Introdução: A meningite é uma doença infecciosa de caráter inflamatório que acomete as membranas de revestimento da medula espinhal e do cérebro, as meninges. Sua etiologia é diversa, podendo ser decorrente da infecção por bactérias, vírus, protozoários, helmintos, fungos e outros microrganismos. No Brasil ela é endêmica e, geralmente, apresentando-se mais frequentemente nos meses de inverno, com surtos ocasionais em outros períodos, o que caracteriza um padrão cíclico de sazonalidade durante o ano. Nesse sentido, a meningites infecciosa institui-se como um importante problema de saúde pública, sendo categorizada em um grupo de doenças de notificação compulsória, cujos dados são armazenados no Sistema de Informação de Agravos de Notificação (SINAN). Objetivo: Analisar a incidência de casos de meningite no Estado de Alagoas notificados no SINAN durante os últimos cinco anos. Metodologia: Trata-se de um estudo transversal, de caráter quantitativo, retrospectivo. Os dados clínicos, epidemiológicos e laboratoriais foram obtidos no banco oficial do SINAN, do Ministério da Saúde, através do Departamento de Informática do Sistema Único de Saúde (DATASUS), entre os anos de 2015 e 2019. Resultados e Discursões: Foram notificados um total de 524 casos de meningite no Estado de Alagoas no período analisado, tendo uma média de 104,8 notificações por ano. Abiotrophia defectiva is a rare agent of endocarditis and subacute presentation may delay the diagnosis. We present the case of a 41-year-old male who was admitted to the hospital for further investigation regarding a consumptive syndrome with microcytic anaemia. Past medical history included new-onset mitral insufficiency followed by an ischaemic stroke due to small vessel disease. Thoraco-abdominal computed tomography revealed a splenic infarction. In the presence of two ischaemic events associated with mitral valve disease of unknown aetiology, we considered the possibility of subacute endocarditis. Blood cultures were positive for Abiotrophia defectiva, and transoesophageal echocardiography confirmed the diagnosis. As a subacute presentation of endocarditis, the paucity of symptoms caused a five-month delay in diagnosis. New-onset valvular disease and a stroke in an otherwise healthy young patient should always prompt proper investigation. This case highlights several complications caused by septic emboli of undiagnosed and untreated endocarditis.
