El cáncer de mama es uno de los tumores malignos más frecuentes en mujeres; en Paraguay presenta una alta mortalidad. La quimioterapia neoadyuvante fue introducida para disminuir el tamaño del tumor y volver resecables los tumores considerados irresecables, lo que permite realizar cirugías conservadoras y probar la sensibilidad a la quimioterapia. Material y métodos: Estudio descriptivo, transversal y retrospectivo, con componente analítico. Muestra: 130 pacientes con cáncer de mama que recibieron tratamiento neoadyuvante en el Departamento de Oncología Médica del Hospital de Clínicas FCM-UNA del 2010 al 2016. Variables: edad, sexo, tipos biológicos, grado histológico, tipo histológico, permeación linfovascular, etapa clínica, respuesta clínica, respuesta patológica en el tumor (T) y respuesta patológica en la axila (N). El muestreo fue no probabilístico, por conveniencia, de casos consecutivos. Se utilizaron medias, frecuencias y χ². Resultados: Según los criterios RECIST (Response Evaluation Criteria in Solid Tumors) 1.1, se observó mayor frecuencia respuesta parcial en el 43.85% y según los criterios de Sataloff para respuesta patológica del tumor T, mayor frecuencia de la T-C en el 37.5%; respuesta patológica en la axila N mayor frecuencia la N-D en el 36.46%. El tipo biológico más frecuente fue el luminal A, en el 31.54%. En todos los fenotipos moleculares las tasas de respuesta patológica en la mama fueron mayores en comparación con las de axila. La tasa de respuesta clínica y patológica en la mama (T) fue estadísticamente significativa según fuese el tratamiento completo o no (p < 0.05). Conclusión: La respuesta patológica en la mama fue mayor que la encontrada en la axila. Los subtipos moleculares HER2+ y los triple negativos tuvieron mayor tasa de respuesta que los luminales.
Introduction:Breast cancer is one of the most frequent malignant tumors in women; in Paraguay it has a high mortality. Sample: 130 patients with breast cancer who received neoadjuvant treatment in the Department of Medical Oncology of Hospital de Clínicas FCM-UNA from 2010 to 2016. Variables: age, sex, biological types, histological grade, histological type, lymphovascular permeation, clinical stage, clinical response, pathological response in the tumor (T) and pathological response in the armpit (N). The sampling was non-probabilistic due to the convenience of consecutive cases. Mean, frequency, and c 2 were used. Results: According to RECIST (Response Evaluation Criteria in Solid Tumors) 1.1 criteria, a higher frequency of partial response was found in 43.85%, and according to Sataloff criteria for pathological response of T tumor, the highest frequency was T-C, in 37.5%. Pathological response in the armpit N increased frequency of N-D in 36.46%. The most frequent biological type was luminal A, in 31.54%. In all molecular phenotypes, higher rates of pathological response in the breast compared to those in the axilla were observed. Rate of clinical and pathological response in breast T was statistically significant depending on whether the treatment was complete or not (p < 0.05). Conclusion: Pathological response in the breast was greater than that found in the axilla. The HER2+ and the triple negative molecular subtypes had a higher response rate than the luminal.
