Introducción: Los exámenes de alto impacto o altas consecuencias tienen una larga historia en la educación superior y han contribuido al desarrollo científico de la evaluación educativa como una disciplina sofisticada. A pesar de ello, han surgido reacciones encontradas sobre el tema en diversos sectores de la sociedad y los profesionales de la educación, cuestionando su valor real y enfatizando sus potenciales efectos negativos. Es necesaria una discusión balanceada de esta temática, fundamentada en argumentos académicos con sustento en investigación, específicamente en educación médica. Objetivo: Proveer un panorama de las implicaciones educativas de la evaluación sumativa con exámenes de alto impacto, con énfasis en la educación médica. Método: Revisión narrativa de la literatura. Se identificaron publicaciones relevantes al tema en las bases de datos disponibles de literatura académica publicada y gris, sobre los exámenes de alto impacto en educación superior en niveles internacional y nacional. Se enfocó en artículos académicos que reportaran aspectos metodológicos y resultados, principalmente en evaluación de educación médica. Discusión: Los exámenes de alto impacto han tenido en general efectos positivos en la educación, aunque también se han reportado efectos negativos importantes y sobre los cuales siempre se debe reflexionar. Existe abundante literatura sobre el tema, pero más del 95% no son trabajos formales de investigación, lo que hace difícil tener una discusión razonable usando argumentos con sustento metodológico. La mayoría de los estudios sobre este tema están publicados en el litigioso contexto de Norteamérica, por lo que es necesario realizar investigación original sobre evaluación educativa en el contexto nacional y local, sin perder la perspectiva global. Conclusión: Los exámenes de alto impacto tienen efectos positivos y negativos en el currículo, los métodos de enseñanza y las estrategias de aprendizaje. Es necesario hacer un uso prudente y profesional de los resultados de estos exámenes, incorporando el concepto moderno interpretativo de validez para obtener inferencias apropiadas de estos datos.
Introducción: Los exámenes de opción múltiple son la herramienta más utilizada en la evaluación del conocimiento en estudiantes de medicina. Se ha demostrado que tres opciones para cada ítem son suficientes, sin embargo, muchos exámenes en nuestro medio aún están compuestos por preguntas con cinco opciones. El estudio de los distractores no funcionales (DNF) es necesario para mejorar la calidad de los exámenes. Objetivo: Identificar los DNF y su comportamiento en una evaluación sumativa de altas consecuencias en estudiantes de medicina. Método: Se realizó análisis psicométrico del Examen Profesional Teórico de la Facultad de Medicina de la Universidad Nacional Autónoma de México (UNAM), en la versión de 2008. Se calcularon dificultad, discriminación y correlación punto-biserial de cada ítem y de sus cuatro distractores. Se obtuvo la frecuencia de preguntas con cero a cuatro DNF y se valoraron las diferencias de DNF por ítem y sus características psicométricas. Se contrastó el comportamiento psicométrico del examen completo, con una versión en la que se eliminaron los ítems con cuatro DNF. Resultados: El examen tuvo 420 reactivos de opción múltiple con cinco opciones de respuesta, fue contestado por 882 sustentantes. El instrumento tuvo un alfa de Cronbach de 0.93. De los 1 680 distractores evaluados, sólo 788 (46.9%) fueron funcionales. Más de dos tercios del total de ítems contaron con dos o más DNF. Se encontró un promedio de 2.12 ± 0.99 DNF por ítem, la mayoría de los cuales fueron elegidos por menos de 5% de los sustentantes. A mayor cantidad de DNF, mayor índice de dificultad y menor poder de discriminación de los ítems. Conclusiones: Los ítems con dos distractores funcionales comprenden la mayoría de los reactivos del examen. Dos distractores plausibles representan una alternativa asequible para elaborar los ítems, manteniendo o mejorando la confiabilidad y el perfil psicométrico de este tipo de evaluaciones.
Introducción: La certificación de médicos especialistas es una tarea fundamental para proteger a la sociedad y documentar el logro de competencias en un campo específico. Los procesos de certificación médica han avanzado y se han diseminado globalmente, haciéndose más sofisticados, complejos y basados en evidencia. La certificación formal de médicos especialistas en México inició en 1963, aunque la calidad y validez del proceso ha sido, por diversas razones, heterogénea. El Consejo Mexicano de Otorrinolaringología y Cirugía de Cabeza y Cuello (CMOCCC) fue creado en 1970. Recientemente el Consejo implementó una iniciativa para mejorar los procesos de desarrollo y análisis del examen de certificación. Objetivo: Describir el proceso de mejoría de calidad de un examen de certificación de una especialidad médica en México. Método: Estudio de caso único descriptivo instrumental. Revisión de los documentos normativos y reportes de exámenes previos del CMOCCC. Se realizaron entrevistas con los integrantes del Consejo, y se obtuvo información durante las actividades de desarrollo y análisis del examen. Se planeó e implementó un proceso de cambio de dos años para la mejoría de calidad del examen de certificación del CMOCCC, usando elmodelo de desarrollo de exámenes objetivos de Downing y el modelo de Greenhalgh para la diseminación de innovaciones.
