Introducción: El tratamiento quirúrgico de los tumores de cavidad oral y el seno maxilar puede dejar grandes defectos que tienen consecuencias tanto estéticas como funcionales. La masticación, la deglución y la fonación son las principales funciones comprometidas con este tipo de defectos. El colgajo temporal miofascial constituye una gran opción para la reconstrucción con colgajos regionales debido a gran vascularización y fácil transferencia. Presentamos el caso de un paciente que requirió cirugía reconstructiva con un colgajo regional miofascial tras una hemimaxilectomía por un carcinoma epidermoide en fosa nasal derecha y seno maxilar ipsilateral. Método: Paciente de 79 años sin antecedentes de importancia es valorado en hospital comarcal por una masa en fosa nasal derecha de meses de evolución, en dicho evidencian una masa que ocupa toda la fosa nasal y toman biopsia de la lesión con un TC de senos paranasales. La biopsia reporta un papiloma invertido y en el TC se observa una masa que ocupa toda la fosa nasal derecha, el seno maxilar y etmoides anterior ipsilateral con importante destrucción ósea y áreas de calcificación intralesional. Es remitido al hospital de referencia donde se realiza maxilectomía media derecha vía endoscópica con previa embolización del tumor por parte de cirugía vascular. La pieza resecada reporta una anatomía patológica de carcinoma escamoso infiltrante sobre papiloma invertido con amplia infiltración ósea. Posteriormente el paciente es llevado a una hemimaxilectomía media en conjunto con cirugía maxilofacial, durante la intervención se decide que es una lesión irresecable debido a la extensión de la misma. Se decide manejo con quimioradioterapia y un obturador con adaptación adecuada por el paciente. 2 años después el paciente presenta como secuela una comunicación orosinusal derecha importante con limitación en su calidad de vida, se realiza la reconstrucción por cirugía maxilofacial con un colgajo temporal miofascial derecho. Se cubre el defecto del temporal con una prótesis porosa de polietileno. Discusión: El colgajo temporal miofascial consiste en un colgajo tipo III según la clasificación de Mathes y Nahai. Debido a su localización y gran vascularización constituye una excelente opción para la reconstrucción de defectos en cavidad oral, órbita y senos maxilares. Es de vital importancia reconocer la fascia superficial temporal para evitar la lesión de la rama frontal de nervio facial, que constituye uno de los pocos riesgos de utilizar este colgajo. Un aspecto favorable de este colgajo en la reconstrucción de cavidad oral es su rápida epitelización debido a la protección que confiere la fascia de la saliva, esto permite el inicio temprano de la radioterapia si es necesario en el paciente oncológico. Conclusiones: El colgajo temporal tiene unas indicciones precisas que le confieren una ventaja en la reconstrucción en el paciente oncológico. Su principal ventaja constituye que se puede reconstruir de forma inmediata el defecto. La seguridad del pedículo vascular y la proximidad a cavidad oral, órbita y tercio medio de la cara, así como la rápida obtención del colgajo, lo convierten en una excelente opción para la reconstrucción en estas áreas.
Introducción y objetivo: En los últimos años se ha producido un gran avance en el tratamiento de la hipoacusia de transmisión y mixta debido al gran desarrollo de los implantes de conducción ósea. Los implantes de conducción ósea o audífonos de anclaje óseo son sistemas que se emplean para el tratamiento de hipoacusias uni o bilaterales, tanto de conducción como mixtas, y también en casos de sorderas unilaterales.Existen dos tipos de sistemas de conducción ósea: los transcutáneos y los percutáneos. En los percutáneos existe una comunicación directa entre el procesador y el implante, el cual se encuentra atravesando la piel. Sin embargo los transcutáneos mantienen intacta la piel y se unen procesador e implante por un sistema de imanes. Dentro de los transcutáneos se ha desarrollado un nuevo sistema, el sistema piezoeléctrico Osia®.El objetivo de este trabajo es presentar el nuevo sistema de Osia® a propósito del primer paciente al que se implanta en nuestra provincia.Método: Se presenta el caso de un varón de 47 años, con alergia a AINEs y yodo y sin antecedentes médicos de interés. Diagnosticado de otitis media crónica colesteatomatosa bilateral hace años. Intervenido de ambos oídos de timpanoplastia con mastoidectomía a demanda y reconstrucción y posteriormente mastoidectomía radical con meatoplastia en oído derecho tras recidiva de colesteatoma. En la audiometría tonal liminar presenta hipoacusia de transmisión bilateral, con GAP de 40dB en ambos oídos. Se decide colocación de implante transcutáneo piezoeléctrico Osia® en oído derecho.Resultados: La intervención se realiza bajo anestesia general. Inicialmente medimos sobre la piel el punto donde irá colocado el tornillo del implante, aproximadamente a la altura del meato auditivo. Se puede marcar con azul de metileno el punto del tornillo. Se pueden emplear múltiples incisiones para realizar esta técnica. En nuestro caso empleamos la incisión retroauricular que presentaba el paciente de intervenciones anteriores de ese oído, ampliando con una incisión de descarga en el borde caudal para facilitar la implantación. Se mide el grosor de piel, se recomienda que este sea menor de 9mm. En nuestro paciente rebajamos el tejido ya que presentaba demasiado volumen, lo cual dificulta la fijación de los imanes. Se diseca el plano subperióstico y se crea un bolsillo donde irá el transductor. Identificamos el punto marcado donde fijaremos el tornillo y se procede a su colocación. La perforación que se realiza tiene una profundidad de entre 3-4mm. Posteriormente aseguramos que el hueso en torno al implante es un lecho liso para la correcta colocación del transductor. Por último fijamos el transductor aplicando una fuerza de 25 Newton y procedemos al cierre por planos. En nuestro caso no se observaron complicaciones durante el procedimiento ni en el postoperatorio inmediato.Discusión: Osia® es un tipo nuevo de implante activo piezoeléctrico. En este tipo de implantes, la transmisión transcutánea entre el procesador y el transductor se produce por una señal digital de radiofrecuencia, la cual se transforma en vibraciones en el transductor que es lo que se transmitirá al hueso...
Introducción: El espacio parafaríngeo es un espacio anatómico complejo. Los tumores que surgen en esta región son raros y la cirugía constituye la primera opción de tratamiento en la mayoría de los casos(1). Aunque tradicionalmente las masas de gran tamaño en este espacio se abordan de forma transcervical, la cirugía transoral robótica (TORS) aporta grandes ventajas, siendo más segura, menos mórbida y potencialmente más efectiva, mejorando claramente el postoperatorio del paciente con menos dolor, menos riesgo de daño de estructural nobles cervicales y recuperación más rápida. La TORS permite la exéresis de lesiones de hasta 8 cm, como se ha comunicado en algunas publicaciones(2). Presentamos el caso de un paciente con una masa cervical en el espacio parafaríngeo y retrofaríngeo derecho de gran tamaño que fue resecada mediante cirugía TORS. Métodos: Paciente de 21 años con antecedente de neurofibromatosis tipo 1 que presenta una masa cervical en el espacio parafaríngeo y retrofaríngeo derecho de 71 x 32 x 45 mm de diámetros craneocaudal, anteroposterior y trasversal, aparentemente dependiente de la cadena simpática cervical, que en el momento del diagnóstico no provoca ninguna sintomatología. Durante el seguimiento del paciente mediante estudios de imagen (RM cervical) se objetiva un aumento del tamaño de la lesión, así como inicio de alteraciones en la deglución y presencia de voz engolada por oclusión significativa de la orofaringe. Se decide extirpar la masa mediante abordaje transoral con cirugía robótica (TORS). Durante el postoperatorio presentó un síndrome de Horner transitorio por afectación de la cadena simpática cervical. Además, asoció trismo y paresia del XII par craneal por compresión en el abordaje que mejoró progresivamente gracias al tratamiento logopédico recibido en el postoperatorio inmediato. Histopatológicamente el tumor fue diagnosticado de "Neurofibroma intraneural grado I (WHO)”. Discusión: La cirugía transoral robótica constituye una indicación terapéutica emergente para la resección de tumores del espacio parafaríngeo(1,2,3,4). Con los avances tecnológicos se ha extendido su indicación para tumores de mayor tamaño en los cuales el facultativo debe tener en cuenta múltiples factores del paciente y del tumor, tales como la histopatología de la lesión, extensión lateral y craneocaudal de la masa y la relación con la arteria carótida entre otros(2). Es imprescindible una buena apertura bucal. Conclusión: La cirugía transoral robótica presenta una disminución en la morbilidad cuando se realiza una adecuada selección del paciente(1,2). En el caso que presentamos se realizó una exéresis competa de la lesión a pesar del tamaño de la misma. Es necesario documentar resultados a largo plazo de este tipo de abordajes para determinar sus ventajas sobre el abordaje abierto.
Introducción y objetivo: La enfermedad de Kikuchi-Fujimoto (EKF) o linfadenitis necrotizante no granulocítica fue descrita por primera vez en 1972 en mujeres jóvenes japonesas, sin embargo actualmente se han visto casos tanto en distintas etnias como países. La etiología es desconocida, aunque se postula que podría tener un origen autoinmune o por infecciones virales, siendo los resultados de los estudios sobre la etiología viral inconsistentes.Método: Paciente de 41 años que presenta 2 adenopatías laterocervicales derechas dolorosas que aumentan de tamaño progresivamente. No presenta fiebre, sudores nocturnos ni pérdida de peso asociada. No refiere otra clínica.Resultados: En la exploración se palpan adenopatías laterocervicales derechas de 1,5cm, blanda, móvil y no adherida con el resto de la exploración dentro de la normalidad. Se realizó análisis de sangre, incluyendo proteinograma, estudio de autoinmunidad y serología de VHB, VHC, VIH, CMV, VVZ, VEB, Rubeola, Toxoplasma y Treponema pallidum. Todas ellas con resultado negativo. Se decide realizar ecografía con PAAF de la misma con el resultado compatible con linfadenitis histiocítica necrotizante o linfadenitis de kikuchi. Se decide realizar exéresis del ganglio, confirmando el diagnóstico y posterior tratamiento conservador.Discusión: La EKF es una forma benigna y autolimitada de linfadenitis necrotizante, existiendo una recurrencia del 15% hasta 8 años tras la clínica inicial. Entre el 60-90% de los pacientes presentan adenopatías posterocervicales dolorosas a la palpación, que se pueden asociar a síntomas B sistémicos. En el diagnóstico diferencial debemos incluir las linfadenitis de origen infeccioso, linfomas y enfermedades autoinmunes como el lupus eritematoso sistémico. El diagnóstico definitivo es el examen histológico de una adenopatía, en la que se observa hiperplasia reactiva con focos de necrosis paracortical con detritus celulares, rodeados por infiltrado inflamatorio de linfocitos CD4+, histiocitos CD68 y escasa presencia de polimorfonucleares neutrófilos y células plasmáticas. El tratamiento más habitual es la observación, y en caso de pacientes muy sintomáticos pueden beneficiarse de la administración de corticoides o antiinflamatorios no esteroideos.Conclusiones: La EKF se debe incluir en el diagnóstico diferencial de un paciente que presenta linfoadenopatías cervicales y fiebre de origen desconocido. A pesar de su carácter autolimitado y resolución completa en la mayoría de los casos, existe el riesgo de progresión a una enfermedad autoinmune o de una recidiva. El diagnóstico definitivo es el examen histológico.
