Evidências empíricas, biológicas e clínicas apoiam que a mucosa oral é um local inicial de entrada para SARS-CoV-2 e que sintomas orais, incluindo perda de paladar / olfato e xerostomia, podem ser sintomas precoces da COVID-19 antes mesmo da febre, tosse seca, fadiga, falta de ar e outros sintomas típicos, sendo assim um sinal de alerta. Objetivo: Revisar sistematicamente os estudos acerca das manifestações orais em pacientes com COVID-19. Metodologia: Trata-se de uma revisão sistemática, a qual foi orientada de acordo com os itens do Preferred Reporting Items for Systematic Reviews and Meta-Analyses Statement e a partir da publicação de um protocolo na base de dados PROSPERO. A formulação desta revisão sistemática buscou responder a seguinte pergunta: Qual associação das manifestações orais, apresentações clínicas e tratamentos, com a COVID-19? Para tal, foi realizada uma busca nas principais bases de dados (MEDLINE, EMBASE, ScienceDirect, Scopus), e bases regionais (LILACS, SciELO). Além disso, as seguintes bases de dados de literatura cinzenta foram pesquisadas: OpenGrey.eu e DissOnline.de. Foram considerados como critérios de inclusão: (1) estudos observacionais de indivíduos com diagnóstico de infecção por COVID-19; (2) registro das manifestações orais da COVID-19. Foram considerados como critérios de exclusão: (1) estudos sobre coronavírus não-humano; (2) artigos de revisão; (3) estudos experimentais (em animais ou in vitro); (4) relato clínico de distúrbio olfatório e/ou gustativo com início prévio ao quadro de infecção por SARS-COV-2; (5) estudos de manifestações orais em pacientes com outros vírus que não seja SARS-COV-2. Foi realizada uma extração de dados, previamente definido em um formulário eletrônico com os seguintes tópicos, por exemplo: Informações básicas do estudo, Elegibilidade, Métodos, Participantes, Intervenções, Resultados, entre outros. O risco de viés foi avaliado de acordo com as recomendações do manual e da escala de Newcastle-Ottawa. Resultados: Foram encontrados 1167 artigos após a busca realizada nas bases de dados. Após a remoção das duplicatas, restaram 809 estudos para análise. Por conseguinte, com base nos critérios de inclusão, foram excluídos 624 pelo título e 150 pelo resumo, resultando em 35 artigos para leitura na íntegra e 12 artigos incluídos no estudo. O tempo médio para o surgimento da manifestação oral foi de três a quatro dias, com duração de aproximadamente 11 dias. Os pacientes do sexo feminino apresentaram mais disfunções quimiossensoriais. A ferramenta diagnóstica variou de questionários subjetivos a testes psicofísicos. As comorbidades mais associadas à manifestação oral foram as doenças cardiovasculares. De maneira geral, não houve associação da manifestação oral com a gravidade clínica da COVID-19 e instituição de tratamento. Conclusão: A manifestação oral mais prevalente foi a disgeusia. Porém, de maneira geral, não houve correlação com a gravidade clínica da COVID-19, tampouco com tratamento.
Introduction: Countless changes were necessary to overcome the coronavirus pandemic (SARS-CoV-19) since its dissemination. Measures such as physical and social distancing, constant antisepsis surveillance, urgency in checking tasks and the possibility of imminent infection were the pillars of this virus control strategy, and as a consequence, the impacts on the psychosocial development among children and adults teenagers. Psychopathological symptoms found in post-traumatic stress, anxiety and depression disorders have become predominant, with such losses being even more reinforced among children with attention deficit hyperactivity disorder (ADHD). Objective: To analyze behavioral changes among children and adolescents previously diagnosed with ADHD, during the COVID-19 pandemic, in a multidisciplinary children's clinic in the city of Maceió, in Alagoas, Brazil. METHODOLOGY: This is a prospective cohort descriptive quantitative research, carried out in a population of 40 participants, with a sample of 35 responses from parents or legal guardians of children with ADHD. Two questionnaires were carried out to assess anxiety, depression and sleep parameters, one during the quarantine period and the other after the restrictive measures of social isolation. Results: The sample had a higher prevalence among male children, with 74.3%. Of the new behaviors presented daily during the period of social distancing, irritability prevailed with 62.9%, followed by agitation, with 42.9%. In the post-quarantine period, the daily presence of these symptoms dropped to 14.3% and 17.1%, respectively. As for the reported feelings, sadness represented 45.7% of the sample, with a frequency of 1 to 2 times a week, during the quarantine period. After the return of face-to-face activities, there was an increase to 48.6%. Regarding sleep, at the peak of the pandemic, 54.3% of children showed anxiety and objection behavior at bedtime and 31.4% stated initial insomnia “every day”. With the end of social restriction, only 4% reported difficulty sleeping. Conclusion: The effects of the pandemic are perceived even more sensitively among children with ADHD. The work gathered scientific knowledge about mental health and emergency care for these children. Therefore, immediate efforts must be employed, at all levels and in the most diverse areas of knowledge, in order to minimize even more negative results in the mental health of children with ADHD in the face of the COVID-19 pandemic.
Introdução: A Infecção pelo HTLV-I é causada por um retrovírus que agride o sistema imunológico e pode levar a um processo inflamatório medular. Devido à queixa de tontura espera-se que os reflexos relacionados ao equilíbrio corporal em indivíduos com essa infecção possam estar alterados. Por esse motivo, o potencial evocado miogênico vestibular (VEMP) seria importante para avaliar o reflexo e confirmar o comprometimento da via. Objetivo: Analisar se existem diferenças significativas para as latências e amplitudes do VEMP cervical e do VEMP ocular entre indivíduos com e sem HTLV-I. Métodos: Trata-se de uma revisão sistemática, realizada nas seguintes bases de dados: Medline (via Pubmed), Web of Science, Scopus, Science Direct, Literatura Latino Americana e do Caribe em Ciências da Saúde e Scientific Eletronic Library Online. Os critérios de inclusão foram: artigos originais que abordassem o VEMP e a infecção pelo HTLV-I em adultos. E os critérios de exclusão: perda auditiva condutiva, sensorioneural igual ou superior a 50 dB em qualquer frequência; grupo controle com patologias vestibulares, neurais, diabete ou Parkinson. Resultados: Foram identificados 69 artigos. Após a leitura dos títulos, resumos e leitura do artigo na integra foram selecionados quatro artigos. Os estudos incluídos nesta revisão demonstraram prolongamento das latências ou ausência das ondas do VEMP cervical e ocular em indivíduos infectados com HTLV-I . Conclusão: Conclui-se que há um prolongamento das latências ou ausência das respostas do VEMP cervical e ocular em indivíduos infectados com HTLV-I de acordo com o comprometimento do trato vestibulospinal ou da desorganização dos aferentes primários envolvidos no reflexo vestíbulo-ocular. Introdução: Dificuldades de desempenho comunicativo e de reconhecimento de fala em ambientes ruidosos são associadas ao envelhecimento. O declínio do reconhecimento da fala com ruído competitivo é devido a uma combinação de fatores auditivos e não-auditivos que acentuam ao longo do tempo e com o avanço da idade. Objetivo: Identificar os aspectos cognitivos e auditivos que contribuem para o declínio do reconhecimento de fala no ruído em idosos. Método: Trata-se de um estudo prospectivo, analítico, observacional e transversal. A amostra foi composta por dois grupos: um de adultos e outro de idosos, selecionados conforme os seguintes critérios de inclusão: ouvintes normais; idade entre 18 e 70 anos, de ambos os sexos. Os aspectos cognitivos foram analisados pelo Montreal Cognitive Assessment e, nos processos auditivos, foram aplicados testes de audiometria tonal limiar, psicofísicos de reconhecimento de fala em escuta difícil e de resolução temporal. Resultados: Quando comparados por grupo etário, os testes psicoacústicos apresentaram diferenças significativas nas condições: 1) SSI / OD - S/R 0 (p=0,001), 2) SSI / OD - S/R -15 (p=0,000), 3) HINT / OE S/R -10 (p=0,03), 5) HINT / OE S/R -15 (p= 0,02) quando aplicado o teste Mann Whitney U. Quando comparados por grupo etário, os testes GIN e TDD não apresentaram diferenças significativas. Já quando os testes psicoacústicos foram comparados independentes do grupo etário, os sujeitos com MoCA normal e alterados não apresentaram diferenças significativas (p=0,280). Conclusão: A partir da amostra estudada, observou-se que os aspectos cognitivos não contribuíram no desempenho da percepção de fala com estímulos competitivos quando comparados os grupos de adultos e idosos. Por outro lado, os aspectos auditivos avaliados mostraram que os idosos apresentam maiores dificuldades de compreensão da fala no ruído quando comparados aos indivíduos mais jovens.
