen. Die Wirksamkeit pharmakologischer Ansät-ze ist bisher nur unzureichend belegt. Primär-und sekundärpräventive Maßnahmen sollten empfohlen werden. Darüber hinaus werden überblicks-artig Demenzen und die wichtigsten diagnostischen Verfahren sowie die Unterscheidung der Alzheimer-Demenz zu LKB und depressiver Pseudodemenz vorgestellt. Depressionen Depressive Störungen sind trotz ihres im höhe-ren Lebensalter gehäuften Vorkommens als bedeutsame eigenständige Erkrankungen vom nicht-pathologischen Alterungsprozess abzugrenzen. Zu den begünstigenden Faktoren zäh-len u.a. der durch Eintritt in den Ruhestand veränderte soziale Status, eine Abnahme sozialer Kontakte älterer Menschen oder ein Verlust der Selbstständigkeit infolge körperlicher Gebrechlichkeit. Als Leitsymptome depressiver Störungen sind gedrückte Stimmung, innere Freudlosigkeit und verminderte affektive Resonanz, aber auch eine erhöhte Grübelneigung sowie ein gestörtes Antriebsniveau zu nennen. Erkrankte empfinden zumeist Sinn-und Hoffnungslosigkeit, innere Leere oder auch ein "Gefühl der Gefühllosigkeit". Die Gedankenwelt ist häufig von Selbstvorwür-fen, Angst-und Schuldgefühlen bestimmt, das Suizidrisiko ist deutlich erhöht. Patienten klagen zudem oftmals über ein Kloßgefühl im Hals, das sog. "Globusgefühl", des Weiteren über Druckgefühle im Kopf-, Brust-oder Bauchbereich und über Schmerzen einzelner Gliedmaßen oder ganzer Körperteile ohne erkennbare somatische Ursache. Ebenso werden Interessenverlust, Gedächtnis-und Konzentrationsbeeinträchtigun-gen erlebt. Zusätzlich können vegetative Funktionsstörungen mit Symptomen wie Abgeschlagenheit, Schlafstörungen, verändertem Blutdruck, Inappetenz oder Gewichtsverlust auftreten. Depressionen im Alter sind allerdings vielfach durch einen somatischen Schwerpunkt der Symptome bei häufiger Abwesenheit einer gedrückten Stimmung gekennzeichnet, sodass in vielen Fällen das rechtzeitige Erkennen der Erkrankung erschwert und eine angemessene Therapie hinausgezögert wird. Inadäquate Therapieformen und somatische Komorbidität erhö-hen das Risiko einer Chronifizierung depressiver Erkrankungen des höheren Lebensalters. Diagnostisch unterscheidet die ICD-10-GM [1] zwischen einer depressiven Episode (F32) als einem einmaligen Ereignis und einer depressiven Störung (F33), die durch rezidivierende Krankheitsphasen charakterisiert ist. The existing 160 thresholds are complex and defined inconsistently, making it difficult for firms to obey the law. Moreover, exceeding a threshold may result in costs for the firm such as establishing a works council or paying a penalty for not employing disabled persons. Although the empirical evidence is mixed, some studies suggest that employment growth is dampened by firms trying not to exceed thresholds. In order to minimize these transaction costs and side effects, we make several suggestions to simplify and unify the threshold regulations.

Abstract. Apathy is a diminished goal-directed behavior stemming from a lack of motivation. It is one of the most common symptoms in dementia. While the Apathy Evaluation Scale (AES) has proved to be a psychometrically robust measure for assessing apathy, the German version (AESD) requires further validation. Associations of apathy with important variables in dementia care, such as cognitive impairment or caregiver burden, were repeatedly found, though contradictory findings have also been reported. The present study assessed apathy in 100 community-dwelling persons with dementia. Their informal and formal caregivers used the German informant version of the Apathy Evaluation Scale (AESD-I) to investigate the severity of dementia and cognitive deficits as measured with the Mini-Mental State Examination (MMSE) and Global Deterioration Scale (GDS), respectively. A reliability analysis was performed to estimate the internal consistency of the AESD-I, and the data were tested for potential correlations with results from a shortened version of the AESD-I (AESD16-I). Cronbach’s α was .880 for the AESD-I and .904 for the AESD16-I. The prevalence of apathy was 77%, its severity was on average moderate to high. Significant (p < .05) negative correlations were shown between the AESD16-I, the Barthel Index (BI), and the Quality of Life-Alzheimer’s Disease (QOL-AD). Correlations between the AESD16-I, GDS, and Zarit Burden Interview were positive. After Bonferroni correction, the only correlations that remained significant were between the AESD16-I and BI and QOL-AD. The present study underlines the favorable psychometric properties of the AESD-I and confirms the high prevalence of apathy with considerable severity in the sample of community-dwelling persons with dementia. In addition, it contributes to the diversity of evidence on clinical correlates of apathy which require further clarification.
